The *Classic of Internal Medicine* states: All pain, itching, and sores/ulcers belong to Heart Fire; all swelling and fullness caused by dampness belong to Spleen Earth. The Heart governs Blood, and the Spleen governs Flesh. When Heat in the Blood and Dampness in the Flesh interact, they cause the skin and muscles to ulcerate and disperse, oozing incessantly without a fixed name, thus termed "Erythema Ulcer" (Evil Ulcer). However, differentiation is necessary. For instance, if Qi and Blood are stagnant in the skin and interstices (the spaces between skin and muscle), it can be dispersed through the exterior via sweating, such as in cases of Scabies, Tinea, Sores, and Rash. Sweating will cure the ulcer. If, however, Qi and Blood are stagnant within the layers of the flesh, extending externally to connect with the skin, presenting with aversion to cold, fever, and pus formation; or if caused by emotional disturbances or rich/greasy diet, internal support (Nei Tuo) should be used rather than sweating. Sweating would lead to complications. Therefore, for ulcers formed between the skin, one should treat them according to their lightness by dispersing them upward; hence, sweating is indicated, accompanied by topical application of insecticidal and moisturizing herbs. For ulcers formed deep within the muscle, one should treat them according to their heaviness by reducing them; clearing heat from the meridians, cooling the Qi and Blood, and applying topical poultices that promote pus discharge and tissue regeneration externally. For ulcers located on the vertex of the head, one should lift and extract them using ascending methods; employing wine-processed cooling herbs, combined with wind-dispelling herbs to ascend upwards, and applying topical insecticidal and heat-clearing herbs externally. Understanding these three principles covers more than half of the knowledge required for treating sores and ulcers.
