After the wind pathogen passes, the patient's spirit is greatly damaged, and the Fenglun (cornea) suffers severe injury. The Fenglun is compromised, becoming as fragile as if it could be broken by a blow or a touch. Although the pupil remains round and active, with the coagulated fat (lipoid material) intact, and although scars exist, the visual light has not fallen (vision is preserved). Since the visual light has not fallen, one can merely make do; why worry excessively?
This condition presents with a black nodule protruding on the Fenglun (cornea), slightly wrapped around by peripheral skin-like nebula (fibrous tissue). This arises from conditions such as "violent wind guest heat" and "water deficiency with fire flaring up." Due to improper medical treatment, diseases such as coagulated fat (lipid keratitis), yellow fluid (hypopyon), and wood ulcer (corneal ulcer) erode and perforate the Qingjing (cornea/sclera complex). The black cornea protrudes from the rupture site, initially resembling a fly's head, then like a crab's eye, and in severe cases, elongating horizontally like a black bean. Hence the aforementioned names. If the lesion is soft and painless, and the Jinjing (pupil) is only deviated but not ruined, recovery can indeed be promised. Occasionally, scabs form like bean shells, healing only after the shell falls off. However, even with masterful skill, scars are ultimately unavoidable. If the lesion becomes sharp, hard, and tightly painful, and medication further errs, black and white will mix indistinguishably, and the crab-eye deformity cannot be resolved. Otherwise, it will inevitably rupture, with green-yellow colors overlapping. Will the eye not become blind?
Crab-eye deformity is originally caused by reckless and chaotic medical interventions forcing the condition. All methods of sweating, vomiting, and purging are unnecessary. One should select formulas that are harmonious and tonifying, adding Schisandra chinensis (Wuweizi), Ziziphus jujuba seed (Zaoren), and White Peony Root (Baishao) to slowly astringe and consolidate. Over time, it will naturally retract. If untreated, Wood and Fire are currently strong, and the pulse must be floating, wiry, and rapid. One must use formulas such as Yiqing Wan (Suppress Green Pill), Xieqing Wan (Drain Green Pill), Bazheng San (Eight-Normal Powder), and Zhuke Tang (Expel Guest Decoction) to cleanse the zang-fu organs, before proceeding to harmonization. Nourishment and careful regulation are indicated; nineteen out of twenty cases will suffer no harm.
Distinct red vessels resembling dragon-like threads emerge from Yin Hai (inner canthus/tear duct area) and enter Shui Chi (outer canthus/blood vessel network), crisscrossing in varying thicknesses, lengths, densities, and sparseness. There is a heavy, cloudy sensation rising slowly, tight pain, and dripping tears, resembling a white rainbow piercing the sun, similar to a red thread threading pearls. It is understood that Water is trapped and Metal lacks support, causing Wind to rage and Fire to intensify.
This condition involves red vessels and dragon-like threads crisscrossing, ascending the Qi Lun (sclera/conjunctiva) and wrapping around the Fenglun (cornea). It is most difficult to treat. This is because Water drains Metal's origin, leaving Wind-Wood dry and uncontrolled. Moreover, when Fire is excessive, Wood burns; when Wind prevails, Wood breaks. Even the resilience of pine and cypress cannot escape this fate, let alone the Liver and Gallbladder. Therefore, Wind and Fire combine, generating red vessels. Once red vessels appear, they cause diffuse corneal opacity and nebula, hot tears flow, and pain is tight. The world calls this phenomenon "White Rainbow Piercing the Sun," naming it based on its signs and shape. Vessels confined only to the Qi Lun, even if rough, tight, and accompanied by slight tearing and red dragon-like threads, are common ocular conditions and need not be worried about. Even if there is an obstruction on the Fenglun, physicians can study it themselves; no need to elaborate here. *Neijing* states: Red vessels moving from top to bottom indicate Taiyang disease; from bottom to top, Yangming disease; from outside to inside, Shaoyang disease; from inside to outside, Shaoyin disease. Taiyang disease should be treated with warming and dispersing; Yangming disease with purging and cooling; Shaoyang disease with harmonizing; Shaoyin disease with clearing. Understanding this makes the principles of generation and restriction easy to grasp, allowing treatment to be as accurate as a drum responding to a mallet. When encountering red filaments and dragon-like veins, which are inevitable in Wind-Fire eye diseases, their size, thickness, and position vary unpredictably, making it impossible to strictly divide them into upper/lower or inner/outer categories! Simply observe if the large vessel penetrates through the eyeball. In such cases, administer Daochi San (Guide Out the Red Powder) with added Coptis chinensis (Huanglian). If ineffective or if opacity increases, and it persists for a long time, this indicates unconstrained Wind-Heat, likely becoming a chronic stubborn disease. One must use Jiji Wan (Ji Ji Pill), Renshen Guben Wan (Ginseng Root-Solidifying Pill), and Baihe Gujin Tang (Lily Bulb Kidney-Solidifying Decoction), applying flexible and adaptive strategies, which should certainly be effective.
