Generally, phlegm-fire syndromes often manifest as left hypochondriac pain. If the pain is so severe that one cannot turn over in bed, it indicates exhaustion of the Liver channel, where its "leaves" have scorched; this condition is mostly incurable.
A patient suffered from severe right hypochondriac pain. Upon pulse diagnosis, the pulses in all six positions were slippery, indicating pain caused by phlegm flowing and obstructing. I prescribed:
Citri Reticulatae Pericarpium (Chenpi) (1 qian), Pinelliae Rhizoma Preparatum (Banxia, prepared) (7 fen), Cyperi Rhizoma (Xiangfu, stir-fried with child's urine) (5 fen), Arecae Pericarpium (Dafupi, washed) (5 fen), Atractylodis Rhizoma (Cangzhu, soaked) (5 fen), Magnoliae Officinalis Cortex (Noupu, stir-fried with ginger), Aurantii Fructus Immaturus (Zhiqiao) (3.5 fen), Curcumae Longae Rhizoma (Pianzi Jianghuang) (4 fen), Maltum (Shenqu, stir-fried) (5 fen), Hordeum Fructus Germinatus (Maiya, stir-fried) (5 fen).
After several doses, the patient recovered.
Jing (Startle) refers to the sudden palpitation and restlessness of the Heart. Ji (Palpitation) refers to heart palpitations accompanied by fear of being startled. Zhengchong (Severe Palpitation) refers to inner restlessness and anxiety, feeling terrified as if someone were about to capture them. These conditions often arise from excessive worry among the wealthy or unfulfilled desires among the poor, leading to Jianwang (Forgetfulness). This manifests as suddenly forgetting matters, striving hard to recall but failing, starting tasks without finishing them, and speaking without coherence. Although these three syndromes have different names, their pathology is identical, rooted in Blood deficiency of the Heart. Since the Heart lacks blood nourishment, it is like a fish out of water, jumping in terror. The symptoms come and go because phlegm moves with fire. Thin individuals are mostly deficient in Blood, while obese individuals mostly have Phlegm-fluid retention. The treatment method should first nourish Heart Blood and regulate Spleen Earth. One should also maintain a tranquil and joyful state of mind, restrain worries, and avoid the Seven Emotions and Six Exogenous Pathogens to ensure health. *Suwen* states: "The color of the East is green, entering through the Liver; its disease manifests as startle and horror." It also states: "When Heat transfers from the Spleen to the Liver, it causes startle and epistaxis." Zhang Zhongjing stated: "When food intake is low but fluid intake is high, water stagnates and fails to descend; severely, it causes palpitations; mildly, it causes shortness of breath." He also stated: "When five types of fluid retention accumulate and block the Middle Jiao, it most easily causes palpitations." He further stated: "When caused by shock from major events, it is called 'Heart Startle and Gallbladder Cold.' The disease location is in the Heart and Gallbladder channels; the pulse is large and trembling, beating like a bean, shaking without head or tail." Zhu Danxi stated: "When a disease originates from startle, the Spirit leaves its residence; when the residence loses fluids, phlegm forms; when Qi and Blood enter the residence, phlegm repels the Spirit, preventing it from returning."
The Yellow Emperor asked: "When the Foot-Yangming Stomach Channel is diseased, the patient dislikes people and fire, and is startled by the sound of wood, yet unmoved by bells and drums. Why?" Qibo replied: "The Yangming channel is the Stomach channel; the Stomach belongs to Earth. Therefore, hearing the sound of Wood causes startle, because Earth fears Wood." He also said: "Phlegm-fluid and startle-palpitations belong to Spleen-Earth pathology." After hemorrhage or night sweats caused by Fire diseases, this syndrome is frequently seen; therefore, it is appended here.
