A patch here, a star there, obscuring the clear cornea. Tears flow down, accompanied by thread-like discharge. Worsening at night and improving in the morning, or affecting the right side then the left; which meridian does this primarily belong to? It is due to the stagnation of Wood (Liver) Qi and the soaring of Fire (Heart) Qi, with both contending. It can change rapidly; if not clearly diagnosed, the outcome is uncertain. Like fog shrouding flowers or clouds hiding the moon, it may persist for a lifetime.
This condition presents as fine granules on the cornea, varying in color from white to slightly yellow; they may be connected, clustered, scattered, raised simultaneously, or generated sequentially over time. Generally, this is caused by the disturbance of Wood and Fire Qi, and is also a common ophthalmic disease. If treated according to the principles of "Star and Moon Opacity Erosion" at the appropriate time, the clustered opacities will slowly disperse, and the scattered ones will vanish instantly. However, if the condition worsens day by day, merging into a single mass and intertwined with several red vessels (blood vessels), although it does not resemble the rapid changes seen in Flower Opacity White Depression or Purulent Corneal Ulcer, it inevitably leads to patient suffering and diagnostic difficulty for the physician. Patients are advised to maintain a淡泊 (indifferent/calm) and tranquil mindset, avoiding interference from Phlegm-Fire.
Ocular diseases manifest like sores, where Excess pathogens show distinct signs. Hyperactive Liver Qi causes excessive tearing; abundant Blood heat naturally leads to photophobia (aversion to light); Damp-Heat causes erosion of the orbital margins; Dry Wind results in the formation of granules resembling pepper seeds or millet grains. Head and Heart conditions have specific indications; lips and mouth reflect the state of Spleen/Stomach (Earth); below the waist involves the accumulation of the three Yin meridians; the six Yang meridians ascend to the top of the head. The root of the ear indicates Kidney dryness; nasal paturity tests Lung clarity (Metal). These symptoms eventually appear throughout the body, originating from the twelve meridians. One must carefully guard against the turbidity of Essence/Jing; fortunately, the main treatment principles are clear.
This section specifically addresses ocular diseases, discussing how sores appearing after eye diseases can be deceptive and variable. Lesions vary in position, generally located above, below, to the left, or right of the eyeball. This is because Ministerial Fire (Heart Fire) is constrained by pathogens; constraint leads to blood congestion; congestion causes muscle heat, thus manifesting on the skin. Over time, the congested blood transforms into fluid, and the constrained pathogens stagnate within the fluid to form pus, resulting in widespread damp erosion. Initial slight itching indicates Deficient pathogens; swelling, pain, redness, and heat indicate Excess pathogens; severely, chills and fever occur with reduced appetite. If not treated urgently, turbid Qi may penetrate into the eye and affect the lens/core. If only the eyelids are affected without other itching or pain, this is merely irritation from eye discharge and tears; the eyes will heal and dissipate naturally. Whenever sores appear, if one does not examine the local color and shape, determine their origin and progression, and identify whether they are caused by Heat, Dampness, Blood excess, or Wind excess, and instead uniformly prescribe pills such as Sanxian Dan or Wuhu Dan, treating only the branch (symptoms) and not the root (cause), this cannot be called medical knowledge. Treatment method: Collect tender mulberry leaves, honeysuckle flowers, and hibiscus roots, decoct into a strong soup, and wash/scrape the area thoroughly for a long time. Mix Babao Dan with rapeseed oil and apply topically. Internally administer Tuoli Xiaodu Yin or Renshen Baidu San. Persisting in this manner for half a month to a month allows for purification. Some argue these are sores; since toxins are visible, it seems like a severe recurrence. I reply: External toxins and internal toxins are distinct. Furthermore, this understanding is coherent though unconventional. Even if severe, it poses no harm; other cases follow this logic.
