At the initial stage, there is high fever, redness of the cheeks and face, dry and withered hair, restlessness, thirst with desire to drink water, incessant crying and inability to calm down, preference for lying in cool places, removing clothes and blankets, constipation, and scanty, dark, and dysuric urine. It is appropriate to administer Shengma Gegen Tang (Cimicifugae Rhizoma and Puerariae Radix Decoction) combined with Shishen Jiedu Tang (Ten-God Detoxification Decoction). Once all heat symptoms have subsided, Taiyi Baohe Tang (Great Unity Harmonizing Preservation Decoction) is used to assist in the formation of pustules; after the pustules are full, Baoying Baibu Tang (Infant Protection Hundred-Tonics Decoction) is administered. If on the eighth or ninth day the pustules turn purple-black and become dry and depressed, drugs such as Dusheng San (Sole Victory Powder) and Renya (Human Tooth) can be used. Only when diarrhea occurs and heat symptoms have been eliminated, revealing deficiency-cold patterns, Yigong San (Yi Gong Powder) and Muxiang San (Aucklandia Powder) may be applied.
In cases where children suffer from prolonged illness or extreme weakness of Yuan Qi (Primordial Qi), the fever is mild, alternating between hot and cold, with hands and feet occasionally cold, cyanotic complexion, mental fatigue, poor appetite, lethargy, and clear, free urination and defecation. If mild exterior-releasing treatment is needed, Zi Su (Perilla Leaf), Chuanxiong (Ligusticum Rhizome), Fangfeng (Saposhnikovia Root), Baizhi (Angelica Dahurica Root), Jiegeng (Platycodon Root), Dafupi (Areca Peel) are suitable, with small amounts of Baizhu (Atractylodes Macrocephala Rhizome), Fuling (Poria), and Gancao (Licorice Root), slightly assisted by Shengma (Cimicifuga Rhizome). After the pox are fully erupted, herbs such as Renshen (Ginseng Root), Huangqi (Astragalus Root), Chuanxiong (Ligusticum Rhizome), and Jiegeng (Platycodon Root) should be added. If the pus is thin, the pox are ash-white, and spontaneous sweating occurs, Baoyuan Tang (Preserve Origin Decoction) is the main formula, or Shiquan Dabu Tang (Complete Great Tonifying Decoction) with Chuanxiong removed and Shanzha (Hawthorn Fruit) added. If diarrhea, cold feet, chills, and teeth grinding appear, indicating a deficiency-cold pattern, Yigong San (Yi Gong Powder) or Muxiang San (Aucklandia Powder) can be selected.
At the onset, high fever feels like burning iron, with red face, withered hair, coarse breathing, dry mouth and thirst, abdominal distension, angry crying, delirium, and constipated and difficult urination and defecation, along with facial edema and swollen eyes. One dose of Shengma Gegen Tang (Cimicifugae Rhizoma and Puerariae Radix Decoction) with exterior-releasing herbs is appropriate, followed by Qianghuo San Yu Tang (Notopterygium Dispersing Depression Decoction). Once heat toxins are pacified, Yiyuan Touji Tang (Benefit Origin Muscle-Penetrating Decoction) and Baoying Baibu Tang (Infant Protection Hundred-Tonics Decoction) are used. Subsequent treatments are the same as for blood-heat patterns. For patients with excessive heat in both interior and exterior, only An Biao (Stabilize Exterior), He Zhong (Harmonize Middle), Yun Qi (Regulate Qi), Tou Ji (Penetrate Muscle), and Jie Du (Detoxify) methods should be employed. As long as these five principles are not deviated from, even severe cases can recover.
Voice originates from Fei (Lung); when Lung Metal is constrained by Fire, and toxins obstruct the airway, voice cannot be produced. Throat pain also arises from this cause. Ganjie Tang (Licorice and Platycodon Decoction), Xuanshen (Scrophularia Root), Niubangzi (Arctium Fruit), etc., are indicated. Water choking occurs because the respiratory tract is anterior and the esophagus posterior; if the respiratory tract is obstructed by pox, tea or water enters mistakenly, causing coughing. Treatment focuses on detoxification with Ganjie Tang (Licorice and Platycodon Decoction), adding Xuanshen (Scrophularia Root), Niubangzi (Arctium Fruit), Maidong (Ophiopogon Tuber), Xingren (Apricot Seed), Jingjie (Schizonepeta Spike), etc. Once the pox heal and contract, peace will naturally return.
