Due to the mother's consumption of alcohol and meat, as well as excessive sexual activity generating heat-toxin qi, this pathogenic factor flows into the breast milk. When the infant drinks it, the heat-toxin is transmitted, causing body fluids to condense into phlegm (Tan, Phlegm) which accumulates internally. Combined with external contraction of wind-cold (Wind-Cold), this leads to high fever, palpitations with fright, upward rolling of the eyeballs, convulsions of the hands and feet, intermittent crying or laughing, and in severe cases, cyanosis of the fingernails, resembling possession by spirits. The pulse is large and excess (Shi, Excess). The treatment should focus on resolving wind-heat (Feng-Re, Wind-Heat).
Main Formula: Gouteng Tang (Uncaria Decoction) ingredients include Uncariae Ramulus cum Uncis (Gouteng, 5 fen each), Ginseng Radix et Rhizoma (Renshen, 5 fen each), Cornu Rhinocerotis (Xijiao, 3 fen, ground separately), Gastrodiae Rhizoma (Tianma), Scorpio (Quanxie, washed and baked), and Glycyrrhizae Radix et Rhizoma Preparata (Zhigancao, 2 fen each). Add fourteen Mung Bean seeds (Lvdou) finely ground, and Fine Tea (Xicha, 3 fen). Decoct in water and take orally.
If wind pathogen is prominent, add Schizonepetae Herba (Jingjie). If there is phlegm clouding the mind causing unconsciousness, add Tianshuhuang (Tian Zhu Huang), Cinnabaris (Chensha), Realgar (Xionghuang) (1 fen each), and Alum (Kufan, 0.5 fen). Grind these into fine powder and mix into the decoction before taking.
Abdominal pain with frequent crying, sweating with loose stools, biting at the breast, dull eyes, salivation, or blackened dry lips, scrotal swelling, stooping posture, red veins inside the eyes, and the pulse pattern at the Tiger's Mouth (Hukou) extending into the palm indicate a condition best treated with Zaoming Dan (Life-Preserving Pill) or Lijing Wan (Relieve Fright Pill). (Both formulas are described under the section for Acute Fright Convulsion/Ji Jing.)
Main Formula: Taofu Wan. Ingredients include Resina Burserae (Ruxiang, ground to powder, 1 qian), Vermilion (Yinzhu, 5 fen), and Garlic Bulb (Suan, one clove, roasted, peeled). Pound into pills the size of millet seeds. Take five pills per dose, administered with Mint Decoction (Bohe Tang).
The Classics state: Epilepsy (Xian) is when the Spirit (Shen) does not reside in its house, meaning the spirit is chaotic. It is caused by fright disturbing the organ qi, leading to imbalance, stagnation producing saliva/phlegm, blocking the meridians, and resulting in reversal/convulsion. Causes may include fright during gestation, contraction of wind-cold-damp-heat, or irregular diet disrupting organ qi. There are three types of epilepsy: Fright Epilepsy (Jing Xian), Wind Epilepsy (Feng Xian), and Food Epilepsy (Shi Xian). Fright Epilepsy arises from shock; Wind Epilepsy arises from overdressing causing extreme heat generating wind (initially characterized by flexed fingers, as if counting); Food Epilepsy results from overfeeding, leading to vomiting and fever. Symptoms include staring straight ahead, facial muscle traction, locked jaw with drooling, abdominal distension, convulsions of limbs, opisthotonos (backward arching of neck and back), with episodes occurring intermittently. Treatment should use herbs that regulate qi, resolve phlegm, and harmonize blood and qi, taken with Suheli Wan (Suheli Pill) or preemptively with Wu Xian Wan (Five Epilepsies Pill). Zaoming Dan is the primary formula.
Main Formula: Ren Shen, Changpu Rhizome (Shichangpu), Dictamni Cortex (Baixianpi), Poria (Fushen) (5 fen each), Polygalae Radix (Yuanzhi Rou, 5 fen), Ophiopogonis Tuber (Maidong, 5 fen), Rhinoceros Horn (Xijiao, ground separately), Gypsum Fibrosum (Shigao, ground, 3 fen each), and Licorice Root (Gancao, 1 fen). Decoct in water and take orally.
Wu Xian Wan: Treats various epileptic conditions in children, as well as manic-depressive psychosis (Dian Kuang) in adults. Lead (1 liang, melted in a copper spoon), Mercury (5 qian, mixed evenly with lead, cooled, then ground extremely fine together), Pearl (Zhenzhu, water-triturated, re-ground, 1 liang), Realgar (Xionghuang, 1 liang), Cinnabar (Zhusha, 5 qian). All ingredients are triturated with water ten thousand times, then mixed and re-ground. Form into pills the size of hemp seeds using honey. Take one pill per dose, delivered with Gold and Silver decoction.
Niuhuang Gao (Calculus Bovis Ointment/Paste): Treats Wind Epilepsy. Symptoms include convulsions and tidal fever. Ingredients include Arisaema Cum Bile (Dannanxing), Scorpio (Quanxie, washed and fried), Cicada Slough (Chantui, feet removed, 2 qian each), Bombyx Batryticatus (Jiangcan, mouth removed, fried, 2 qian), Typhonii Rhizoma (Baifu), Saposhnikoviae Radix (Fangfeng), Gastrodiae Rhizoma (Tianma, baked, 1 qian each), and Calculus Bovis (Niuhuang, 2 fen). Grind all into fine powder. Form pills the size of small beans using jujube flesh. Administer with decoction of Schizonepetae Herba (Jingjie) and Ginger (Jiang).
