If the sha toxin is not completely cleared and remains stagnant between the muscles and couli (interstices), it will form a swelling and toxicity. One should first perform sha release therapy, using herbs that disperse sha and detoxify to eliminate the root cause. Then, examine where the toxin manifests, and according to the twelve meridians and zang-fu organs, differentiate yin and yang, cold and heat for treatment. For mild cases, use resolution; for severe cases, use expulsion/support; for deficiency, use tonification; for excess, use purgation. If redness and swelling are severe, it belongs to Yang syndrome; use Ren Dong Jie Du Tang (Honeysuckle Detoxification Decoction) with channel-directing herbs for treatment. If the color is white without redness, and the swelling is flat and difficult to erupt, it belongs to Yin syndrome; use Shen Gui Hua Du Tang (Ginseng and Angelica Toxin-Transforming Decoction) with channel-directing herbs to support expulsion. If the toxin is half-Yin and half-Yang, use Huo Luo Tou Du Yin (Collateral-Activating Toxin-Expelling Drink) with channel-directing herbs to penetrate and expel. After the abscess ruptures, apply Shen Xian Tai Yi Gao (Immortal Taiyi Plaster). If there is no pus but only toxic fluid oozing out, or if there is little pus and much blood, use Fei Long Duo Ming Dan (Flying Dragon Life-Seizing Pill), grind a small amount, mix into the Taiyi plaster, extract the toxic fluid and bloody pus, then apply the Taiyi plaster alone. If the wound is difficult to heal and close, sprinkle Hong Rou San (Red Flesh Powder). If the flesh is black, use Dai Dao San (Knife-Replacement Powder), lightly sprinkle with cotton wool, which will turn red, and apply the plaster for self-healing.
Clinical Cases:
Jiang Yunqu suffered from whole-body pain and developed a toxic sore on his back, which was black, ulcerated, and painful. Upon examination, his pulse was deep and faint, his fingertips were black, and he disliked hot drinks. This indicated that sha had transformed into malignant toxin, resulting in a carbuncle of the back due to the application of cold compresses. He was instructed to remove the cold compresses. After sha release was completed and the sha qi dissipated, he was treated internally with Ginseng, Zingiberis Rhizoma Recens (Dried Ginger), Cinnamomi Ramulus (Cinnamon Twig), and Aconiti Lateralis Radix Praeparata (Prepared Forked Root of Chinese Aconite) to warm and support the body. Externally, Dai Dao San was applied. The blackness turned red, and after applying Taiyi plaster, he recovered.
The eldest son of Su Cheng suffered from vomiting and diarrhea during the summer months, accompanied by colicky abdominal pain. After scraping therapy, the pain ceased, but his arms became red, swollen, and itchy. A prescription of Xiang Ru Yin (Elsholtzia Decoction) was administered for one dose, leading to recovery.
The wife of Ge Hongxian suffered from sha pain without vomiting or diarrhea, experiencing severe intestinal cramping and unbearable pain. After sha release, a toxic sore appeared on the top of her head, which healed after draining pus.
Zhao Gongyan experienced alternating chills and fever, dizziness, and mental restlessness. He recovered after scraping therapy. However, residual toxin manifested near the anus, forming a fistula with pus discharge, resulting in a lifelong ailment.
