Pain caused by blood stasis (blood clots) after childbirth is mostly due to difficult labor or excessive fatigue of the mother. It may also result from improper care or stagnation of cold pathogenic factors, leading to blood stasis and pain. Ancient methods often used Suxmu (Lignum Pterocaryae Indicae), Sanleng (Rhizoma Sparganii), and Ezhu (Rhizoma Curcumae) to rapidly attack and resolve the stasis. Contemporary physicians frequently used Guiwei (Caudex Radix Angelicae Sinensis), Honghua (Flos Carthami Tinctorii) to urgently promote blood circulation, or Yanyuan (Fructus Corydalis) and Niuxi (Radix Achyranthis Bidentatae) to strongly break up blood stasis. Others such as using Shanzha (Fructus Crataegi), Sha Tang (Saccharum) to dissolve clumps, Jiang (Rhizoma Zingiberis Preparatum), Jiao (Fructus Zanthoxyli), Ai Ye (Folium Artemisiae Argyi), and Jiu (Vinum) to alleviate pain are also not considered good prescriptions. As for treating Qi distension with Wuyao (Radix Linderae), Xiangfu (Rhizoma Cyperi Rotundi) to regulate Qi, Zhike (Fructus Aurantii) and Houpo (Cortex Magnoliae Officinalis) to smooth Qi, or using Qingpi (Pericarpium Citri Reticulatae Viride), Suzi (Semen Perillae) to descend Qi and calm asthma, Huangqin (Radix Scutellariae), Huanglian (Rhizoma Coptidis), Zhizi (Fructus Gardeniae), Huangbai (Cortex Phellodendri) to clear heat and relieve irritability. Cases where constipation due to blood dryness was cured by purging with Chengqi Tang (Miracle Soup for Reversing the Deficiency). Also, cases where sweating and scanty urination were treated with Wuling San (Five-Ingredient Powder with Poria) to promote diuresis, resulting in apparent recovery but actually worsening constipation. These all exacerbate deficiency; they are not only ineffective for postpartum mothers but also lead to various complications. Generally, while postpartum blood stasis should be resolved, new blood must also be generated. One must combine blood-moving with tonifying, and resolving stasis with generating new blood; this can be called skillful treatment. If Shenghua Tang (Decoction for Generating New Blood) can eliminate blood stasis, stop pain, clarify the mind, and restore Qi, it is the supreme treasure for postpartum conditions. Whenever the baby is born, before any food intake, one dose should be taken immediately, followed by two consecutive doses, which can prevent all critical postpartum syndromes.
Shenghua Tang (Decoction for Generating New Blood)
Danggui (Radix Angelicae Sinensis) (8 qian) Chuanxiong (Rhizoma Chuanxiong) (3 qian) Zhi Gancao (Radix Glycyrrhizae Preparata) (5 fen) Paojiang (Rhizoma Zingiberis Preparatum) (5 fen, 4 fen in summer) Taoren (Semen Persicae) (10 grains, skin and tip removed, crushed)
Decoct in water, add 6-7 spoons of aged wine and take. Store the dregs separately in a container; after finishing the first decoction of three doses, combine the dregs from all three doses into one batch, decoct in water, and take.
All diseases after childbirth stem from blood deficiency and flourishing fire, along with reckless movement of blood stasis. Occasionally, there are internal injuries from diet or external contraction of wind-cold, yet the primary treatment must always be expelling stasis and tonifying deficiency. If one does not combine expelling stasis, but merely takes tonics like Renshen (Radix Ginseng) that may cause stagnation, blood stasis attacking the heart will inevitably occur, potentially leading to death; eating too early causes the same issue. Therefore, whenever a postpartum mother feels her urine is short in volume, she knows disease has arisen and must take medicine to regulate the Spleen, Stomach, Liver, and Kidney. If there is no improvement, it must be due to Qi stagnation and rebellion. Generally, women are prone to worry, thought, anger, and resentment. Excessive worry and thought cause Qi binding and subsequent blood binding; excessive anger causes Qi rebellion and subsequent blood rebellion, severely manifesting as breast swelling and costal pain. In summary, female diseases are all due to Qi and Blood stagnation, so ancient formulas often use Qi-regulating herbs.
