The heaven has the sun and moon, representing the essence of Yin and Yang; humans have two eyes, gathering the essence of Zang-Fu organs. The convergence of all essences forms the eye. The essence of flesh constitutes the upper and lower eyelids, the essence of blood constitutes the inner and outer canthi, the essence of Qi manifests as the white part (sclera), the essence of tendons manifests as the black part (cornea and iris), and the essence of bone manifests as the pupil. The structures that constrain and envelop the essences of Qi, blood, tendons, and bones are connected superiorly to the brain. The origins of the eyeball must be clearly understood.
[Note] Just as there are the sun and moon in heaven, so there are two eyes in humans. The sun and moon in heaven are formed by the essence of Heaven's Yin and Yang; similarly, the two human eyes are formed by the essence of the five Zang and six Fu organs flowing upward into the eyes. Therefore, the convergence of all essences is called the eye. The essence of flesh appears as the upper and lower eyelids, the essence of blood appears as the inner and outer canthi, the essence of Qi appears as the white part, the essence of tendons appears as the black part, and the essence of bone appears as the pupil. The tissues that constrain and envelop the essences of Qi, blood, tendons, and bones connect superiorly to the brain; this point must not be ignored. This is the origin of the eyeball.
The Five Wheels correspond to Flesh, Blood, Qi, Wind, and Water: the upper and lower eyelids constitute the Flesh Wheel, the inner and outer canthi constitute the Blood Wheel, the white part constitutes the Qi Wheel, the black part constitutes the Wind Wheel, and the pupil constitutes the Water Wheel. These should be known.
[Note] The Five Wheels refer to the Flesh Wheel, Blood Wheel, Qi Wheel, Wind Wheel, and Water Wheel. They are termed "Wheels" because the movement of the eyeball resembles a rotating wheel. The upper and lower eyelids are the Flesh Wheel, the inner and outer canthi are the Blood Wheel, the white part is the Qi Wheel, the black part is the Wind Wheel, and the pupil is the Water Wheel. This clarifies the locations of the Five Wheels and their correspondence to the five Zang organs.
Flesh Wheel — corresponds to the Spleen, governing flesh.
Blood Wheel — corresponds to the Heart, governing blood.
Qi Wheel — corresponds to the Lung, governing Qi.
Wind Wheel — corresponds to the Liver, governing tendons.
Water Wheel — corresponds to the Kidney, governing bone.
Diseases of the eyelids primarily involve Spleen disorders; diseases of the inner and outer canthi primarily involve Heart disorders; diseases of the white part primarily involve Lung disorders; diseases of the black part primarily involve Liver disorders; diseases of the pupil primarily involve Kidney disorders. Diseases of the Five Wheels are primarily governed by the five Zang organs. Their Cold, Heat, Deficiency, and Excess patterns should be differentiated according to the manifested symptoms.
[Note] The eyelids constitute the Flesh Wheel, which governs Spleen disorders. The inner and outer canthi constitute the Blood Wheel, which governs Heart disorders. The white part constitutes the Qi Wheel, which governs Lung disorders. The black part constitutes the Wind Wheel, which governs Liver disorders. The pupil constitutes the Water Wheel, which governs Kidney disorders. Disorders of the Five Wheels are dominated by the five Zang organs. The Cold, Heat, Deficiency, and Excess conditions should be distinguished based on the specific clinical manifestations.
