The eyelid margin is inverted, presenting the most unsightly appearance, with tears and blood pooling upon the lid. If this sight were to startle a child, there would be no need to use both hands to forcibly pull the eyelids down. Combined with the previously mentioned unbearable ulceration of the eyelids, it is collectively termed "Can Feng" (Residual Wind), which was historically considered incurable.
This condition arises from the laxity and inversion of the lower eyelid's supporting structures, causing the inner palpebral conjunctiva to evert externally, resembling a manual turning inside out. This is caused by qi forcing blood stasis, compounded by wind-dampness pathogenic factors, leading to tendon traction and skin tightness, allowing downward eversion but preventing retraction. In severe cases, redness and ulceration occur, accompanied by excessive discharge (discharge) and tearing. The ophthalmology specialty calls this "Can Feng." It often affects individuals prone to phlegm-wind and alcohol consumption; historically, such patients were difficult to cure. If not caused by traumatic wind-storm injury, and without significant swelling or pain, internal regulation can be achieved with Qingwei San (Stomach-Clearing Powder), Xiehuang San (Yellow-Draining Powder), or Ganlu Yin (Sweet Dew Decoction), while external treatment involves scarification (Bian therapy) and washing. Treating both internally and externally is actually not difficult. However, if the eyelid contour is already deformed, although the pain and suffering are removed, the patient may feel ashamed when looking in the mirror, likely facing social stigma, rendering medical advice superfluous.
Yang Zhu wept at the crossroads because paths could lead north or south. Mozi grieved over dyed silk because it could be turned yellow or black. What injury befell the child, causing tears to soak his chest as he opened his robes? Like one crying out lovesickness, dark hair turning green [metaphor for extreme sorrow], or mourning parting, tears dropping incessantly. To express my feelings, I prepare a decoction of Ginseng Radix et Rhizoma (Renshen) and Poria (Fuling). As dew thins and autumn winds chill, pure joy remains distant and hard to attain.
This condition presents with normal eyelid contours internally and externally, yet the tears are thick and turbid like sour wine or soy milk, flowing continuously. Occasionally, the eyelids swell and close tightly; forcibly opening them stimulates a spray of tears. Contemporary physicians mistook this for pus, failing to identify its origin. Moreover, children suffer from this frequently, yet lacked effective treatments. Little did they know that clearing the lung and regulating the spleen makes treatment quite easy. When asked for the rationale, the answer is: The Lung is not inactive; it governs the descending directive. When food enters the stomach, the Spleen disperses essence upward to the Lung. If the Lung is disharmonious, it cannot regulate water passages and irrigate the body, causing fluid to overflow from high sources, infiltrate the skin, and cause swelling and dampness. Furthermore, with Liver Wood and Heart Fire rising, producing acidity, the water condenses and becomes turbid, hindering permeation and excretion. Consequently, following its associated pathways, it exits via the Qi Wheel (White Eye/Conjunctiva) as discharge and tears. Thus, the disease root lies not in the Liver, but in the Spleen; not in the Spleen, but in the Lung. If unresolved for a long time, both Spleen and Lung may become exhausted, failing to transport essence and transform qi, leading to the internal depletion of Shen Water (ocular fluid). While longevity might be preserved, vision will ultimately dim. Treatment for children involves Liu Jun Zi Tang (Six Gentlemen Decoction) added with Bupleuri Radix (Chaihu) and Paeoniae Radix Alba (Baishao). Subsequently, remove Chaihu and Baishao, add Ophiopogonis Radix (Maidong) and Schisandrae Fructus (Wuweizi); after a few doses, tearing stops immediately. For men and women, Baiju Qingjin San (White Chrysanthemum Metal-Clearing Powder) or Jiuxian Wan (Nine Immortals Pill) are used. If pulses are deficient, Liu Jun Zi Tang modified, or Buzhong Yiqi Tang (Middle-Wardening Qi-Supplementing Decoction) added with Aconiti Lateralis Radix Praeparata (Fuzi), Saposhnikoviae Radix (Fangfeng), Wuweizi, and Baishao is also excellent.
This condition is common in ophthalmology, yet no previous medical texts mention it. Why?
