(Attached: Thunderhead Wind)
When pathogenic wind invades the upper body, a recent external contraction causing headache is termed "Headache," while a deep-seated, chronic condition is termed "Head Wind" (Toufeng). Its symptoms include heaviness and vertigo at the vertex of the head, numbness of the scalp, tinnitus, dizziness, and tight, pulling pain in the supraorbital ridge. If the patient has pre-existing phlegm-fire, and subsequently exposes themselves to wind while cooling off, the pathogen enters the brain via the Fengfu point (GV16), stagnates, and transforms into heat and pain. In severe cases, it may cause eye diseases, blurred vision, and vertigo. Untreated Head Wind will inevitably damage the eyes. Treatment should differentiate between lateral or central location, left or right side, cold or heat patterns, and Qi or Blood deficiency.
Pain located at the center of the vertex often indicates wind stagnation in the Taiyang channel; it is appropriate to use Chuanxiong Rhizome (Chuanxiong), Notopterygium Rhizome (Qianghuo), Vitex Fruit (Manjingzi), and Perilla Leaf (Suye) to disperse the pathogen. The Taiyang channel runs from the forehead to the vertex and connects with the back of the head. Pain on the left or right sides often indicates fire stagnation in the Shaoyang channel; it is appropriate to use Chrysanthemum Flower (Ganjuhua), Moutan Bark (Danpi), Gardenia Fruit (Shanzhi), Mulberry Leaf (Sangye), and Uncaria Ramulus cum Uncis (Gouteng) to vent the fire. The Shaoyang channel descends from the temporal angle to the ear and surrounds the anterior and posterior aspects of the ear.
Pain偏于 the left is due to wind deficiency; use Chuanxiong Rhizome (Chuanxiong), Angelica Sinensis Root (Danggui), Saposhnikovia Root (Fangfeng), and Mint Herb (Bohe). Pain偏于 the right is due to phlegm-heat; use Atractylodes Rhizome (Cangzhu), Pinellia Tuber (Banxia), Scutellaria Root (Huangqin), and Gypsum Fibrosum (Shigao). Cases due to Qi deficiency are often caused by overexertion; use Buzhong Yiqi Decoction (Tonify the Middle and Augment the Qi Decoction) added with Chuanxiong Rhizome (Chuanxiong) and Gastrodia Rhizome (Tianma). Cases due to Blood deficiency involve susceptibility to fright; use Siwu Decoction (Four Substance Decoction) added with Mint Herb (Bohe) and Angelica Dahurica Root (Baizhi). Heat-type pain involves aversion to heat; use Xiaofeng San (Dispel Wind Powder). Cold-bi type pain involves aversion to cold; use Zhuifeng San (Chase Away Wind Powder).
If cold and heat have been stagnant for a long time, causing stifling pain during attacks where the patient desires to wrap the head in cotton, this indicates much phlegm; use Erchen Decoction (Two-Cured Decoction) added with wine-fried Scutellaria Root (Jiu Huangqin), Schizonepeta Spike (Jingjie), Chuanxiong Rhizome (Chuanxiong), Mint Herb (Bohe), Gypsum Fibrosum (Shigao), and Asarum Root (Xixin). When wind combines with heat, use Chatiao San (Tea Seasoning Powder) or Juhua San (Chrysanthemum Powder). When cold clings with dampness, use Daotan Tang (Phlegm-Conducting Decoction) added with Atractylodes Rhizome (Cangzhu) and Angelica Dahurica Root (Baizhi). Pain extending to the gums uses Gouteng San added with Schizonepeta Spike (Jingjie) and Mint Herb (Bohe). Pulling pain in the supraorbital ridge uses Xuanqi Tang (Select Extraordinary Decoction). Foul-smelling nasal discharge uses Xiongxī San (Asarum and Rhinoceros Horn Powder, note: modern substitutes used) or Tou Ding San insufflated into the nose to induce salivation. Spasms of tendons at the back of the head use Uncaria Ramulus cum Uncis (Gouteng), Lotus Leaf Margin (Heyebian), Forsythia Fruit (Lianqiao), Kudingcha Leaf (Kudingcha), and Chrysanthemum Flower (Ganju). Pain driven upward by Qi uses Quanxie San (Scorpion Powder). Chronic cases unyielding to treatment use Aconite Lateral Root Preparatum (Wutou) and Pinellia Tuber processed with ginger and alum (Nanting) powdered, mixed with green onion juice and applied to the Taiyang acupoint. For women whose blood level is affected by wind, use Yangxue Qufeng Tang (Nourish Blood and Dispel Wind Decoction).
For dizziness and pain caused by rebellious Gallbladder fire, treat by draining heat using Antelope Horn (Lingyangjiao), Rehmannia Root (Shengdi), Moutan Bark (Danpi), Chrysanthemum Flower (Ganju), Kudingcha Leaf (Kudingcha), and young Mulberry Leaf (Nen Sangye). For vomiting caused by Liver Yang invading the Stomach, treat by extinguishing wind using Poria Sclerotium (Fushen), Charred Chrysanthemum Flower (Ganju Tan), Uncaria Ramulus cum Uncis (Gouteng), Fermented Pinellia Paste (Banxia Qu), Mint Herb (Bohe), and Gardenia Fruit (Shanzhi). For internal wind movement due to Liver Yin deficiency, treat by nourishing fluids using Maixuan Decoction (Fuma Tang - Pulse Restoring Decoction) minus Ginseng Radix (Renshen), Zingiberis Rhizoma Recens (Shengjiang), and Cinnamon Twig (Guizhi), added with Egg Yolk (Jizihuang) and White Peony Root (Baishao). For clear orifices obscured by summerheat-heat, treat by clearing and leaching out dampness using Gypsum Fibrosum (Shigao), Lotus Petiole (He Geng), Mint Herb (Bohe), Antelope Horn (Lingyangjiao), Taro Stem (Tongcao), and Coix Seed (Yimi). For floating Yang due to Yin injury, presenting with toothache and tendon twitching, treat by anchoring and subduing using Donkey-hide Gelatin (Ejiao), Oyster Shell (Muli), Rehmannia Root (Shengdi), Ginseng Radix (Renshen), White Peony Root (Baishao), and Uncaria Ramulus cum Uncis (Gouteng). For headache due to internal wind, with cold tears and blurred vision, treat by moistening and nourishing using Lycium Fruit (Qizi), Polygonum Multiflorum Root (Shouwu), Poria Sclerotium (Fushen), White Peony Root (Baishao), Biota Seed (Baiziren), and Charred Chrysanthemum Flower (Ganju Tan). For buzzing in the head resembling insect chewing, named "Tianbaiyi" (Heavenly White Ant), insufflating Camellia Seed powder (Chazi mo) into the nose is effective. For white dandruff and itching on the head, decoct Linglingxiang (Linglingxiang) and Angelica Dahurica Root (Baizhi), mix with egg white, and apply externally.
Thunderhead Wind presents with swelling, pain, lumps, aversion to cold with high fever, and a sensation of thunderous vibration in the brain; use Qingzhen Tang (Clear and Shake Decoction) or Jielei Tang (Resolve Thunder Decoction). Thunderhead Wind affects the three Yang channels; do not overuse heavy cold-cooling agents to avoid damaging righteous Qi. Liu Wansu established Qingzhen Tang. Brain Wind presents with chills in the neck and back, and coldness at the Naohu acupoint (GV17); use Shensheng San (Divine and Sacred Powder). First Wind, caused by recent hair washing, with excessive sweating and aversion to wind; use Chuanxiong Wan or Baizhi Wan. Refer to the Headache section for other treatments.
