Yang, at the age of twenty, developed consumptive disease (Xu Sun), presenting with coughing and hemoptysis, panting and shortness of breath, fever, sweating, reduced appetite, and loose stools. The pulse was weak and rapid. This indicates damage in the upper jiao affecting the middle jiao (Spleen/Stomach). Tonifying Earth to generate Metal is the approach, though there are no fixed formulas. Si Jun Zi Tang (Four Gentlemen Decoction) was modified with Shan Yao (Dioscoreae Rhizoma) stir-fried with Shu Di Huang (Rehmanniae Radix Preparata) and Sha Ren (Amomi Fructus), Fu Shen (Poria Cum Radix), Wu Wei Zi (Schisandrae Fructus), Bai Shao (Paeoniae Radix Alba), Lian Zi (Nelumbinis Semen), and Xiao Mai (Tritici Fructus). After several doses, bleeding stopped, and panting and fever subsided. However, when the Yang Qi first begins to grow, if the condition does not worsen during solar term transitions, recovery can be expected.
The daughter of the Hu family presented with alternating chills and fever, cough, amenorrhea, poor appetite, emaciation, dry mouth, and insomnia. The pulse was deficient and rapid, indicating established consumptive signs. The Classics state: "Diseases of the Two Yangs originate from the Heart and Spleen; having unspoken grievances leads to amenorrhea in women." The Two Yangs refer to the Foot-Yangming Stomach. When Stomach Qi is deficient, grain intake is low, and blood generation fails, thus symptoms manifest in the Heart and Spleen. The Heart governs Blood, and the Spleen controls Blood. Emotional distress and daily worry consume Qi and Blood, leading to menstrual depletion. The treatment should nourish the source of transformation. Following Mr. Li Zai's method, Gui Pi Tang (Restore the Spleen Decoction) with Bai Zi Ren (Biota Seed) was administered in the morning, and Du Qi Wan (All-Qi Pill) with Gou Qi Zi (Lycii Fructus), Bai Shao, Suan Zao Ren (Ziziphi Spinosae Semen), and Bei Mu (Fritillariae Bulbus) in the evening. After two months, all symptoms resolved, menstruation resumed, and the patient recovered completely.
Mrs. Di had amenorrhea with labor-heat. The physician used blood-invigorating and menstruation-promoting herbs, resulting in panting, shortness of breath, palpitations, and spontaneous sweating. Cold-natured herbs were then used to clear heat, causing loss of appetite and emaciation, with alternating chills and fever. The pulse was weak, rapid, and hurried. This indicates damage in the lower jiao affecting the middle jiao. Urgently, Lu Shen (Codonopsis Radix), Fu Shen, Huang Qi (Astragali Radix), Zhi Gan Cao (Prepared Licorice Root), Bai Shao, Dang Gui (Angelicae Sinensis Radix), Wu Wei Zi, Suan Zao Ren, and Yin Chai Hu (Stellariae Radix) were prescribed. After four doses, symptoms gradually decreased. Shan Yao, Shu Di Huang Tan (Charred Rehmannia), Lian Zi, and Da Zao (Jujube) were added. This formula tonifies the Heart and Spleen while regulating the Lungs and Kidneys. Heat and cough ceased, and appetite returned. A month later, sudden lower abdominal pain occurred, and a fetus about three cun long was expelled, with half its head damaged and necrotic. I deeply blamed myself for not carefully examining whether the mother's tongue coating was green-black. Considering she had been amenorrheic for six months, it was known that the uterine blood was exhausted, preventing fetal growth. The fetus had been dry and black for months, likely embedded deeply due to severe wasting. Dozens of doses of blood-breaking and menstruation-promoting herbs failed to induce abortion. Waiting for Qi and Blood to regulate allows the necrotic membrane to detach and pass naturally. Otherwise, using San Leng (Curcumae Rhizoma), E Zhu (Curcumae Rhizoma), Niu Xi (Achyranthis Bidentatae Radix), and Tao Ren (Persicae Semen) on blood-depletion amenorrhea would lead to rapid death. This case serves as a warning against excessive purgation.
Mr. Li suffered physical strain from carrying loads, presenting with cough, chest pain, and lung damage. Huang Qi, Lu Shen, Fu Shen, Bai He (Lilii Bulbus), Bei Mu, Xing Ren (Armeniacae Semen), Dang Gui, Bai Shao, Gan Cao (Licorice Root), and Hong Zao (Red Jujube) were used. Improvement was seen after two doses. This is a modification of An Fei Tang (Peace the Lung Decoction), following the principle: "For lung damage, benefit the Qi."
Mr. Sui had Liver and Kidney Yin deficiency with chronic consumptive disease. After Winter Solstice, he had phlegm-cough with pink sputum, worsening significantly at midnight. This suggests floating Yang failing to anchor and Dragon Fire not subduing. At midnight, Yang moves, causing heat to flare upward, damaging the delicate Lung organ, leading to fever and panting. Recently, appetite decreased, cheeks flushed in the afternoon, and chills were felt. This indicates Yin damage affecting Yang, which cannot be treated with sour/astringent herbs like Yu Zhu and Shu Di Huang alone. Sweet herbs must tonify Yuan Qi, assisted by shell-type minerals to anchor Yang, hoping for storage and consolidation until Spring Equinox. Lu Shen, Shan Yao, Bai He, Gan Cao, Wu Wei Zi, Bai Shao, Mu Li (Ostreae Concha), Dan Cai (Mussel Meat), and E Jiao (Donkey-Hide Gelatin) were given. Symptoms stabilized after several doses.
Cousin presented with harsh cough and急促 breathing, pulse wiry and rapid. In autumn, this is interpreted as Fire insulting Metal, requiring nourishment of the source. Self-medication with Pear Paste was incorrect. In winter, alternating chills/fever, rebellious Qi, gray-red sputum indicated hyperactive Yang and deficient Yin, with Dragon/Thunder fire disturbing and poor winter storage. Approaching Spring Equinox, rising Earth Qi aggravates internal Qi, worsening cough/panting. Formula: E Jiao (melted), Shan Yao (stir-fried), each 2 qian; Yang Shen (American Ginseng), Shu Di Huang (stir-fried), Fu Shen, Ou Jie (Lotus Rhizome Node), each 3 qian; Chuan Bei Mu (stir-fried/powdered), 1 qian; Tian Xing Ren (stir-fried/powdered), 1.5 qian; Suan Zao Ren (stir-fried/powdered), 8 fen; Wu Wei Zi, 5 fen. Several doses were effective. Swallow nest soup was taken at dawn, Qiu Shi (Autumn Stone) soup at night to descend deficient fire and calm panting.
Cousin: The Lung governs exhalation, the Kidney governs inhalation. Kidney grasping Qi is deficient. At Jingzhe (Awakening of Insects), Yang Qi rises greatly. Guan and Chi pulses are slippery and wiry. Qi rebels via the Chong Mai (Penetrating Vessel). At midnight, Yang moves, causing panting, cough, sweating, and inability to lie down. This is failure of True Yuan to grasp. Repeated use of Ren Shen (Ginseng) for protection failed because Lung/Spleen were insufficient. Using Zhi Mu (Anemarrhenae Rhizoma) and Bai Guo Pi (Phellodendri Cortex), named "nourishing Kidney," cannot bring immediate peace. Following Ye Tianshi's anchoring method: Qing Qian (Lead), Mu Li (powdered), Fu Shen, Wu Wei Zi, Pao Jiang (Prepared Ginger), Yuan Zhi (Polygalae Radix, charred), Gu Zhi Bu (Psoraleae Fructus, salt-stir-fried). Three doses stopped panting and calmed Qi, improving appetite and reducing wiry pulse. Strong tonic paste completed the cure.
Mr. Wang suffered essence depletion from labor. Right Chi pulse was vigorous, indicating Water-Fire non-interaction. Afternoon chills/fever, hemoptysis, and fatigue required careful regulation. Otherwise, Fire burning Metal would cause worsening hemoptysis and panting. Wu Wei Zi, Shu Di Huang, Shan Yao, Fu Ling (Poria), Gou Qi Zi, Dan Pi (Moutan Cortex), Lu Shen, Bai Shao, Chuan Bei Mu, Yuan Zhi, Lian Zi were prescribed. Numerous doses resolved symptoms.
Sister: Chronic weakness, irregular menses, heat steaming from legs/knees. With Jueyin Wind-Wood governing the year and warm weather, dampness rose. Days of chills/fever, thirst, insomnia, palpitations, cough, and fright indicate Yin deficiency with blazing Heart Fire, compounded by Wood/Fire dominance. Qi leaks, failing to guard internally. Yang Wei Mai is abnormal, failing to support the framework. Bian Que said: "Yang Wei disease suffers chills and fever." Now defensive Qi is empty, feet cold, bones hot. First tonify Yuan Qi, resolve chills/fever, await fluid return to stop thirst. Lu Shen, Fu Shen, Mai Dong (Ophiopogonis Radix), Bai Shao, Dan Pi, Gui Ban (Testudinis Carapax et Plastrum), Shu Di Huang, Bai Zi Ren, Hong Zao, Zhe Zhi (Sugarcane Juice) were used. Three doses reduced chills/fever and thirst. Cold sensation in calves remained. Mai Dong and Gui Ban were removed; Shou Wu (Polygoni Multiflori Radix), Gou Qi Zi, Niu Xi (charred) added to strengthen extraordinary vessels. Two doses changed cold/fever pattern. Shou Wu, Gou Qi Zi, Bai Zi Ren removed; Lian Zi, Long Yan Rou (Longan Aril) added. Dozens of doses brought stability.
Mr. Gong: Young unmarried man, Yang damaged by labor. Chills/fever, bloody phlegm, cough with shortness of breath/vomiting foam, dizziness, poor appetite, emaciation, no recovery in half a year. Pulse deficient/rapid, right side lacking spirit. Lung Qi injured, Spleen Yuan exhausted. Regulate Yang and drain turbidity. Liu Jun Zi Tang (Six Gentlemen Decoction) with Shan Yao, Lian Zi, Nan Zao (Southern Jujube), Dan Jiang (Light Ginger) was used. Four doses stopped chills/fever and turbidity. Ban Xia (Pinelliae Rhizoma) removed; Bei Mu, Fu Shen, Wu Wei Zi added. Cough decreased, appetite improved, rapid pulse slowed. Yi Mi (Coicis Semen), Qian Shi (Euryales Semen), Huang Qi, Dang Gui, Bai Shao decocted and made into pills. Later, self-administered rich/cloying Shu Di Huang pills caused poor appetite/loose stools. Eating beef tripe caused persistent diarrhea/dysentery. Pressure to marry led to hoarse voice, afternoon chills/fever, frequent stools. Pulse more deficient/rapid. Spleen damage from poor appetite, Kidney damage from dysentery. Yin/Yang depleted. Rescue difficult. Tentatively tonify Lung/Spleen to secure Kidney. Lu Shen, Fu Ling, Zhi Gan Cao, Bai Shao, Shan Yao, Yi Zhi Ren (Alpiniae Oxyphyllae Fructus), He Zi (Terminalia Chebulae Fructus), Wu Wei Zi, Lian Zi, Da Zao. Symptoms stabilized. Qi sinking causes dysentery; body feels cold; hands/feet/mouth hot; dry mouth upon waking. This is Yang deficiency generating cold, Yin deficiency generating heat, fluids not ascending. Morning: Bu Zhong Yi Qi Tang (Tonify the Middle and Augment the Qi Decoction) minus Chai Hu (Bupleuri Radix) and Dang Gui, plus Yi Zhi Ren, Fu Shen. Evening: Shu Di Huang Tan, Wu Wei Zi, Suan Zao Ren, Bai Shao, Bei Mu, Yi Mi, Mai Dong (all stir-fried), mixed with Zhe Zhi. Chills/fever lightened, dysentery persisted. Tao Hua Tang (Peach Blossom Decoction) plus Ren Shen, Fu Ling, Wu Wei Zi, Wu Mei (Mume Fructus) given. Warmly secure Lower Jiao, but dysentery continued due to prolonged intestinal lubricity loss and Qi deficiency unable to accept warming/astringents. Throat pain, hoarseness, white foam expectoration occurred due to post-Spring Equinox temperature rise. Formula adjusted: Ren Shen, Fu Ling, Lian Zi, Shan Yao tonify Spleen; Wu Wei Zi, Bai Shao astringe Lung; Sha Shen (Glehniae Radix), Jie Geng (Platycodonis Radix) clear throat; Shu Di Huang Tan, Shi Hu (Dendrobii Caulis) nourish Yin; He Zi, Mu Li (vinegar-treated) astringe lower orifice.
Mrs. Xie: Post-metrorrhagia and leukorrhea, steaming heat, dizziness, toothache, belching with food stagnation, frequent diarrhea. Initially Chong/Ren meridians injured, Yin deficiency generating fire. Physician used Qing Qian (Lead) to anchor, exacerbating deficient fire and sinking Middle Qi. Sweat poured from hairline; gas escaped from lower body. Ignoring Yin deficiency and unrestrained Liver Yang, symptoms manifested as dizziness above, steaming diarrhea below, and vomiting in the middle. Treatment: Harmonize Yang, extinguish wind, assist Spleen transport. Otherwise, tonification causes bloating, Kidney nourishment hinders Spleen, leading to emaciation and lower-to-middle damage. Gou Qi Zi Tan, Gan Ju (Chrysanthemi Flos), Mu Li Fen, Bai Shao, Shan Zhi (Gardeniae Fructus), Shen Qu (Medicated Leaven) (all stir-fried), Ban Xia (green salt-stir-fried), Fu Shen, Dan Pi, Nen Sang Ye (Mulberry Leaf), Fu Xiao Mai (Triticum Fructus) decocted. Three doses stabilized symptoms. Shan Zhi, Shen Qu removed; Bie Jia (Trionycis Carapax), Shan Yao, Shu Di Huang Tan added. Steaming heat resolved.
Cousin: Alcohol injury to Middle, damp-heat accumulation. Cough, tidal fever, abdominal pain, vomiting/diarrhea. Pulse deep/slight-rapid, left particularly choppy. Continued drinking leads to alcohol consumption. Sheng Bai Zhu (Atractylodis Macrocephalae Rhizoma), Fu Ling, Yi Mi, Ju Hong (Citri Reticulatae Pericarpium), Hui Xiang (Foeniculi Fructus), Zhi Ban Xia, Zhi Li Zi (Ardisiae Fructus), Sha Ren, Xiang Lian (Lotus Seed) (stir-fried). Several doses stabilized symptoms. Ge Hua Jie Cheng Tang (Kudzu Flower Decoction to Resolve Intoxication) minus Qing Pi (Citri Reticulatae Pericarpium Viride), Gan Jiang (Zingiberis Rhizoma), plus Zhi Ban Xia. Dozens of doses cured.
Mr. Shen: Young, weak, afternoon fever, harsh cough, dizziness, poor appetite, frequent vomiting of bitter/sour turbid foam, alternating loose/constipated stools. Pulse wiry/rapid, Right Chi forceful. Hyperactive Ministerial Fire, failure of dispersion/assimilation, turbid phlegm descending, dream emission with burning soles. First drain damp-heat to extinguish Dragon/Thunder. Huang Bo (Phellodendri Cortex, wine-stir-fried), Ban Xia (green salt-prepared), Fu Ling, Yi Mi, Wu Yu (Evodiae Fructus, salt-soaked), Yuan Zhi, Shan Zhi, Ze Xie (Alismatis Rhizoma), Ju Hong. Six doses lowered phlegm-fire, stopped emissions. Wu Yu, Shan Zhi removed; Sheng Bai Zhu, Wu Wei Zi, Mu Li Fen added. Symptoms reduced, regulation achieved.
Nephew: Mental fatigue from reading, afternoon chills/night heat, sweating/cough/poor appetite. Pulse deficient, Right Chi wiry/large. Childhood consumption. Heart/Spleen/Lung affected, involving Kidney Yin. Following Li Zai's method: Morning, Bu Zhong Yi Qi Tang minus Sheng Ma (Cyathulae Radix), plus Fu Ling, Suan Zao Ren, Xiao Mai. Evening, Liu Wei Di Huang Wan (Six-Ingredient Pill with Rehmannia) minus Shan Zhu Yu (Corni Fructus), plus Bai Shao, Bie Jia, Wu Wei Zi. Ten-plus doses stopped chills/fever and restored spirit.
