Postpartum patients may experience delirium and hallucinations of ghosts due to fright disturbing the Heart Spirit, causing Stasis Blood (stagnant blood) to rush to the Heart, resulting in confusion and madness; or due to excessive blood loss failing to nourish the Heart, allowing Stasis Blood to harass the Heart. Zaoming Dan is effective.
Decoct dry lotus leaf, Sheng Dihuang (Rehmanniae Radix), and Mudanpi (Moutan Cortex) in equal parts, and take with raw Puhuang (Typhae Pollen). Symptoms will stabilize. Alternatively, use Xiong Gui Yi Mu Decoction taken with water-washed Zhusha (Cinnabar); or use Niuhuang Zaoming Dan from pediatric formulas for fright, which is particularly effective.
Speech originates from the Heart, which governs Blood. Loss of Blood leads to Heart Deficiency. It is suitable to use Chuanxiong (Ligusticum Rhizome), Danggui (Angelicae Sinensis Radix), Yimucao (Leonuri Herba), Maidong (Ophiopogonis Tuber), Yuanzhi (Polygalae Radix), Fushen (Poria with Hostwood), and Renshen (Ginseng Root), adding a small amount of Honghua (Carthami Flos). Decoct together with gold and silver vessels, and take with Zhusha (Cinnabar). Zaoming Dan can also be taken. Alternatively, take one qian of Baifan (Alumen) mixed with hot water.
To determine if there is residual Stasis Blood, check if the abdomen is painful. If the abdomen is soft and painless, the Stasis Blood has been expelled. The severity of pain indicates the amount of blood stasis. If there is severe lower abdominal pain, take the Main Formula and Heishen San, and externally wrap hands in cotton clothing, heat them, and massage the painful area. If treatment is delayed, sudden metrorrhagia, coma, syncope, and death may occur.
Metrorrhagia (non-menstrual heavy bleeding) in women is a critical condition. Young women often have vigorous Fire and Hot Blood. Indulgence in sexual activity within the curtains, or intercourse before menstruation has completely ceased; anger causing Qi rebellion while holding cold water; or consuming raw and cold foods, all lead to Stasis Blood stagnation. Labor overexertion causing Yang hyperactivity, or emotional disturbances generating internal Fire, cause Stasis Blood to flow recklessly, appearing red or purple; or forming purple-black clots, resulting in this disease. Generally, cooling Blood, astringing, lifting Qi, and nourishing Ying (Nutritive) Qi can cure it. Women past middle age and elderly women often suffer from excessive worry. Anger injures the Liver; when the Liver is injured, it cannot store Blood, leading to irregular and erratic menstruation. Pain may precede menstruation, followed by intermittent flow; or near the age of forty-nine (seven sevens), Tian Gui (menstruation) should cease but does not. These are caused by constrained Fire unable to disperse, attacking organs and forcing Blood to flow downward excessively or abnormally, constituting symptoms of "Mountain Collapse" (severe metrorrhagia) and leakage. This involves deficiency of both Qi and Blood, making rapid recovery difficult. One must cool Blood and stop bleeding, dissipate constrained Fire, harmonize Liver Qi, and simultaneously greatly tonify Qi and Blood to achieve success.
