A patient contracted dysentery due to summerheat, presenting with fever, thirst, and mental restlessness. There was a sense of urgency in the abdomen and tenesmus (里急后重), with discharge that was red and white, thick, and sticky; this indicates damp-heat accumulation. Liu Yi San (Six-to-One Powder) was used with added Trichosanthis Radix (Huafen), Coicis Semen (Yiren), Menthae Haplocalycis Caulis (Bohegeng), Aurantii Fructus (Zhike), Poria Cocos with Red Cortex (Chiling), Paeoniae Radix Rubra (Chishao), and Moutan Cortex (Danpi). After the fever subsided and tenesmus decreased, Huafen, Bohe, and Danpi were removed, and Scutellariae Radix (Huangqin), Paeoniae Radix Alba (Baishao) [both wine-fried], Aquilae Lignum Preparatum (Weimuxiang), and Citri Reticulatae Pericarpium (Chenpi) were added. After several doses, the condition resolved.
A young child suffered from Jin Kou Dysentery (thirstless dysentery with inability to eat) in autumn, accompanied by fever and lower abdominal dragging pain. Initially, the stool was thick and red, followed by bloody water, occurring day and night without restraint. This was caused by heat pathogen obstructing the epigastrium, qi stagnation in the lower jiao, and forcing damage to the Ying (Nutritive) level. Initially, Zhike, Trichosanthis Semen (Gualouren) [both fried], Gardeniae Fructus Nigra (Heishanzhi), Chiling, Perillae Caulis (Sugeng), and Muxiang were used to guide heat downward and unblock rebellious qi. Subsequently, Baishao, Glycyrrhizae Radix et Rhizoma (Gancao) [charred black], Foeniculi Fructus (Huixiang), Zingiberis Rhizoma Preparatum (Paojiang), and Zizyphi Fructus Nigra (Heizharou) were used to soothe the middle and warm the lower jiao. Later, Nelumbinis Plumula (Shilian), Codonopsis Radix (Lushen), Poria Cocos (Fushen), Amomi Fructus (Sharen), Yiren, Rehmanniae Radix Preparata (Shuditan), Dioscoreae Rhizoma (Shanyao), Jujubae Fructus (Hongzao), and Oryza Sativa (Jingmi) were used to support Yang and harmonize Yin. Gradual regulation led to recovery.
Mr. Bao suffered from dampness-induced dysentery after continuous spring rain for over ten days, presenting with epigastric fullness and reduced appetite; the treatment focused on the Spleen. Ping Chen Tang was used with Gancao removed, adding Shenqu, Setariae Fructus (Guyaya) [both fried], decocted with Yimi. One dose brought relief. Paojiang and Sharen were then added, leading to cure.
The author's cousin developed dysentery in early autumn. Due to excessive consumption of Pear, Lotus Root, and Raphani Fructus (Laifu) for thirst, he experienced vomiting and diarrhea, mouth closure preventing eating, lethargy, and faint consciousness. The pulse was fine and nearly imperceptible, limbs were cold, eyes closed, and teeth clenched, making oral medication difficult. Fuzi Lizhong Tang was urgently administered, removing Renshen and Gancao. Prepared Chuan Fuzi (2 qian), Paojiang (2 qian), Banxia Preparata (3 qian), and Amomi Fructus Baikou (8 fen) were decocted. Using chopsticks to open the teeth and a spoon to administer, one full dose warmed the limbs, and porridge could be taken without vomiting. Zhike, Fuling, Zhigancao, and Guya were added to the previous formula. Two more doses stopped the dysentery. Xiangsha Liujunzi Tang was subsequently used for stabilization.
Sun suffered from chronic dysentery for years, inevitably damaging Kidney Yin. If only the Spleen is tonified without avoiding greasy and fishy foods, Spleen Yang may not recover, and Kidney Yin will further decline. Miuzhongchun's Pi Shen Shuangbu Wan was used, containing Renshen, Fuling, Shanyao, Corni Fructus (Shanyu), Cuscutae Semen Biscuit (Tusibing), Sharen, Myristicae Semen (Roukou), Psoraleae Fructus (Buguzhi), Paojiang, Photiniaefolii Fructus (Nanzhu), and Nelumbinis Semen (Lianshi), made into pills with paste. One dose showed effect.
Wang had chronic dysentery with fresh red blood and tenesmus, complicated by San Yin Malaria attacks. These were lingering Summerheat-Damp-Heat pathogens in meridians and fu-organs. Chronic dysentery damages the Kidney, and chronic malaria damages the Spleen. With both pathogens combined, the three Yin meridians of the foot are severely impaired, leading to debility. According to classic principles for "Heat rampant within," sour astringents and bitter dispersants were used. Prepared Magnoliae Officinalis Cortex (Zhinoupo), Scutellariae Radix Wine-processed (Jiu Huanglian) [each 5 fen], Mume Fructus (Wumei) [2 pieces], Gancao [charred black, 1 qian], Baishao, Chiling, Chenpi, Schizonepetae Spica Nigra (Heijingjie) [each 1.5 qian], and Polygoni Multiflori Caulis Recens (Xian Yejiaoteng) [5 qian] were used. Two doses stopped the dysentery completely.
Ms. Li suffered from dysentery from autumn to winter. The pulse was slow, and she could eat. Previous lifting and astringent treatments failed because the syndrome was not Qi sinking or slipping. Now, lower abdominal pain triggers discharge of thick filth; only warming and unblocking can cure it. Do not confuse it with Resting Dysentery (Xiuxi Li) and treat with tonification and retention. Fried Foeniculum Vulgare Fructus (Xiaohuixiang) [8 fen], Gingeris Rhizoma (Ganjiang) [8 fen], Aquilae Lignum Preparatum (Weimuxiang) [5 fen], Guangpi, Zhigancao [each 6 fen], Sharen [1 qian], Sanguisorbae Radix (Diuyu) [wine-fried, 8 fen] were used. Several doses stopped the symptoms.
Miss Zhang suffered from epigastric heat and pain in early winter, with bloating and bloody dysentery after eating. The pulse was fine and rapid, especially at the left Guan and Chi positions. Both Yin and Yang were injured. At an age when Tian Gui (menstruation) was approaching, this condition feared progression to consumptive weakness. Fried Trichosanthis Semen (Gualouren) [1.2 qian], Zhike, Huangqin, Baishao [both fried, 1.5 qian], Prepared Magnoliae Officinalis Cortex (Noupo) [5 fen], Diuyu [wine-fried, 8 fen], Rehmanniae Radix Preparata (Shengditantan) [1.5 qian] were used. Symptoms gradually decreased. Gualou, Huangqin, and Noupo were removed. Danggui [vinegar-fried], Cyperi Rhizoma (Xiangfu) [both 1.5 qian], and Red Yeast Rice (Hongqu) [1 qian] were added. Bloody dysentery became sparse. Liuwei Wan was taken later for stability.
Zhu, a youth, suffered from bloody dysentery from early spring to winter without cure. The Yin collaterals were long damaged. During autumn coolness, he caught malaria, where external pathogens may have mixed with food stagnation. Practitioners ignored meridians and recklessly administered Dahuang, Zhishi, and Corydalis Rhizoma (Yanhusu) to promote descending and resolve stasis, further depleting Yuan Qi. The pulse was deep, weak, and lacking spirit. In winter, he felt cold, ate little, and passed blood ten-plus times daily. The abdomen did not hurt, but there was slippery incontinence. Treatment adopted warming and astringing to secure True Yuan. Lushen, Fuling, Hei Gancao, Paojiang, Roukou, Ostreae Concha (Muli) [vinegar-processed], Shanyao [fried], and Terminalia Chebula Fructus Meat (Hezire) were used. One dose reduced symptoms; ten-plus doses stopped them completely.
A clan member suffered from painful dysentery in early spring, with fresh blood dripping after defecation, abdominal pain, and tenesmus. The pulse was large, mouth dry, and aversion to cold; exertion caused shortness of breath. Over sixty years old, chronic dysentery damaged Yin affecting Yang. Fever is strictly contraindicated. Following the Chuche Wan formula, Huanglian was removed. Wumei, Ejiao [roasted and dissolved], Danggui, Baishao, Shudi [all fried], Wuweizi, Muxiang [roasted], Zhigancao were decocted in Jingmi soup. Three doses thinned the dysentery and stopped bleeding. Wumei and Muxiang were removed. Renshen, [missing character, likely Huangqi], Chenpi, Fuling, Lianzi, Jiang were added. Ten-plus doses stabilized the condition.
Ms. Tan, over sixty, suffered from dysentery for over ten days, with persistent abdominal pain and tenesmus. Urination was difficult, pulse was surging, eyes were red, malar flush was present, and she suffered nighttime irritability and dry mouth. Treatments alternated randomly between Xianglian Wan, Sukeshang Tang, Dahuang, and Rougui, showing lack of coherence. Initially, Marantis Pulvis (Biqifen), Gardeniae Fructus (Shanzhi), Dendrobii Caulis (Shihu), Danpi, Chiling, Ophiopogonis Radix (Maidong), Baishao, Muxiang Juice, Zhike, Diuyu, and Juncus Effusus (Dengxin) were used. One drink reduced symptoms, and she could eat porridge. The formula was changed to Weimuxiang, Chenpi, Baishao, Danggui, Fuling, Diuyu, Plantaginis Semen (Cheqianzi), and Gancao Shao. Dysentery greatly reduced. However, abdominal pain moved to different locations, indicating stagnant deficient Qi. Wheat bran fried with Jiang and Congbai was wrapped in silk and applied hot. Pain ceased, and regulating medicine restored health.
