Treatment for fever, irritability, thirst, chest tightness, and mental confusion caused by miasma toxins. Manifesting as external heat with internal cold, spontaneous diarrhea, vomiting and regurgitation, and cold extremities.
Radix Aconiti Lateralis Preparata (Da Fuzi, prepared, peel removed, umbilicus removed) Radix Ginseng (Ren Shen, radix removed) Radix Glycyrrhizae Preparata (Zhi Gan Cao) Rhizoma Zingiberis Preparatus (Pao Ganjiang) Rhizoma Atractylodis Macrocephalae (Bai Zhu, radix removed, soil removed, stir-fried).
Equal parts of the above ingredients, decocted in water, taken warm. If the patient desires cold drinks, take the decoction cold. Fever, irritability, and thirst in miasma disease are due to the failure of Heart and Lung Yang to descend. Spontaneous diarrhea, vomiting, and cold extremities are due to Spleen Earth lacking Yang and being deficient-cold; this formula applies. When Cold invades internally, treat with Pungent-Hot substances. The Pungent-Hot nature of Ganjiang and Fuzi treats internal cold. Vomiting and diarrhea indicate Spleen Qi deficiency; for Spleen deficiency, use the Sweet tonifying properties of Renshen, Baizhu, and Gancao. Da Fuzi can guide Upper Jiao Yang into the Middle Jiao, thereby reducing irritability and heat; when the Middle Jiao is warm, vomiting and diarrhea cease.
Treatment for Malaria (Zhang Ji). Symptoms include vomiting, abdominal distension, and pain. Use Renshen, Baizhu, Ganjiang, Gancao, Chenpi, and Qingpi. Equal parts of the above, decocted in water, taken hot, at any time.
Malaria often involves vomiting due to Spleen Earth deficiency-cold causing Phlegm-Qi to rebel upward. Therefore, Ganjiang's Pungent-Hot nature treats Cold, while Renshen, Baizhu, and Gancao's Sweet-Warm nature tonifies the Spleen. This is also a Lizhong-type formula, but Chenpi and Qingpi's Pungency disperses Qi; when Qi descends, Phlegm moves down and vomiting stops.
After several days of treating Miasma Disease, symptoms such as fever, abdominal pain, dizziness and heaviness of the head and eyes, limb pain, spontaneous diarrhea, urination either free or obstructed, vomiting, or coughing may occur.
Radix Aconiti Lateralis Preparata (Da Fuzi, prepared) Poria (Bai Fuling, peel removed) Rhizoma Atractylodis Macrocephalae (Bai Zhu, stir-fried) Radix Paeoniae Alba (Bai Shao, stir-fried until yellow).
Decoct the above herbs with five slices of Shengjiang in water, take warm before meals. If urination is free, remove Fuling. If stool is loose, remove Baishao and add Ganjiang. If vomiting occurs, add five slices of Shengjiang. *Yijian Fang* states: For those without diarrhea but with vomiting, remove Fuzi and add Shengjiang. However, removing Fuzi leaves only three ingredients, which seems too simple; one should adjust according to circumstances. Treatment methods should indeed be considered broadly. *Huoren Shu* states: In Taiyang disease, after inducing sweating, if the condition does not resolve and fever persists with palpitations, dizziness, muscle twitching, and trembling desire to fall, Zhenwu Tang is indicated. This implies that Taiyang wind-strike treated with Mahuang failed to resolve, leading to severe deficiency; thus, Baizhu, Fuzi, and Baishao are suitable. It also states: In Shaoyin disease, if unrelieved after two to three days, progressing to four to five days with abdominal pain, difficulty in urination, heavy and painful limbs, spontaneous diarrhea, vomiting, coughing, or free urination, this indicates Water Qi retention, and Zhenwu Tang is indicated. This is added here for reference, so practitioners do not limit their views to one aspect. Dizziness, heaviness of the head and eyes, and heavy, painful limbs in Miasma Disease are external signs of Cold-Damp injuring the Spleen. Abdominal pain, spontaneous diarrhea, and difficulty in urination are internal signs of Cold-Damp injuring the Spleen. The Spleen dislikes Dampness; excessive Dampness leads to loose stools. Therefore, Fuling and Baizhu's Sweetness tonifies the Spleen and removes Dampness. When Cold prevails, treat with Pungent-Hot. Fuzi and Shengjiang tonify the Spleen, benefit Yang Qi, and dispel Cold. Baishao consolidates Spleen Qi, relieves abdominal pain, retains Dampness to generate fluids, and promotes independent urination. However, it must be stir-fried with wine until yellow to remove its Cold nature. Named Zhenwu because Zhenwu is the deity of the North, capable of suppressing Northern Cold Water Qi to prevent disaster.
