Lei Gong Zhen Jun said: Heatstroke leading to the collapse of Yang Qi, with incessant sweating and immediate Qi exhaustion, is a fatal condition. This is because when Yang Qi collapses, all Yang Qi is lost through sweat, leading to death due to Qi exhaustion. The treatment method should urgently tonify Yang Qi, so that Yang Qi connects with Yin Qi, avoiding the danger of sudden collapse. Using Dushen Tang (Single Ginseng Decoction) is excellent, but how can poor families obtain ginseng? It is better to use Danggui Buxue Tang (Angelica Sinensis Decoction for Tonifying Blood). Use Dang Gui (Dong Quai Root) one liang; Huang Qi (Astragalus Root) two liang, add thirty leaves of Sang Ye (Mulberry Leaf) to save the patient. These two herbs are inexpensive, and their efficacy is not inferior to ginseng. Moreover, Sang Ye has the function of nourishing Yin; without Yin, Yang cannot be transformed. Huang Qi tonifies Qi; combined with Dang Gui, it tonifies Blood; combined with Sang Ye, it generates Yin even more effectively.
Heatstroke causing mania, panting, and profuse sweating like rain, resembling the loss of spirit and mental control, is also a death sentence within moments. This is because extreme heat lacks water to nourish the spirit, causing internal burning, forcing sweat outwards, resulting in both Yang collapse and spirit loss. Urgent rescue with Baihu Tang (White Tiger Decoction) is required. However, a small dose is ineffective; one must use Shi Gao (Gypsum Fibrosum) four liang, Ren Shen (Ginseng Root) four liang, add Huang Lian (Coptis Rhizome) three qian, decoct in water and take orally. After one dose, the spirit settles; after two doses, sweating stops. Some may doubt: if there is no water in the heart, why is there sweat? One does not know that patients with mania invariably have thirst. Thirst compels them to drink water to save themselves. Water entering the abdomen does not circulate in the Heart but moves to the Spleen. The Spleen then transports it to the Lung. Since the Lung governs the skin and hair, the fluid is excreted externally. Thus, this sweat is external water, not internal body fluids. Furthermore, as sweat is excreted externally, Yang Qi also exits with it. When Yang exits, the Yin Qi in the Heart also dissipates and is lost, hence the loss of spirit and mental control. I use Huang Lian to calm Heart Fire, Shi Gao to clear Stomach Fire, and add large amounts of Ren Shen to rescue collapsing Yang Qi. In this way, fire dissipates while righteous Qi remains alone; spirit is preserved, and external pathogens are defeated.
Heatstroke manifesting as grasping at clothes and bed edges (xun yi mo chuang), and picking at empty air (cuo kong), is originally a fatal condition. However, it can be rescued by clearing the heat in the Heart caused by summer-heat. Once the Heart heat is resolved, the Five Viscera regain vitality. The prescription is Dushen Tang (Single Ginseng Decoction) three liang, added with Huang Lian (Coptis Rhizome) three qian, administered via灌 (gavage). If symptoms like grasbing at clothes and picking at air cease suddenly, the patient will live. This is because Ren Shen rescues the perishing Heart Qi, while Huang Lian disperses Heart Fire. With fire dispersed and Qi returning, survival is certain.
Heatstroke causing sudden collapse and severe heart pain approaching death is an incurable condition. Summer-heat is extremely hot, and the Heart is the palace of Fire. How can fire invade fire to cause such severe heart pain? One must understand that Heart Fire is Ministerial Fire (Jun Huo); Summer-heat Fire is Pathogenic Fire (Xie Huo). Pathogenic Fire harassing the Heart is no different from pathogenic water flooding the Heart. Therefore, cold or summer-heat qi, if it does not invade, is fine; but once it invades, sudden heart pain invariably occurs. The Heart Monarch is extremely still, separated by the Dan Zhong (Pericardium). What invades the Heart actually invades the Dan Zhong. When pathogens invade the Dan Zhong, sudden heart pain occurs. At this moment, using heat-resolving medicines directly introduced into the Dan Zhong will disperse summer-heat and retreat fire, leaving the Heart Monarch tranquil. The prescription is San Shu Jiu Xin Tang (Dispersing Summer-heat Rescuing Heart Decoction): Qing Hao (Artemisia Annua Herb) one liang, Huang Lian (Coptis Rhizome) three qian, Ren Shen (Ginseng Root) three qian, Fu Shen (Poria with Hostwood) five qian, Bai Zhu (Atractylodes Macrocephala Rhizome) three qian, Xiang Ru (Elsholtzia Herb) one qian, Huo Xiang (Patchouli Herb) five qian, Ban Xia (Pinellia Tuber) one qian. Decoct in water and take orally. Pain ceases after one dose. The miraculous effect of this formula lies in the use of Qing Hao, which enters the Dan Zhong directly, driving away formless summer-heat, thus stopping pain instantly.
Sudden collapse in heatstroke, foaming at the mouth, impending mania, body burning like fire, and obvious purple macules indicate a condition mostly incurable. However, I believe there is a sliver of hope precisely because the macules have erupted. If they do not erupt, fire-toxin remains internal, inevitably attacking the Heart and causing death. Now, although the macules have erupted, their pure purple color indicates excessive toxin, making resolution difficult. The prescription is Jiu Ban Zai Su Tang (Rescuing Macules Restoring Life Decoction): Xuan Shen (Scrophularia Root) three liang, Sheng Ma (Cimicifuga Rhizome) three qian, Jing Jie (Schizonepeta Spike) three qian, Huang Lian (Coptis Rhizome) three qian, Huang Qin (Scutellaria Root) three qian, Mai Dong (Ophiopogon Tuber) three liang, Tian Dong (Asparagus Tuber) one liang, Qing Hao (Artemisia Annua Herb) one liang. Decoct in water and take orally. After one dose, macule color turns red; after two doses, red fades; after three doses, macules disappear completely, signaling recovery. Otherwise, death is inevitable.
Summer heatstroke leading to Huoluan (Cholera-like syndrome with vomiting and diarrhea), where one wants to vomit but cannot, and cannot bear not vomiting, is an urgent condition. Xiang Ru Yin (Elsholtia Decoction) can save life. However, if the patient rejects the medicine, vomiting immediately upon ingestion, it is extremely critical. The treatment method should follow the disease's trend. I have a miraculous formula named Zhuan Zhi Tang (Turning Treatment Decoction). Bai Zhu (Atractylodes Macrocephala Rhizome) three qian, Fu Ling (Poria) three qian, Shao Yao (Peony Root) five qian, Huo Xiang (Patchouli Herb) one qian, Zi Su (Perilla Leaf) five fen, Chen Pi (Citrus Reticulata Peel) five fen, Tian Hua Fen (Trichosanthes Root) one qian, Rou Gui (Cinnamon Bark) five fen, Xiang Ru (Elsholtzia Herb) five fen, Bai Dou Kou (Black Cardamom Seed) one grain. Decoct in water, cool, and take orally. The medicine is retained upon swallowing, and Huoluan stabilizes. The brilliance of this formula lies in using Shao Yao as the sovereign herb, assisted by Bai Zhu and Fu Ling, thereby calming Liver Qi. If Liver Qi does not overact on Spleen Earth, Huoluan resolves naturally. Moreover, it contains heat-resolving herbs. Especially brilliant is the use of warming herbs like Rou Gui, Xiang Ru, and Huo Xiang to comply with summer-heat qi, guiding pathogens downward, and secretly resolving chaos. This possesses divine and unpredictable mechanisms applied within the dosage.
Huoluan with abdominal pain, inability to vomit or defecate, cold limbs, cyanosis, and retracted testicles (Nang Suo) is a fatal condition. Why should I prescribe further? However, as long as the person is not dead, how can we abandon them to die? This condition involves lower deficiency-cold and upper summer-heat invasion, causing chaotic Yin-Yang and disconnection between upper and lower. It is the most critical and urgent state. The method is to induce vomiting with Yin-Yang Water. If ineffective, urgently treat with Jiu Luan Tang (Rescuing Chaos Decoction). Ren Shen (Ginseng Root) five qian, Xiang Ru (Elsholtzia Herb) three qian, Wu Zhu Yu (Evodia Fruit) three qian, Fu Ling (Poria) three qian, Bai Zhu (Atractylodes Macrocephala Rhizome) three qian, Fu Zi (Aconite Lateral Root) five fen, Huo Xiang (Patchouli Herb) one qian, Mu Gua (Chaenomeles Fruit) three qian. Decoct in water and take orally. Qi returns immediately upon swallowing. This is truly a divine formula for treating Gan Huoluan (Dry Cholera). For Shi Huoluan (Wet Cholera), this formula is contraindicated. Instead, use Bai Zhu (Atractylodes Macrocephala Rhizome) five qian, Xiang Ru (Elsholtzia Herb) one qian, Qing Hao (Artemisia Annua Herb) five qian, Fu Ling (Poria) five qian, Chen Pi (Citrus Reticulata Peel) one qian, Sha Ren (Amomum Fruit) three grains. One dose brings spring-like recovery.
Postpartum sudden heatstroke with Huoluan vomiting and diarrhea is generally considered incurable. However, some can be saved with medication. Crucially, avoid Xiang Ru. Use Xiao Shu Huo Chan Dan (Clearing Heat Rescuing Birth Pill): Ren Shen (Ginseng Root) one liang, Dang Gui (Angelica Sinensis Root) two liang, Chuan Xiong (Ligusticum Rhizome) one liang, Rou Gui (Cinnamon Bark) two qian, Qing Hao (Artemisia Annua Herb) one qian. Decoct in water and take orally. Cured after one dose. Postpartum women only need blood and Qi tonification; once blood and Qi return, summer-heat dissipates naturally. Moreover, the formula contains cold-dispelling and heat-resolving ingredients, achieving unique efficacy. Some may question: since heatstroke is heat, why use Rou Gui (Cinnamon Bark)? Postpartum women suffer severe blood and Qi deficiency and are cold throughout the body. Upon feeling summer-heat, there is conflict, but not deep penetration of intense heat. It is merely slight summer-heat clashing with inherent cold, causing Huoluan. Here, Rou Gui warms deficiency-cold, and Qing Hao resolves mild summer-heat. Used within a large dose of blood and Qi tonics, it is controlled and prevents chaos. This is the brilliance of the prescription and its miraculous achievement. Why doubt it?
In summer, when heat is scorching, humans contracting summer-evil with sudden vomiting, diarrhea, and immediate death face the most terrifying condition. Do not lightly use Xiang Ru Yin (Elsholtia Decoction), nor recklessly use Baihu Tang (White Tiger Decoction). I have a formula named Re Xiao Shu San (Heat-Resolving Summer-heat Powder). Qing Hao (Artemisia Annua Herb) one liang, Ge Gen (Kudzu Root) one qian, Xiang Ru (Elsholtzia Herb) one qian, Fu Ling (Poria) one liang, Bai Zhu (Atractylodes Macrocephala Rhizome) three qian, Bai Bian Dou (Hyacinth Bean) two qian, Chen Pi (Citrus Reticulata Peel) one qian. Treatment brings peace. The formula's brilliance lies in using Qing Hao and Fu Ling as sovereign herbs. Qing Hao best resolves summer-heat and clears heat, serving dual purposes: guiding summer-heat entirely out through the Bladder. Ge Gen and Xiang Ru merely assist Qing Hao in dispelling summer-heat. Especially brilliant is the minimal use of Bai Zhu to strengthen Spleen and Stomach Qi, ensuring summer-heat retreats without damaging Stomach Qi. This far surpasses Xiang Ru Yin.
