The white eye (sclera) floats above the black eye (cornea/iris area), presenting a deficiency-type swelling that gradually rises. The shape is round or elongated, suddenly interrupted in the middle, with pathogenic factors and healthy qi confusing each other. Some may form pearl-like nodules, while others are harmless, resembling fish roe blisters; if the condition transmits involving the stirring of Wood and Fire, its subtle changes must be distinguished as carefully as autumn down.
In this syndrome, the eyes themselves suffer no pain, but only one or two areas of the Qi Wheel (white sclera region) exhibit deficiency-type stagnation and bulging, without redness or purpleness. The shapes vary from round to elongated, sometimes interrupted, vaguely resembling white blisters within a fish's belly. This arises from the decline and obstruction of Qi mechanism, allowing Cold-Dampness pathogens to take advantage of the deficiency. Treatment can employ Zhuyang Huoxue Decoction (Assisting Yang and Invigorating Blood Decoction) to support healthy qi, and Sijunzi Tang (Four Gentlemen Decoction) added with Sangbai Pi (Morus Alba Root Bark) and Maidong (Ophiopogon Tuber) to suppress pathogenic factors, leading to natural resolution and recovery. Otherwise, it will first transform into a watery red color, causing general eyeball distension and fullness; subsequently turning crimson-purple, leading to the emergence of vascular stasis with tearing, photophobia, and astringent pain, which would indeed be a great suffering. At this point, should one soothe the Liver? Clear the Lungs? Or let nature take its course? Occasionally, after treatment, a few solid pearl-like remnants remain for life; daring not to apply knife-cauterization, yet this is also harmless. *Yaohan* attributes this to accumulated heat in the Lung organ, treating it with cooling methods. However, sudden heat belongs to Fire; when manifesting in the eyes, it inevitably causes redness and pain. Now, since there is naturally no suffering, only a vague floating like a fish blister, yet calling it accumulated heat—if not being mad by wind and losing one's heart, how could one be so deluded as to this extent?
Behold the pines and bamboos, lush and verdant. Yet look at someone's Qingjing (black cornea), where red nebulae obscure it densely, distinguishing neither the Gan Palace nor the Xun Chamber, blending black and white into one, flesh and blood fused together, transmitting left and right, fearing it will become an intractable disease. People are startled upon seeing it, and do not recognize the patient. Treatment specializes in Bianzhen (stone needle therapy), with less danger and more auspicious outcomes.
At the onset of this syndrome, whether on the left or right, there is severe redness, swelling, and pain, with tears flowing like boiling water, avoiding sunlight and afraid to face the sun, resembling Baofeng Kere (Guest Heat from Violent Wind). If neglected, red vessels of varying sizes crisscross, penetrating through the Fenglun (wind wheel/black cornea). After another night, accompanied by headache and constipation, the red vessels suddenly enlarge, transforming into a Blood Obstruction (Xuezhang). The obstruction becomes solid forming a nebula, densely obscuring the Zhen-Xun contours. Forcibly opened for viewing, black and white are indistinguishable, seeing only a mass of flesh and blood, hence called "Blood Nebula Enveloping the Eye" (Xueyi Baojing). This syndrome is considered dangerous; initial treatment requires Juhua Tongsheng San (Chrysanthemum Reaching the Supreme Powder) or Qingdu Zhuyu Tang (Clear Toxins and Drive Out Stasis Decoction) brewed in large doses. After ingestion, use stone needles to guide/open, applying Jiangxue Dan (Crimson Snow Pill) and Feixiong Dan (Flying Bear Pill) alternately day and night. Once the situation stabilizes slightly, adjust and administer Fenhu San (Separate Pearl Powder), Bazheng San (Eight Rectifications Powder), or Xiaofeng Sanxue Tang (Eliminate Wind and Resolve Blood Decoction) according to modifications, naturally turning deterioration into improvement and danger into peace. If the physician lacks patience, or the patient fails to abstain from desires, although it may not necessarily lead to protrusion of the eyeball, the facial appearance is certainly marred.
