On Mania with Hallucinations of Ghosts, On Mania without Hallucinations of Ghosts, On Stroke Leading to Falling, On Sudden Collapse and Loss of Consciousness.
The Heavenly Master said: When a person suffers from illness manifesting as mania with hallucinations of ghosts, or falls down and loses consciousness, or suffers from stroke with aphasia, or lies down but falls off the bed, these are all caused by deficiency of Zheng Qi (Upright Qi) allowing Xie Qi (Pathogenic Factors) to invade. Superficially, it seems correct to treat the pathogen directly; however, where the pathogen gathers, there must be deficiency of Qi. If one does not treat the deficiency, how can one attend to other matters? This is why it is urgent to consolidate Zheng Qi (Upright Qi), supplemented by small amounts of herbs that resolve Phlegm and dispel Pathogens, which is the subtle approach. For example, mania with hallucinations of ghosts is due to deficiency. The prescription uses Ren Shen (Ginseng Radix) 1 liang, Bai Zhu (Atractylodis Macrocephalae Rhizoma) 1 liang, Ban Xia (Pinelliae Tuber) 3 qian, Tian Nan Xing (Arisaematis Rhizoma Preparatum) 3 qian, and Fu Zi (Aconiti Lateralis Radix Praeparata) 1 qian. Administered in a large dose as a decoction, the mania will naturally subside. (Comment: This is the divine pill formula for dispelling mania, exquisite.) If the patient falls and loses consciousness, this is due to Qi deficiency, and the same prescription should be used. If the patient has stroke with aphasia, use Ren Shen (Ginseng Radix) 1 liang, Tian Nan Xing (Arisaematis Rhizoma Preparatum) 3 qian, raw Ban Xia (Pinelliae Tuber) 3 qian, and one raw Fu Zi (Aconiti Lateralis Radix), named San Sheng Yin (Three Lives Decoction). Administer urgently. Those who fall off the bed while lying down suffer from a condition similar to stroke, also known as Shi Jue (Corpse Faintness/Coma), and should also be saved with San Sheng Yin (Three Lives Decoction).
Mania with loss of consciousness but without hallucinations of ghosts is due to Heat. One must not use the previous decoction. The presence of hallucinations indicates deficiency, not Excess Heat. The prescription uses Ren Shen (Ginseng Radix) incorporated into herbs that resolve Phlegm and dispel Pathogens. This employs Fan Zhi Fa (Contrary Treatment), treating the disease by using methods contrary to its apparent nature.
Falling and loss of consciousness, although caused by Qi deficiency, never occur without Phlegm. Once fallen, consciousness is invariably lost. Therefore, resolving Phlegm is necessary, assisted by herbs that support Zheng Qi (Upright Qi). This explains why the previous prescription can treat these conditions concurrently.
Stroke and falling are purely due to Qi deficiency. Those with Qi deficiency inevitably generate Phlegm. Heavy accumulation of Phlegm leads to sudden stroke and collapse, which has its origins. Thus, treating only the Phlegm without tonifying Qi is tantamount to killing the patient. The subtlety of San Sheng Yin (Three Lives Decoction) lies in using raw Ren Shen (Ginseng Radix) 1 liang, combined with raw Fu Zi (Aconiti Lateralis Radix), Ban Xia (Pinelliae Tuber), and Tian Nan Xing (Arisaematis Rhizoma Preparatum)—herbs that dispel pathogens and cleanse the body—to drive them against the disease. It is like a general taking command, leading brave soldiers to sweep away demons; surely, life can be saved amidst death. If one merely treats the pathogen directly without addressing the root through contrary treatment, nine out of ten will die, and vengeful spirits will wail at night. Whose fault is this? (Comment: Pure.)
Master Zhang said: Mania with hallucinations of ghosts is clearly deficiency complicated by Phlegm. Using Ban Xia (Pinelliae Tuber), Tian Nan Xing (Arisaematis Rhizoma Preparatum), and Fu Zi (Aconiti Lateralis Radix) to resolve Phlegm requires substantial doses of Ren Shen (Ginseng Radix) and Bai Zhu (Atractylodis Macrocephalae Rhizoma) to guide the medicinal power to success. The subtlety of this prescription is not only in its efficacy for mania with hallucinations but also in its miraculous effect on stroke with aphasia and sudden collapse with loss of consciousness, because Phlegm arises after Qi deficiency. The Heavenly Master Qi analyzed this with great precision and further cited San Sheng Yin (Three Lives Decoction) for stroke. In fact, except for mania without hallucinations where this prescription is contraindicated, it is applicable to all other conditions, so there is no need to use San Sheng Yin (Three Lives Decoction) separately. However, San Sheng Yin (Three Lives Decoction) is also an extraordinary formula and can be adopted. In summary, by weighing between these two prescriptions, one can save the dying.
Mania without hallucinations of ghosts is clearly an internal Heat syndrome. The Heavenly Master Qi did not prescribe a formula, waiting for me to supplement it. The prescription uses Ren Shen (Ginseng Radix) 3 qian, Bai Shao (Paeoniae Radix Alba) 3 qian, Bai Jie Zi (Sinapis Semen) 3 qian, Ban Xia (Pinelliae Tuber) 3 qian, Tian Nan Xing (Arisaematis Rhizoma Preparatum) 2 qian, Huang Lian (Coptidis Rhizoma) 2 qian, Chen Pi (Citri Reticulatae Pericarpium) 1 qian, and Gan Cao (Glycyrrhizae Radix et Rhizoma) 1 qian. Decoct in water and take orally. The subtlety of this prescription lies in the use of Huang Lian (Coptidis Rhizoma). As the depth of Jue Ni (Reversal Cold/Collapse) increases, the heat also deepens; removing the heat causes the Jue Ni (Reversal Cold/Collapse) to settle naturally. Huang Lian (Coptidis Rhizoma) enters the Heart channel, guiding various Heart-tonifying ingredients to assist each other, either tonifying or draining. It is like neighbors arguing in a room, where relatives and friends intervene to mediate; naturally, anger subsides and repentance arises. The use of Huang Lian (Coptidis Rhizoma) within a tonifying formula embodies this principle.
Master Hua said: This text was transmitted to me, containing no other formulas. I have a few words to add. For stroke and similar conditions, if one does not add substantial amounts of Ren Shen (Ginseng Radix) to drive out pathogens, Phlegm cannot be resolved, and pathogens cannot be dispersed. The subtlety of the formula lies in using Ren Shen (Ginseng Radix) up to 1 liang to gain sufficient strength. Otherwise, a small dose would be manipulated by Phlegm and pathogens, and how could it assist in restraining Fu Zi (Aconiti Lateralis Radix Praeparata) to directly sweep away the demonic pathogens?
Lord Lei said: Extremely subtle. Each has expounded upon this without omission, leaving nothing more to discuss.
Truly the words of a Sage. (Recorded by Li Ziyong.)
