Lei Gong Zhen Jun (Deified Lord Lei Gong) said: Mania (Kuang Bing, a syndrome characterized by manic behavior and mental confusion) can arise from Taiyang disease (Taiyang disorder), which is a temporary state of mania. It should be treated according to the methods in Zhang Zhongjing’s *Treatise on Cold Pathogenic Diseases* (*Shanghan Lun*), using Baihu Tang (White Tiger Decoction) to clear and drain fire heat. There are cases of chronic mania that persist throughout the year without healing. Patients may wish to take a knife to kill others, or go to the magistrate to curse loudly; they do not recognize relatives, nor do they know their children; they feel great joy when seeing water, but extreme anger when seeing food. This is due to deficiency of Heart Qi (vital energy of the heart), allowing pathogenic heat to invade and phlegm-qi to encroach, thereby forming mania. In such cases, one wishes to drain fire, but fire located within the Heart cannot be directly drained; one wishes to resolve phlegm, but phlegm located within the Heart is difficult to resolve. The only approach is to tonify the Qi of the Spleen and Stomach, so that the Heart naturally receives nourishment. There is no need to forcibly dispel phlegm; it will transform itself. There is no need to forcibly drain fire; it will disappear naturally.
The prescription is called Hua Kuang Dan (Mania-Transforming Cinnabar Pill). Ren Shen (Ginseng Radix) 1 liang, Bai Zhu (Atractylodis Rhizoma) 1 liang, Gan Cao (Licorice Radix et Rhizoma) 1 qian; Fu Shen (Poria with Hostwood) 1 liang, Fu Zi (Aconiti Lateralis Radix Praeparata) 1 fen, Ban Xia (Pinelliae Tuber) 3 qian, Chang Pu (Acori Tatarinowii Rhizoma) 1 qian, Tu Si Zi (Cuscutae Semen) 3 qian. Decoct in water and take orally. After one dose, mania subsides; after two doses, the illness is cured. The brilliance of this formula lies in tonifying the three meridians of the Heart, Spleen, and Stomach, thereby transforming phlegm, without resorting to draining fire. For if fire is drained, Heart Qi is further injured, and saliva/phlegm becomes more abundant, how then can mania cease? Particularly ingenious is the use of 1 fen of Fu Zi (Prepared Aconite Root) to guide the Heart-tonifying and phlegm-resolving agents directly into the Heart center. Thus, Qi is easier to tonify, and phlegm is easier to resolve, rendering the excessive measure of draining fire unnecessary. This is why its efficacy is like magic.
Lei Gong Zhen Jun said: Dullness disease (Dai Bing, a syndrome characterized by apathy and cognitive dullness) presents as if possessed by a spirit, with silent muteness; it feels like hunger yet vague and lost, as if something is missing; the mind desires to become epileptic but cannot, desires to become manic but dares not. Sometimes sleeping for several days without waking, sometimes sitting for several days without sleeping. Sometimes meticulously sewing one’s own clothes, sometimes deeply hiding other people’s objects. When speaking with others, one has no words and the spirit wanders; when speaking behind others’ backs, one whispers and weeps. When offered food, one despises it and refuses to swallow; when denied food, one swallows charcoal as if it were delightful. Although these symptoms appear to be caused by demonic possession, in reality, the chest and abdomen are filled with nothing but phlegm-qi obstruction. Therefore, there is no extraordinary method for treating dullness; treating phlegm is treating dullness. However, when phlegm accumulation is most severe and dullness is deepest, ordinary Er Chen Tang (Two-Aged Decoction) will certainly not yield results. The prescription used is Zhu Dai Xian Dan (Phlegm-Expelling Immortal Cinnabar Pill): Ren Shen (Ginseng Radix) 1 liang, Bai Zhu (Atractylodis Rhizoma) 2 liang, Fu Shen (Poria with Hostwood) 3 liang, Ban Xia (Pinelliae Tuber) 5 qian, Bai Jie Zi (Sinapis Semen) 1 liang, Fu Zi (Aconiti Lateralis Radix Praeparata) 5 fen, Bai Wei (Cynanchi Atrati Radix) 3 qian, Tu Si Zi (Cuscutae Semen) 1 liang, Dan Sha (Cinnabaris) 3 qian. All herbs are ground into powder. First, decoct the herbs in water, mix with the Dan Sha (Cinnabar) powder, and prepare half a bowl. If the patient refuses to take it, burn the medicine until charred (tan zhi); the patient will then willingly take it. Burn more and administer another half bowl, then let them decide freely. The patient will inevitably feel fatigued and desire sleep. While they are asleep, burn all their clothes and bedding, leaving only the clothes currently worn on their body, cover them with a new quilt, and do not wake them abruptly. This sleep may last several days. Upon waking, they will search for clothes but find none, search for quilts but find they are not the original ones, and will inevitably cry bitterly. Then, administer the previous dose of medicine again. They will surely refuse, even if charred. Do not hesitate to whip them to provoke anger. Have strong individuals forcibly hold and pour the medicine into their mouths. They will become furious, then fall asleep again. At this time, fresh clothes and bedding must be prepared. Wait until they wake up after half a day. Seeing that the room is full of relatives, they will have a sudden realization, likely crying uncontrollably. Everyone should comfort them with kind words. They will inevitably speak of ghosts and spirits. Relatives should say, "Fortunately, Dr. [Name] treated you, and all ghosts and spirits have been expelled. You need not worry." Hearing this, they will accept it gladly, and the illness will be completely cured. The brilliance of this formula lies in greatly tonifying the Heart and Spleen. Using Fu Shen (Poria with Hostwood) as the sovereign herb, it expels phlegm from the Heart. The remaining phlegm-resolving herbs, guided by Fu Zi (Prepared Aconite Root), penetrate every meridian, driving phlegm from the entire body into the Bladder (Pang Guang) for digestion and elimination. Bai Wei (Cynanchi Atrati Radix) and Tu Si Zi (Cuscutae Semen) are excellent calming herbs, while Dan Sha (Cinnabar) stabilizes the soul and calms the spirit, achieving remarkable effects. Hence, its efficacy is achieved through this mechanism.
