(For this syndrome, it is difficult to treat if the patient is over sixty years old; it is difficult to treat if the stool resembles sheep dung; it is difficult to treat if white froth is vomited; if there is a burning sensation and pain in the chest and diaphragm as if cut by a knife, the prognosis is poor and considered fatal.)
Fanwei (Regurgitation) and Yige (Dysphagia with obstruction) are fundamentally caused by the depletion of blood, leading to dryness and withering of the stomach cavity. If the withering is located in the upper jiao, water and drinks can pass down, but food is difficult to enter; this is called Yige (Dysphagia). If the withering is located in the lower jiao, food may enter but is regurgitated after a long time; this is called Fanwei (Regurgitation). Both syndromes involve pathology in the region of the diaphragm, hence they are collectively referred to as "Ge" (Obstruction) syndromes. Generally, these conditions arise from a deficiency of Qi and Blood, compounded by emotional factors such as grief, excessive thinking, and worry, which injure the Spleen and Stomach and gradually deplete the blood. Qi stagnation generates Phlegm; the Phlegm obstructs and blocks the pathways, causing Qi to rebel upward instead of descending normally. This hinders the passage for food and drink, making ingestion difficult, which leads to the formation of Yige. If the Spleen and Stomach are deficient and injured, their transportive and transformative functions fail, unable to properly digest grains and transform them into essence. This results in morning food being vomited in the evening, or evening food vomited in the morning. Although food enters the Stomach, it is subsequently regurgitated, which leads to the formation of Fanwei. Both syndromes originate from pathology in the diaphragmatic region, thus they are collectively named Ge. The difficulty in treating this condition lies in the therapeutic dilemma: if one attempts to strengthen the Spleen and resolve Phlegm, drying agents may damage the Body Fluids; if one attempts to nourish Blood and generate fluids, moistening agents may hinder the transportation function of the Middle Jiao. Therefore, if Yin deficiency with Fire hyperactivity is identified, nourishing Blood should be prioritized urgently; if Spleen injury with Yin abundance (Cold-Damp) is identified, warming and tonifying should be the primary approach.
