Select clean Honeysuckle Flower (Jinyinhua, 5 liang), Licorice Root (Gancao, 2 liang), and pure wine (1 sheng). Heat them in a double boiler until the medicinal flavor is extracted. If the ulcer appears flat and hard, add Astragalus Root (Huangqi, 3 liang); if the wine amount is insufficient, add more as needed.
Administer this wine when large or small sores and toxins first appear. Honeysuckle Flower is cold to clear heat, sweet to treat deficiency, and fragrant to awaken the Spleen. Licorice Root resolves toxins and harmonizes the Middle Jiao; it can be used without fear in both deficiency and excess patterns. Both are considered sacred medicines for surgery. Boil one pot of medicinal wine and drink it slowly throughout day and night until the medicinal power reaches the disease site. Once the power arrives, decoct and administer the previous formulas sequentially; the disease may be instantly eliminated. Even those who cannot tolerate alcohol must drink to intoxication without refusal.
Beeswax (4 liang), White Winter Honey (2 liang), White Alum (Baimingfan, 2 liang), Amber (Hupo), Realgar (Xionghuang), and Pearl (Zhenzhu) (each 1 liang). If pearls are unavailable, Cinnabar (Zhusha) may be used instead.
First, grind the four herbs into an extremely fine powder. Melt the beeswax and honey in a copper spoon. Remove from fire and wait until slightly solidified, then mix in the powdered herbs evenly. Quickly roll the mixture into pills the size of Adzuki beans by hand. Take 30 pills per dose. For severe cases, take three doses daily to achieve efficacy.
For any carbuncle, furuncle, or injuries from venomous snakes or mad dogs, where the toxin is abundant and urgent, preventing external discharge and leading to internal attack, administer these pills first to protect the Heart. This ensures auspiciousness and no blame. The human heart is clear, empty, upright, and central; evil pathogens find it difficult to invade and dare not offend it. Therefore, if a carbuncle sinks inward, or due to snake/dog bites, toxic vapors attack the center, life will be endangered. In this formula, beeswax and honey are sweet, warm, moistening, and dry-damp-resolving; alum is sour and astringent, dispelling sweat, protecting membranes, and pushing toxins outward, taking the primary role. Additionally, Realgar has a pungent and fierce aroma, repelling evil and killing hundred toxins. Pearl and Amber have guiding properties, calming fire and stabilizing the soul. No matter how abundant or urgent the toxin, it cannot cause harm. Subsequently, prescribe according to symptoms, and calculate the days to determine the recovery period.
Poem: Honeysuckle brews Tusu Wine, adding Astragalus dissolves Yin toxins without trace; Ancient Heart-Protecting Wax-Alum Pill, Pearl-Amber-Realgar new formula is good; lacking Pearl, Cinnabar suffices, honey pills with gold coating are true living treasures.
Cinnamon Bark (Rougui), Angelica Sinensis (Danggui), Astragalus Root (Huangqi), Ginseng Root (Renshen), Honeysuckle Flower (Rendonghua) (each 1 liang), Ligusticum Wallichii (Chuanxiong), Myrrh (Moyao), Trichosanthes Root (Tianhuafen), Angelica Dahurica (Baizhi) (each 5 qian), Frankincense (Ruxiang), Platycodon Root (Jiegeng), Licorice Root (Gancao) (each 7 qian), Peony Root (Shaoyao), Saposhnikovia Root (Fangfeng) (each 3 qian).
This formula primarily treats malignant sores that have not formed pus and cannot dissipate, or have formed pus but cannot ulcerate.
There are many types of malignant sores, too numerous to list. Failure to dissipate or ulcerate is due to severe deficiency of Essence and Blood, unable to generate Qi to form pus. Do not purely use cooling medicines, which would make muscles icy cold and harder to heal. One must use sweet-warm tonifying and harmonizing agents to assist Qi and invigorate Blood, accelerating new tissue growth and pus formation, thus avoiding complications. In the above formula, Ginseng Root, Astragalus Root, Licorice Root, and Frankincense are Qi-tonifying herbs that also strengthen the Spleen and generate fluids. Angelica Sinensis, Cinnamon Bark, Myrrh, and Ligusticum Wallichii regulate Blood, further benefiting Ying (Nutritive) level and moving stasis. Honeysuckle Flower, Trichosanthes Root, and Platycodon Root specifically resolve toxins and drain pus; reduce dosage once pus is discharged. Angelica Dahurica, Saposhnikovia Root, and Peony Root sufficiently eliminate dampness and close wounds; discontinue once dampness is gone. Using Tusu Wine for administration reflects the principle that deficiency does not tire of tonification, and tonification does not dislike abundance. As the saying goes: A formula's value lies in its efficacy, not its quantity; a physician's skill lies in flexibility, not rigid adherence. This is what is meant.
Poem: Thousand-Gold Internal Support relies on Ginseng, Cinnamon, Licorice, Trichosanthes, Platycodon, Angelica Dahurica, and Angelica Sinensis; add Honeysuckle, Ligusticum, Peony, and Frankincense; beware Myrrh if the disease is hard to drive away.
The affliction of carbuncles and furuncles generally arises from desires damaging the True Yuan (Original Qi). When True Yuan is damaged, although the brow area may look fine, the body interior is empty. Various adverse pathogenic factors easily attract and invade. During such attraction, the impact is deeper than in others. Thus, toxicity manifests. Danxi stated that stagnation of Yin and Yang generates disease; the principle is not outside this. However, the term "stagnation" does not accurately describe the pathology; it should be said that Yin and Yang mutually burden each other, leading to accumulation and depression. If evil enters the Qi level, body fluids become thick and turbid, forming Phlegm and Fluid Retention; over time, they seep into the vessels, causing blood to stagnate; this is Yang burdening Yin. If evil depresses in the Blood level, meridians are blocked, overflowing or knotting; over time, they seep out of vessels, causing Qi to become chaotic; this is Yin burdening Yang. The magnitude and depth of toxicity depend on individual constitution and susceptibility. Initially, use Huoming Yin (Life-Saving Decoction), followed by Weisheng Tang (Health-Preserving Decoction). Wait for symptoms to clarify; observe if the top is high, root active, color red, swelling present, painful, skin thin and shiny, indicating impending ulceration. Upon ulceration, if pus is thick, fresh yellow, and odorless, administer Tuoli Xiaodu Yin (Support Interior and Resolve Toxin Decoction). Rotting flesh will naturally detach, and swelling will subside. If the top is flat, root scattered, color dark, non-hot, non-painful swelling, hard but no pus formation, or if after ulceration pain and swelling do not decrease, pus is thin and watery, new flesh does not grow, morphology is ugly, and odor is foul, use Qianjin Neituo San (Thousand-Gold Internal Support Powder). Then use Shiquang Rong Decoction or Bawu Huisheng Decoction to greatly tonify Qi and Blood, turning slight reversal into smooth progression, gradually incorporating resolution. Six out of ten cases may heal. To declare death or untreatability, letting patients fall into ruin, is a great loss of medical ethics.
Garlic-Separated Moxibustion Method: When a sore manifests, place a slice of garlic on the center of the lesion. Apply moxa cones, burning three Zhu (units of moxa consumption) before replacing. The number of applications is unrestricted. Stop when pain ceases upon application to a painful area, or when pain returns upon application to a painless area. If garlic is unavailable, sliced ginger may be used. For Yin-type toxins or diffuse swelling without a distinct head, apply wet paper to the surface; the first spot to dry indicates the focal point. Record this, apply another layer of paper; where it dries first again, regardless of whether it is one or multiple spots, moxibustion should be applied everywhere. After moxibustion, immediately administer medicine for assured efficacy. This is the first essential step in surgical dermatology.
The Classic states: He who knows the essentials needs only one word to conclude. Students should avoid these pitfalls.
In surgery, plasters are the priority, followed by decoctions, pills, and powders. The reasons are three: Convenience (Bian), Awakening (Xing), and Speed (Jie). Convenience means having materials readily available; when toxins first emerge, cool the accumulated heat, dispel wandering wind, allowing them to submerge and internally dissipate. Awakening means alerting the patient; when toxins appear, suppress their momentum, calmly observe the transformation, and facilitate expulsion. Speed means enabling quick action; when toxins are rampant, discern good and evil, prevent changes, and urgently extract them. If using convenient methods with inferior prescriptions, pain will not decrease, and redness/swelling will increase. If awakening without detailed investigation, disaster will intensify, and ulceration/putrefaction will be delayed indefinitely. If relying solely on speed without control, diseases will change in many ways, and deterioration will be unpredictable. Possessing all three allows for successful practice, making the formula renowned. Examples include Taiyi Plaster and Yuhong Plaster, recorded in various books and widely circulated. Examining their ingredients: Taiyi Plaster contains Rehmannia Root (Di Huang), Cinnamon Bark, Angelica Sinensis, Frankincense, and Myrrh to move Blood and stop pain; Angelica Dahurica, Scrophularia Root (Xuanshen), Rhubarb (Dahuang), Red Peony Root (Chishao), and Lead Oxide (Huandan) clear fire and disperse wind; lubricated with Sesame Oil, toxins transform from within. Yuhong Plaster contains Angelica Sinensis, Dragon's Blood (Xuejie), Lithospermum Root (Zicao), and Calomelas (Qingfen) to remove stasis and generate new tissue; Angelica Dahurica, Licorice Root, Beeswax, and Sesame Oil regulate skin and close wounds, naturally restoring healthy flesh. Other remedies like Wanling and Wanying, though numerous, ultimately revolve around these principles. My disciples wishing to master this path should thoroughly search and collect famous sayings and methods.
