The Tiankuo (White Sclera) originates as the Qi Wheel (the ocular region governed by the Lung Meridian); when its transmission function fails, it damages the Yuan Qi (Original Qi). People are accustomed to the phenomenon of "Fire Heaven" and regard it as trivial, but who would claim that "Snatching the Sun" is not shocking?
This condition arises without external causes in previously healthy individuals. Within one or two days, the Tiankuo (White Sclera) swells completely, appearing purple-blue in color and resembling a rotting pig's lung, which is frightening to behold. In severe cases, it may envelop the Fenglun (Black Cornea/Iris), obscuring the Jinjing (Pupil), hence the name "Fire Heaven Snatching the Sun." In reality, this poses no significant harm. It stems from the failure of transmission functions, causing internal fire to flare upward. The treatment requires cleansing the organs and meridians involved to allow pathogenic factors to exit downward. Administering Shengshu Dihuang Yin (Raw and Prepared Rehmannia Decoction) and Fusang Wan (Morus Pill) alternately morning and evening will gradually reduce swelling and restore whiteness to the tissue. If treated according to the protocols for cold damage with redness and heat, the treatment will miss the pathology, merely depleting the True Yuan (True Original Qi). Should the patient's vital fortune be exhausted (indicating critical illness or extreme constitutional deficiency), the damage will be akin to melting metal and destroying wood, far exceeding the harm of axes and swords. Clinicians must adhere strictly to these principles and not neglect them.
Worry and contemplation stagnate, damaging the Heart Spirit; anger and overwork deplete Liver Qi; irregular eating and drinking impair the Stomach and Spleen (Granary); excessive sexual desire empties Water and Fire (Kidney Yin and Yang); as Earth Qi (Spleen/Stomach) declines, Metal Qi (Lung) naturally becomes thin, making the body susceptible to Wind, Cold, and Summer-heat pathogens. This disease involves all five Zang organs; if the physician ensures no aggravation occurs, this constitutes cure.
In this condition, there is no major defect in the eye, but at three or two locations, membrane-like tissues appear, eroding from the Qi Wheel into the Fenglun (Cornea/Iris), thus termed "Flowing Metal Overwhelming Wood." Its appearance resembles Roupanjing (Pterygium invading the cornea), yet it is white and thin, with an unstable position. It is also commonly seen in women with depressive dispositions. The reason lies in the fact that although women are naturally gentle, they are competitive and prone to sorrow. Sorrow causes Qi to descend; when competitiveness is unfulfilled, sorrow transforms into resentment. Unexpressed resentment leads to stagnation; stagnation generates fire; excessive fire consumes essence, injuring both Metal (Lung) and Wood (Liver), resulting in this disease. Once established, the condition can be controlled but not eradicated. One must never recklessly apply hook-and-cut surgery, as this will only cause blindness.
Since the condition can be controlled: because the Fenglun is affected, there will inevitably be slight discharge and tearing, or visual blurring and discomfort. Combining Guipi Tang (Restore the Spleen Decoction), Liujunzi Tang (Six Gentlemen Decoction), and Shengmai San (Generate the Pulse Powder), doubling the dosage and forming pills for long-term administration will prevent further progression. Huanshao Dan (Return to Youth Pill) or Zhujing Wan (Stabilize Vision Pill) are also suitable options. It cannot be eradicated surgically because the pterygium-like flesh is clearly thin and attached to the contour. Simply hooking and clamping it allows immediate removal with a flying knife. Here, what is called pterygium is actually an opacity; what is called opacity is actually a membrane. Describing it as mild and harmless yet claiming acupuncture and herbs have no place to act is contradictory. The statement that "preventing aggravation constitutes cure," while seemingly absurd, reflects profound insight.
