One testis hangs lower. Or both are swollen. Or one testis retracts into the lower abdomen, with unbearable pain. It can only return to its original position after manual pressing and manipulation. This is *Shan Qi* (Hernia, specifically referring to cold-stagnation qi disorders such as Xiao Chang Qi). It is suitable to swallow Zhuyu Neixiao Wan (Calamus Internal-Resolving Pill) with Pancong San (Allium Scallion Powder). If not cured, use Bichengqie San (Cubeb Powder). Administer Dahuixiang Wan (Fennel Pill).
If bowel movements and urination are unobstructed, it is suitable to take half a dose of Muxiang Wan (Aucklandia Pill) to promote unblocking and moistening. Subsequently, use ten Banmao (Mylabris), removing legs, head, and wings, chopping finely and stir-frying together, then remove the Mylabris to eliminate fire toxicity. Decoct Dengxin Cao (Juncus Effusus) in thick liquid, mix with Wuling San (Five-Ingredient Powder with Poria), and administer 50–70 pills. Alternatively, decoct Dengxin Cao (Juncus Effusus) and Cong Bai (White Part of Allium Fistulosum) in water and wine, remove the Juncus and White Part of Allium Fistulosum, mix with Wuling San (Five-Ingredient Powder with Poria).
If pain attacks the abdomen and rebels upward to attack the Heart, leading to vomiting and rebellious Qi, first administer Yangzheng Dan (Nourishing Righteousness Pill) with salt wine. Next, use Shengliao Wuji San (Raw Material Five Accumulations Powder) added with Chao Wu Zhu Yu (Stir-fried Evodiae Fructus), Chao Huixiang (Stir-felled Foeniculum Vulgare Fruit), Tao Ren (Prunus Persica Seed), and Xuanhusuo (Corydalis Rhizoma), each stir-fried and added in half qian. There are cases where falling from heights injures the external genitalia, causing swelling and pain, or hematuria, or urinary obstruction. It is suitable to use Wuling San (Five-Ingredient Powder with Poria) combined with Fuyuan Tongqi San (Restoring Origin Promoting Circulation Powder).
In cases of Yin Nang (Scrotum) swelling as large as a dou (a unit of volume), where various medicines fail, it is suitable to use Danggui Sini Tang (Angelica Decoction for Cold Extremities) added with Sheng Jiang (Zingiberis Rhizoma Recens) and Zhu Yu (Evodiae Fructus). Dai Fu'an once used this formula with efficacy.
Hand Qi: Swelling and pain in the hand. Or involving the palm and fingers extending to the arm. Both Wubi Tang (Five Bi Decoction) and Juanbi Tang (Expelling Bi Decoction) may be used.
Shan Qi (Hernia) forms due to cold aggregation. Xia Jia (Masses formed by blood stasis) forms due to cold aggregation. This is Shan Qi. Now referred to as Heng Xian (Horizontal String) or Shu Xian (Vertical String). Pain wanders around the umbilicus and strikes the lower abdomen. It is suitable to use Pancong San (Allium Scallion Powder) and swallow Zhuyu Neixiao Wan (Calamus Internal-Resolving Pill). Or mix Yigong San (Different Attack Powder) with salt wine.
Regardless of the syndrome, Shengliao Wuling San (Raw Material Five-Ingredient Powder with Poria) can be used, added with half a qian or one qian of stir-fried Huixiang (Foeniculum Vulgare Fruit). If medication is ineffective and severe stabbing pain persists, with retraction of the Yin organ and cold extremities, it is suitable to use Xiangfu Zi (Cyperi Rhizoma). Stir-fry with salt, apply while hot wrapped in silk cloth, and moxa/iron the area below the umbilicus. If bowel movements and urination are not significantly obstructed, use Wuling San (Five-Ingredient Powder with Poria) added with Gui Zhi (Cinnamomi Ramulus) and administer Qingmuxiang Wan (Blue Aucklandia Pill).
At initial onset, if there is headache and fever, or aversion to cold with high fever, Shenru Yin (Ginseng and Perilla Drink) added with Mu Xiang (Aucklandiae Radix) is suitable. If rebellious Qi attacks the Heart, with no pain felt in the lower part but heart pain present, it is suitable to crush fresh Jiu Cai (Chinese Chive) to extract natural juice, mix with Wuling San (Five-Ingredient Powder with Poria) to make pills, and administer with Huixiang Tang (Fennel Decoction). This also cross-references Xin Bi (Heart Bi) pain syndromes. When Kidney Qi moves, Heart Qi also flares, causing pain both above and below, Yigong San (Different Attack Powder) is suitable, swallowing Zhuyu Neixiao Wan (Calamus Internal-Resolving Pill). Or temporarily treat the lower part exclusively; once lower pain settles, upper pain will settle.
When Kidney Qi rebels upward, phlegm-fluids obstruct, causing mental confusion and dizziness, Shen Ni San (Kidney Rebellious Powder) is suitable, swallowing Yangzheng Dan (Nourishing Righteousness Pill). When people suddenly suffer from unbearable pain in the chest, back, hands, feet, neck, waist, and hips, with牵引 (pulling) pain affecting bones and tendons, restlessness in sitting and lying, and wandering unpredictability, folk doctors do not understand and call it Zou Zhu (Wandering Injection). Using wind-dispelling drugs and acupuncture are incorrect. Suspecting wind-toxin aggregation wanting to form carbuncles and ulcers, and blindly using medicinal plasters is also incorrect. Or headache so severe one cannot lift the head, or mental confusion and fatigue, excessive sleep, loss of taste in food, thick and sticky sputum, sawing sounds in the throat at night, excessive salivation, heavy and cold numbness in hands and feet, unobstructed pulse, misdiagnosed as Tan Huan (Paralysis), is also incorrect. All these are actually stubborn phlegm-fluids and rheum hiding above and below the diaphragm, transforming into this disease (select medicine from the following section on retained fluids and hidden phlegm).
