The syndrome is caused by the invasion of wind-dampness, where damp-heat leads to sweating and moisture, resulting in itching of the scrotum. Alternatively, it may be due to the deficiency of Kidney Water (Kidney Yin), with fire flaring up causing blood dryness, leading to itching in the private parts that develops into sores with oozing yellow fluid. This formula is also used to treat sweaty feet; it can be applied to the soles of the feet, placed inside shoes, or used for maceration between the toes.
External Application Formula: Borneolum Syntheticum (Bingpian) is not listed here but in the paste. The ingredients are: Alum (Ku Fan, 1 liang), Oyster Shell (Muli, dosage unspecified in text), and Red Lead (Huang Dan, stir-fried). Each of the specified amounts is 5 qian. Grind into fine powder, apply several times at night.
Note: Externally, this area belongs to the Foot Three Yang Meridians; internally, it belongs to the Foot Three Yin Meridians. These sores are all caused by damp-heat sinking downward and blood stasis stagnating; or they begin due to cold-dampness resembling a skirt hem, with wind-pathogen injuring the fine collaterals of the hands. If one does not observe dietary taboos and recklessly consumes heat-toxic foods, injury occurs in the morning and worsens by evening; prolonged delay causes dampness and stasis to generate heat, with damp-heat steaming each other, gradually forming this affliction. Some cases arise from bruising or trauma, but cuts from pot rims become lifelong ailments because movement and labor cause Qi to flow downward, making healing difficult. External treatment involves applying medicated plasters; internal treatment involves taking Danggui Nian Tong Tang (Angelica Decoction for Relieving Pain).
If Qi and Blood are deficient, tonifying and expelling formulas should be used, and foods that generate heat-toxicity and move Qi recklessly must be strictly avoided.
Plaster Formula: Asafoetida (Wei, 1.5 qian, dissolved in a small amount of wine), Red Lead (Huang Dan, 2 qian), Child's Tea (Ercha), Dragon Bone (Long Gu, 1.5 qian each), Frankincense (Ruxiang), Cuttlefish Bone (Haipiaoxiao, boiled in water and sun-dried, 2 qian each), Red Kaolin (Chishizhi), Dragon's Blood (Xuexie, 1.5 qian each), Mercury (Shuiyin, 1 qian, prepared with lead), Calomel (Qingfen, 2 qian), Myrrh (Moyao, sweated and ground fine, 2 qian). All ingredients are ground into extremely fine powder, mixed with 3 fen of Borneol (Bingpian), and blended evenly for use. Sesame Oil (Mayou, 2.5 liang) is boiled in a copper spoon until old/thick; when a willow stick shows smooth rotation, add Yellow Wax and White Wax (each 5 qian) to melt. Then add young Pine Rosin (Songxiang, 2 liang) and boil until melted. Filter through cotton cloth into a bowl, place in water overnight. Mix in the previous powders evenly. Place between two layers of thin oil paper to form the plaster. If the sore is large, fold the edges of the paper; prick dense holes in the plaster with a pin. Wash the sore with a cleansing formula, or decoct green onion and Sichuan pepper in rice washing water, wash, and pat dry. First sprinkle Xuan Shuang San (Dark Frost Powder), then protect with Geng Sheng Ji San (New Flesh Generating Powder). Bind the plaster externally, rotating its position daily.
Interleaved Paper Plaster: Astragalus Root Powder (Huangqi Mo, 5 qian), Calomel (Qingfen), Frankincense (Ruxiang), Myrrh (Moyao), Cinnabar (Yinzhu, 1 qian each), Dragon's Blood (Xuexie, 5 fen), Verdigris (Tongqing, 2 fen). Grind all ingredients into fine powder and mix evenly. Blend with sesame oil to form a paste, and prepare on thin oil paper as described above.
Another Formula: Cut twelve sheets of soft cotton paper according to the size of the sore, securing the four corners with small paper twists for later use. Boil 2 liang of sesame oil in a copper spoon. Add 49 grains of Sichuan Pepper (Chuanjiao) and fry until black, then remove. Add 49 twigs of Sophora Tree (Huai Zhi, 1 cun long) and fry until charred black, then remove. Add 1 liang of Yellow Wax, 2 fen of Calomel (Qingfen), and 5 fen of Alum (Kufan). Boil the papers until fully penetrated, being careful not to burn them. Remove. Wash the sore warmly with a decoction of Sophora branches, green onion, and Sichuan pepper, wipe dry, apply the plastered paper, cover with oil-coated single paper, and bind securely with silk fabric. Remove once a day; replace the sheet closest to the sore. Healing occurs in twelve days.
Another Formula: Pine Rosin (Songxiang, 3 qian) and Frankincense (Ruxiang, 2 qian), each ground into powder. Blend with sesame oil and apply to the affected area. Cover externally with bamboo leaf sheath (Sunruo) pierced densely with holes and bound. Alternatively, bind with Wintergreen leaves (Dongqing Ye), rotating the position daily.
Another Formula: Phellodendron Bark Powder (Huangbai Mo, 1 liang) and Calomel (Qingfen, 3 qian). Blend with pig bile for application; for moist sores, sprinkle the dry powder directly.
Another Formula: Chinese Gall Powder (Wubeizi Mo, 1 liang), Sesame Oil (Mayou, 1 liang), and Garden Soot (Baicaoshuang, 1 qian). Boil until rolling, dissolve 1 liang of Yellow Wax, blend with the aforementioned powders to form an interleaved paper plaster for application.
Another Formula: Sichuan Pepper Powder, Alum, and Garden Soot are ground evenly, blended with Tung Oil for application. Wash with a decoction of green onion and Sichuan pepper in rice washing water.
Another Formula: Hearth Earth (Zaoxintu), Red Lead (Huang Dan), Calomel (Qingfen), Phellodendron Bark Powder (Huangbai Mo), Frankincense (Ruxiang), and Myrrh (Moyao) are all ground into powder in equal proportions. Blend evenly with clear oil to form a paste. Apply between thin oil paper (boiled in Licorice root water) with perforations to the affected area. Rotate the position frequently daily, avoid disturbing the wound, and observe dietary taboos. Adding Red Kaolin (Chishizhi) enhances efficacy.
Another Formula: Calamine Stone (Luganshi) is calcined until yellow, then quenched in juices of Coptis Rhizome (Huanglian) and Phellodendron Bark (Huangbai). Sun-dry, grind into fine powder (1 liang). Blend with White Beeswax (Baizhan, 5 qian, ground) and Ivory Powder (Xiangya Mo, 3 qian). Mix evenly and apply with sesame oil.
Simple Formula: Chen Xiao Fen (a type of starch/powder, ground), roasted and ground Sesame Seeds, and fine tea powder are blended evenly with sesame oil. Spread on paper, apply, cover with oil paper externally, change once daily, and wrap the medicine.
