Jinye (Body Fluids) and Xue (Blood) nourish the fetus, leading to a lack of moistening in the Fei Zang (Lung Organ), compounded by stagnant Fire ascending. Consequently, coughing persists indefinitely. The treatment principle should primarily focus on moistening the lungs.
Wei Huo (Stomach Fire) rebels upward affecting the lungs. Ingestion of cold/raw foods followed by spicy ingredients like Sichuan pepper (Huajiao) and ginger damages Tai Qi (Fetal Qi), causing persistent coughing. Use Wuhu Tang (Five Tiger Decoction). (Refer to "Harmony" Chapter, Formula 7.)
Pi Fei Qi Xu (Spleen and Lung Qi Deficiency) with invasion of Feng Han (Wind-Cold). Use Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction), adding Sang Bai Pi (Morus Alba Root Bark), Xing Ren (Prunus Armeniaca Seed), and Jie Geng (Platycodon Root). (Refer to "Tonify" Chapter, Formula 6.)
Fei Xu (Lung Deficiency) fails to generate Shui (Kidney Water/Yin), leading to Kidney Yin deficiency and hyperactive Fire. One should clear the Metal (Lungs) and nourish the Water (Kidneys), while prioritizing An Tai (Calming the Fetus).
Yin Huo (Deficient Fire) suppresses the Lung Metal. Use Tiandong Tang (Asparagus Decoction). (Refer to "Cold" Chapter, Formula 12.)
Feng Han Ke Fei (Wind-Cold lodging in the Lungs). Use Ningfei Yin (Lung-Calming Infusion). (Refer to "Cold" Chapter, Formula 13.)
Persistent coughing with blood. Use Qingfei Yin (Lung-Clearing Infusion). (Refer to "Cold" Chapter, Formula 27.)
The patient has a pre-existing Gan Huo (Liver Fire). During pregnancy, Yin Blood congregates to nourish the Tai Yuan (Fetal Origin), causing Liver Blood to become increasingly dry. This results in alternating chills and fever, resembling malaria but not being true Nue Ji (Malaria). Alternatively, it may be due to Qi and Blood deficiency, allowing Wind-Cold to exploit the weakness. Wind is a Yang pathogen that transforms into Heat; Cold is a Yin pathogen that transforms into Cold. With both Yin and Yang pathogens involved, chills and fever appear alternately. The treatment should greatly tonify Qi and Blood and mildly release exterior pathogens. If chills and fever persist, the steaming heat will damage the fetus.
Gan Ku Fa Huo (Liver Dryness generating Fire). Use Xiaoyao San (Free and Easy Wanderer Powder). (Refer to "Harmony" Chapter, Formula 5.)
Insufficient Hua Yuan (Source of Transformation/Spleen-Stomach digestion). Use Liuwei Wan (Six-Ingredient Pill). (Refer to "Tonify" Chapter, Formula 2.)
Qi Xu Han Re (Qi Deficiency with Chills and Fever). Use Liujunzi Tang (Six Gentlemen Decoction), removing Banxia (Pinellia Tuber), and adding Huoxiang (Patchouli Herb). (Refer to "Tonify" Chapter, Formula 1.)
Bu He De Yin Yang Zhi Qi (Disorder of Yin-Yang Qi). Harmonization is indicated; use Xiao Chaihu Tang (Minor Bupleurum Decoction). (Refer to "Harmony" Chapter, Formula 10.)
Zang Fu are already deficient; external contact with Wind-Cold causes Zheng Qi (Upright Qi) and Pathogenic factors to clash, moving up and down with Qi dynamics. If attacking the Xin (Heart), it causes Heart Pain; if attacking the Abdomen, it causes Abdominal Pain. If prolonged and untreated, attacking the Bao Tai (Fetus/Uterus) leads to miscarriage.
Abdominal pain with aversion to cold, lower abdomen feels like a fan opening (cold sensation). The Zi Gong (Uterus) is affected by Cold and cannot close, feeling open like a fan. Use Fuzi Tang (Aconite Decoction). (Refer to "Heat" Chapter, Formula 1.)
Xue Zhang Fu Tong (Blood Distension causing Abdominal Pain). This indicates long-term Zi Gong deficiency. Without prompt treatment, miscarriage occurs. Use Duzhong Wan (Eucommia Pill). (Refer to "Tonify" Chapter, Formula 10.)
Pain radiating to both hypochondria, frequent urination, loose stools. Use Antai Heqi Yin (Fetus-Calming and Qi-Harmonizing Infusion). (Refer to "Harmony" Chapter, Formula 11.)
Mild pain extending to the waist, loss of taste. This indicates Bu An Tai Qi (Unstable Fetal Qi). Use Antai Yin (Fetus-Calming Infusion). (Refer to "Tonify" Chapter, Formula 9.)
Intolerable pain, fetal movement threatening expulsion. For early-stage cases with robust constitution, boil several taels of fine silver first, then add Er Gen (Imperata Rhizome/Mao Gen) two jin, decoct the juice, and drink.
Qi distension causing stabbing pain, borborygmus, vomiting/reversal of Qi. Caused by excessive anger, worry, overthinking, and irregular diet. Use Muxiang San (Costus Powder). If Qi stagnation generates Fire, add herbs that clear Fire. (Refer to "Scatter" Chapter, Formula 9.)
Bao Zu Fu Tong (Womb Obstruction Abdominal Pain). Blood is obstructed by the Bao Gong (Womb) and cannot circulate freely. Blood stasis causes Qi stagnation, resulting in abdominal pain. Use Siwu Tang (Four Substances Decoction) plus Ejiao (Donkey-Hide Gelatin), Aye (Mugwort Leaf), and Gancao (Licorice Root). If complicated by Wind, turbid Phlegm forms. Ejiao dispels Wind and clarifies turbidity; Aye guides blood back to its vessels. (Refer to "Tonify" Chapter, Formula 7.)
Pi Wei Qi Xu (Spleen-Stomach Qi Deficiency) failing to support the fetus. Use Antai Yin plus Shengma (Cimicifuga Rhizome) and Baizhu (Atractylodes Macrocephala Rhizome), or Buzhong Yiqi Tang. (Refer to "Tonify" Chapter, Formula 9, 6.)
Sudden pain. Occurs in the fifth to sixth month of pregnancy. Roast fourteen large dates to charcoal, roast one qian of salt until red, grind together, and take with wine.
Trauma from falling causes fetal movement, abdominal pain, and bleeding. First try Siwu Tang plus Ejiao and Aye. If ineffective, use Bazhen Tang (Eight Treasures Decoction) plus Ejiao and Aye, or add Zhimu (Anemarrhena Rhizome). If palpitations, shortness of breath, abdominal fullness, and pain occur, use Siwu Tang plus Ejiao, Shanren (Amomum Fruit), Chenpi (Tangerine Peel), and Huangqin (Scutellaria Root). (Refer to "Tonify" Chapter, Formula 7, 5.)
Wind Pathogen and Tan Yin (Phlegm-Fluid) knotting in the collateral branches of the Heart. Choose one: Siwu Tang combined with Erchen Tang (Two Aged Ingredients Decoction) plus Huoxiang and Zisu (Perilla Leaf); or Ejiao San plus Xuanjinhua (Inula Flower); or Danggui Shaoyao San (Angelica and Peony Powder). (Refer to "Tonify" Chapter, Formula 7; "Harmony" Chapter, Formula 8; "Tonify" Chapter, Formula 63; "Tonify" Chapter, Formula 11.)
Abdominal pain radiating to the hips. Use Wujiapi San (Eleutherococcus Powder) or Wujiapi Wan (Eleutherococcus Pill). (Refer to "Scatter" Chapter, Formula 10, 11.)
Skin pain, tendon/bone stiffness and spasm, curled hands and feet. Caused by internal consumption of cold/raw foods and external exposure to Wind-Cold. Cold Qi clashes internally and externally, disturbing the fetus. Use Antai Heqi Yin. (Refer to "Harmony" Chapter, Formula 11.)
Bone/tendon pain, limb restlessness. Due to strange dreams, fright, noise, shouting, or palpitations, disturbing Fetal Qi. This causes distension in the hypochondria, urgent pain connecting to the umbilicus, restlessness in sitting/lying, and rapid breathing resembling asthma. Quickly administer Dasheng San (Great Sage Powder). (Refer to "Tonify" Chapter, Formula 12.)
Intermittent abdominal pain with heavy bearing-down sensation in the lower abdomen. Indicates Xue Xu Qi Xian (Blood Deficiency with Qi Sinking). Use Jiawei Antai Yin (Modified Fetus-Calming Infusion). (Refer to "Tonify" Chapter, Formula 13.)
Old Cold clashing with Blood, causing Blood coagulation and Qi stagnation. Use Wulingzhi (Trogopterus Dung) and Puhuang (Lygodium Spore), both roasted to charcoal, mixed with vinegar and taken. Or use Wujunzi Tang (Five Gentlemen Decoction). (Refer to "Tonify" Chapter, Formula 14.)
Whole-body pain, pain rushing to the heart feeling fatal, inability to eat. Use Baizhu (Atractylodes Rhizome), Shaoyao (Peony Root), and Huangqin (Scutellaria Root).
Fullness below the heart, urgent Qi and painful sensation. Use Chifuling (Red Poria), Sangbaipi (Mulberry Root Bark), Qianhu (Peucedanum Root), Yuliren (Apricot Kernel), Sugeng (Perilla Stem), and Wujiapi (Eleutherococcus Bark).
Blood Deficiency with Heat. Crush two jin of Shengdi (Rehmannia Root) for juice, mix with one sheng of wine, decoct to half volume, and take.
Tai Qi Zitong (Fetal Qi Self-Pain). Use Neibu Wan (Internal Tonification Pill). (Refer to "Tonify" Chapter, Formula 15.)
Bao Luo (Fetal Collaterals) deficiency with Wind-Cold attack, severe lower abdominal pain. Use Zisu Yin plus Shengjiang (Fresh Ginger). If Qi and Blood are both deficient, Bazhen Tang can be modified. (Refer to "Scatter" Chapter, Formula 3; "Tonify" Chapter, Formula 5.)
Qi attacking left and right sides, difficult urination. Use Xiao Chaihu Tang plus Qingpi (Green Tangerine Peel) and Zhizi (Gardenia Fruit). (Refer to "Harmony" Chapter, Formula 10.)
