Clansman's younger brother: Yin deficiency causing fever and hemoptysis, with a pulse that is large, empty, and rapid, particularly pronounced at the left Guan (Hepatic) and Chi (Renal) positions. Accumulated damage from overthinking has lasted nearly three years; the "Dragon and Thunder" fire (hyperactive ministerial fire) fails to submerge, leading to the internal consumption of true Yin and injury to the lung collaterals. As Yin becomes increasingly deficient, Yang becomes increasingly hyperactive. Without using salty substances to descend rebellious Qi, it is unlikely to stop abruptly. Prepared a paste using Shich (Larimar/Stone Ash), Ejiao (Donkey-hide Gelatin), Shudi (Prepared Rehmannia Root), Wuweizi (Schisandra Fruit), Shanyao (Dioscorea Rhizome), Baihe (Lily Bulb), Beimu (Fritillaria Bulb), Danpi (Moutan Cortex), Baishao (White Peony Root), and Dancui (Seaweed). Administered with Lotus Root Decoction; bleeding ceased and the deficiency gradually healed.
Mr. Mao: Loss of blood due to consumptive fatigue, with both Cun (Inch) and Chi (Cubit) pulses appearing large and rapid, indicating Lung and Kidney deficiency. Prescribed Sancai Tang (Three Capacities Decoction) added with Danpi (Moutan Cortex), Baishao (White Peony Root), Maidong (Ophiopogon Tuber), and fresh Lotus Root. After several doses, bleeding stopped, but afternoon fever and cough persisted. Prescribed Liuwei Wan (Six-Ingredient Pill with Rehmannia) removing Yuzhu (Cornus Fruit) and Xie (Alisma Rhizome - *Note: Text says 'Xie', likely Ze泻*), adding Wuweizi (Schisandra Fruit), Baishao (White Peony Root), Guiban (Tortoise Shell, prepared), and Ejiao (Donkey-hide Gelatin). Made into honey pills; two courses resulted in complete recovery.
Mr. Ding: Blood spots in sputum, turbid discharge after urination, and nocturnal emission without dreams at the fifth watch (3-5 AM). This indicates Kidney Yin deficiency and strong ministerial fire. Prescribed Liuwei Wan (Six-Ingredient Pill with Rehmannia) removing Yuzhu (Cornus Fruit), adding Huyao (Fish Maw, fried), Lianxu (Lotus Stamen), Tusiji Bing (Cuscuta Seed Cake), with a slight addition of Huangbai (Phellodendron Bark, salt-fried). Made into honey pills. Administered with light salt water; gradual recovery ensued.
Madam Wei: Afternoon fever, vomiting, coughing, and blood-streaked sputum, indicative of upper consumption (Lung deficiency). Prescribed Ejiao (fired with蛤粉 Ge Fen/Molluscum Shell Powder), Baihe (Lily Bulb), Fushen (Poria with Hostwood), fresh Lotus Root (each 3 qian), Luzhen (Codonopsis Radix), Shanyao (Dioscorea Rhizome), Baishao (White Peony Root), Danpi (Moutan Cortex) (each 2 qian), Beimu (Fritillaria Bulb) (1 qian), Wuwei (Schisandra Fruit) (4 fen), and Red Dates (5 pieces). After two doses, bleeding stopped and fever subsided. Removed Danpi, Ejiao, and fresh Lotus Root; added Gualou Ren (Trichosanthes Seed). After two more doses, phlegm and cough were eliminated.
Mr. Sui: Hemoptysis began in early summer and persisted until late autumn. Some attributed it to dryness-fire punishing Metal (Lung); others to hyperactive Dragon-Thunder fire. Pulse diagnosis showed Right Cun as short and choppy, Left Guan as deep and wiry. This corresponds to suppressed worry and frustration, where Qi-level injury extends to the blood collaterals. He reported that every afternoon, qi stagnation in the throat leads to coughing up bloody sputum. Yang governs opening; Yin governs closing. The afternoon belongs to Yin within Yang, governing closure. Qi channel obstruction does not occur without internal factors like suppressed worry. Prescribed Jiegeng (Platycodon Root), Beimu (Fritillaria Bulb), Muxiang (Aucklandia Root), Gualou (Trichosanthes Fruit), Fushen (Poria with Hostwood), Danggui (Angelica Sinensis Root), Baishao (White Peony Root), and Jiangxiang Mo (Aquilaria Wood powder). After two doses, epigastric comfort returned and bleeding stopped. Removed Muxiang and Jiangxiang; added Yujin (Curcuma Tuber) and Shudi (Prepared Rehmannia Root). After two more doses, the pulse normalized. Subsequently prescribed Guipi Tang (Restore the Spleen Decoction) removing Renshen (Ginseng) and Baizhu (Atractylodes Macrocephala Rhizome), adding Shudi (Prepared Rehmannia Root), Beimu (Fritillaria Bulb), Baishao (White Peony Root), and Lianzi (Lotus Seed) for cure.
Mr. Qian: Three episodes of blood loss, all triggered by eating. Current hemoptysis also occurs during meals, indicating that food entry obstructs Qi in the stomach tube, causing blood to spurt; thus, swallowing saliva feels like choking. All pulse positions are wiry. Treatment should regulate rebellious Qi and repair leaking collaterals. Prescribed Gualou (Trichosanthes Fruit), Beimu (Fritillaria Bulb), Danggui (Angelica Sinensis Root), Yuzhu (Polygonatum Odoratum Rhizome), Ejiao (Donkey-hide Gelatin), and Red Dates. Cured after administration; frequent consumption of cow's milk afterwards prevented recurrence.
Mr. Li: After Start of Winter, Yang hides in the earth, and dragons dive into the sea. Now at Winter Solstice, Yang begins to generate, but the dragon fire no longer hides, causing midnight Yang ascent, gray sputum with blood, and an uneven Right Chi pulse. This is an indication for Zhibai Bawei Wan (Anemarrhena, Phellodendron, and Eight-Ingredient Pill). Additionally, he has a history of Liver Qi stagnation causing stabbing pain in the left hypochondrium, indicating Dragon-Thunder fire flaring together. Initial treatment with drugs to strengthen Water and submerge Yang reduced sputum and blood. Subsequent treatment with Modified Guipi Tang (Restore the Spleen Decoction) stopped left hypochondriac pain, reduced gray sputum, and made blood streaks faint, signaling impending recovery. Yesterday’s pulse showed only slightly wiry Liver pulse, with Left Chi stronger than Right Chi, indicating Water can still control Fire. From now on, maintain a calm mind, restrain anger, and moderate desires. If Wood and Fire do not become excessive next spring, full recovery is expected. Shudi (water-boiled), Danpi (wine-fried), Zexie (Alisma Rhizome, salt-fried), Fuling (Poria, milk-steamed), Shanyao (stir-fried), Yuanzhi (Polygala Root, licorice juice-fried), Baishao (stir-fried), Nvzhenzi (Ligustrum Lucidum Fruit), Ou Fen (Lotus Root Powder), Dancui (Seaweed), Muli (Oyster Shell, crushed). Made into honey pills.
Madam Jiang: Sputum cough with blood after spontaneous abortion, aversion to cold in the afternoon, fever at night, decreased appetite, and emaciation, with a small and weak pulse. This chronic damage involves unstable uterine connections; post-abortion, Ying (Nutrient) and Wei (Defensive) Qi are further injured. It is appropriate to imitate Mr. Lizhai’s method: using sweet-warm herbs to tonify Yang will reduce chills and fever. Recent practitioners focus solely on Xingren (Apricot Seed) and Beimu (Fritillaria Bulb) hoping to stop cough, fearing that cold-natured herbs damage the Spleen, potentially leading to untreatable conditions. Prescribed Luzhen (Codonopsis Radix), Shanyao (Dioscorea Rhizome), Fushen (Poria with Hostwood), Zhigancao (Licorice Root, roasted), Ejiao (Donkey-hide Gelatin), Baishao (White Peony Root), Wuwei (Schisandra Fruit), Qizi (Goji Berry), Lianzi (Lotus Seed), and Zao (Red Date). Several doses brought relief. Added Huangqi (Astragalus Root) and Lujiaoshuang (Deer Antler Frost); several more doses stopped various symptoms and increased appetite. Finally cured with pill formulation for regulation.
Madam Jing: Elderly patient became angry after eating, causing Qi to rise and blood to surge, profuse watery diarrhea, body heat with cold back, irritability, and dizziness. The Classics state: "Anger causes Qi rebellion; severely, it leads to vomiting blood and undigested stool diarrhea," hence Qi rises. The pathology stems from stagnant and exuberant Liver Yang, shaking blood collaterals, excessive dispersion, where Wood insults Earth. Undigested food in the Stomach, combined with Summerheat-Dampness commanding the season and damp earth rising, causes violent downward pressure. Upward Qi rebellion causes epigastric tightness. Treatment focuses on descending rebellion, assisted by eliminating fullness, so blood returns to vessels and the Stomach harmonizes. Prescribed Houpo (Magnolia Bark, processed), Shanzhi (Gardenia Fruit, stir-fried), Yujin (Curcuma Tuber, ground), Sugeng (Perilla Stem), Fuling (Poria), Yiyi (Coix Seed), Sharen (Amomum Fruit), Jiangxiang (Aquilaria Wood), and Zhiqiao (Bitter Orange Fruit). One sip induced slight sweating, and previous symptoms vanished.
Madam Dai: Internal damage from emotions, fire forcing collateral injury causing coughing blood, aversion to cold in the afternoon, fever at night, Right pulse empty, Left pulse rapid. Ying deficiency and Wei timidity. First use rich, moistening herbs to repair collaterals, nourish Yin and harmonize Yang. Wait until Summer Solstice when Yin generates, preventing Yang from scorching, allowing recovery. Ejiao (dissolved in water), Shengdi (Raw Rehmannia Root, stir-fried), Maidong (Ophiopogon Tuber) (each 1 qian), Fushen (Poria with Hostwood) (3 qian), Qizi (Goji Berry), Shanyao (Dioscorea Rhizome), Tian Xingren (Sweet Apricot Seed, all stir-fried) (each 2 qian), Danpi (Moutan Cortex), Shihu (Dendrobium Stem) (each 1.5 qian), Wuwei (Schisandra Fruit, baked) (5 fen). Six doses brought general peace; only slight chest pain remained. Added Baishao (White Peony Root) (2 qian) and Lou Pi (Trichosanthes Peel) (8 fen); pain ceased.
Mr. Wang: After cure of strangury, a fistula appeared, likely due to premature use of astringent herbs. The surgeon used thread medication; before the pus tract was removed, sudden hemoptysis occurred. Left pulse empty, Right Chi pulse forceful against the finger. This indicates Dragon Fire failing to submerge, manifesting above as hemoptysis and below as purulent leakage. Fatigue causes strangury and painful urination. Without strengthening Water to control Yang, how can the leak cease? The condition may progress from lower damage to upper damage, becoming consumptive cough or hematemesis/hemoptysis; anal fistula cannot heal easily. Temporarily administer decoction, imitating modified Hujian Wan (Tiger-Knee Pill): Shudi (water-boiled), Guijiao (Turtle Shell Gelatin), Dancui (Seaweed), Baishao (White Peony Root), Danggui (Angelica Sinensis Root), Wuwei (Schisandra Fruit), Qizi (Goji Berry), Zhimu (Anemarrhena Rhizome), Huangbai (Phellodendron Bark, all wine-fried). Six doses normalized pulse and symptoms. Later made into honey pills with Fuling (Poria), Shanyao (Dioscorea Rhizome), Danpi (Moutan Cortex), and Niuxi (Achyranthes Root). Administered with salt water; fistula also healed.
Madam Shi: Chest pain and hematemesis, blood color mixed red and purple, dizziness and epigastric stuffiness, pulse Kou (hollow) and faint. This indicates worry and thought damaging Ying Qi. Treatment should astringe and tonify Heart Spirit, while soothing Spleen constraint. Any blood leaving the vessels turning purple is not necessarily due to accumulated stasis. Prescribed Luzhen (Codonopsis Radix), Fushen (Poria with Hostwood), Baishao (White Peony Root), Wuwei (Schisandra Fruit), Zaoren (Ziziphus Seed), Zhigancao (Licorice Root, roasted), Danggui (Angelica Sinensis Root, vinegar-fried), Hehuanhua (Albizia Flower), Yujin (Curcuma Tuber), Muxiang (Aucklandia Root) (all ground into juice and taken). Three doses brought peace; recovery followed with regulation.
