(The transverse bone above the sacrum) is collectively governed by the Foot-Shaoyin Kidney Meridian (Jingmai). The *Lingshu* states: The divergent branch of the Foot-Shaoyin Kidney Meridian runs parallel upward to below the Pericardium, and externally penetrates the lumbar spine. If the meridian Qi rebels, there is vexation in the heart; if Excess, there is strangury with retention of urine; if Deficiency, there is lumbago. It also states: If Kidney Qi is excessive and anger continues without stopping, it injures the Zhi (Will), and injury to the Zhi leads to forgetfulness of previous words, inability to flex and extend the lumbar spine, and a withered appearance of the hair at the pubic region, potentially leading to death in the late summer season. The *Suwen* states: North Wind arises in winter, causing disease in the Kidneys, with Shu points located in the lumbar and thigh regions. It also states: Kidney disease internally resides in the lumbar spine and marrow, and externally manifests in the Xi Gu (valleys between muscles and bones) and knees. Note: Xi Gu refers to the gastrocnemius muscle. It further states: When the Kidney pulse is firm and long, and the complexion is yellow with redness, one should expect "Zhe Yao" (lumbar pain resembling a broken waist). It also states: In Kidney Heat diseases, patients first experience lumbago and calf soreness, bitter thirst with frequent drinking, fever, inability to lift the lumbar spine, dry bones, and reduced marrow, developing into Bone Wei (bone atrophy/weakness). It also states: The manifestation of Kidney Cough is that coughing causes mutual traction and pain in the lumbar spine. It also states: The waist is the mansion of the Kidney; if turning and shaking are impossible, the Kidney Qi is about to fail. Survival depends on strength (Yang Qi), while loss of strength leads to death. It also states: Forceful exertion injures Kidney Qi, causing the Gao Gu (high bones, i.e., prominent vertebrae) to deteriorate. Note: Gao Gu refers to the high bones of the waist. Zhu Danxi said: All meridians penetrate the Kidney and connect to the lumbar spine. Once Kidney Qi is deficient, various types of lumbago—such as those caused by wind-cold-dampness, cold accumulation, heat retention, blood stasis, Qi stagnation, water accumulation, trauma, or emotional strain and overwork—will emerge continuously. The *Sanyin Fang* states: In Ren Zhuo (Kidney Dampness) disease, the body feels heavy, diet remains normal, urination is free, but there is cold pain below the waist, feeling as heavy as carrying five thousand coins. Treatment should focus on draining dampness and using warming dispersal methods, primarily with Shen Zhuo Tang.
It also belongs to the Foot-Taiyang Bladder Meridian. The *Lingshu* states: The Foot-Taiyang Bladder Meridian runs alongside the spine and reaches the middle of the waist. The *Suwen* states: Taiyang Malaria causes lumbago, heaviness of the head, and cold sensation arising from the back. Li Dongyuan stated: When Taiyang Qi is deficient, pathogenic factors invade. Pathogens include wind, heat, cold, and dampness. If cold-dampness predominates over wind-heat, and lumbago results from sexual exhaustion injuring the Kidney, this is due to Yang deficiency failing to move Qi, requiring tonification of Yang. For individuals consuming rich foods who take herbal decoctions for long periods and engage in intercourse while drunk, damaging True Yin, leading to Kidney Heat, lumbago, and spinal pain with inability to lift the body, prolonged cases result in reduced marrow and dry bones. This is due to Yin deficiency generating internal heat, requiring tonification of Yin. He also stated: Fang Feng (Saposhnikovia Root) is acrid and warm, with thin flavor and qi, floating and ascending Yang. For symptoms such as spinal pain and neck stiffness, inability to look back, waist feeling like breaking, and neck feeling like pulling, which are symptoms of the Hand and Foot Taiyang meridians, Fang Feng is precisely indicated.
It also belongs to the Foot-Jueyin Liver Meridian. The *Lingshu* states: When the Foot-Jueyin Liver Meridian is disturbed, it causes lumbago, inability to flex and extend, hernia in men, and swelling of the lower abdomen in women. The *Suwen* states: The Jueyin Meridian causes lumbago, with the waist feeling tight like the string of a drawn bow. Zhu Danxi said: Softness of the waist is due to latent heat in the Kidney and Liver, treated with Huang Bo (Phellodendron Bark) and Fang Ji (Stephania Root).
It is also associated with the Foot-Yangming Stomach Meridian and Shaoyang Gallbladder Meridian. The *Lingshu* states: For lumbago with coldness at the painful site, select Foot-Taiyang and Foot-Yangming; for heat at the painful site, select Foot-Jueyin; for inability to flex and extend, select Foot-Shaoyang. The *Suwen* states: Shaoyang causes lumbago feeling like needle pricks in the skin, slowly unable to flex and extend, and unable to look back; Yangming causes lumbago, inability to look back, hallucinations when looking back, and propensity for sadness. It also states: Rouli Mai (Muscle-Li Channel) causes lumbago where coughing makes tendons contract tightly; puncture Rouli Mai at two points, located outside Foot-Taiyang and behind Juegu of Foot-Shaoyang. It also states: Tongyin Mai causes lumbago with pain feeling like a small hammer lodged inside, accompanied by郁结 swelling. Note: Tongyin is the divergent branch of Foot-Shaoyang.
It also belongs to the Foot-Taiyin Spleen Meridian. The *Suwen* states: San Mai (Scattered Channel) causes lumbago with heat; severe heat leads to irritability, and a sensation of a horizontal log under the waist; in severe cases, incontinence occurs. Note: San Mai is the divergent channel of Foot-Taiyin.
