Stroke (Zhong Feng)
Patients with stroke exhibit a warm body, excessive salivation in the mouth, sudden collapse, and inability to speak.
Urgent use of drugs to open the orifices is required; resolving phlegm and smoothing qi should be the primary treatment principle.
Plague (Wen Yi)
During plague epidemics, people suffer from similar symptoms due to invasion by abnormal external pathogens of the four seasons.
Treatment must be differentiated according to spring, summer, autumn, and winter; do not confuse warm diseases with cold diseases.
Heat Stroke (Zhong Shu)
Heat stroke differs from heat exhaustion. Heat exhaustion occurs when pedestrians are exposed to the sun.
Illness contracted by avoiding heat in deep halls is called "heat stroke." Summer-heat is divided into cold and hot types, which differ in nature and should not be treated indiscriminately.
Dampness Invasion (Zhong Shi)
Dampness invasion results from both internal injury and external exposure: externally affected by wind and rain, internally caused by excessive alcohol consumption.
It manifests as generalized swelling and fullness, and whole-body pain. Treatment should promote urination, strengthen the spleen, and induce sweating to dispel dampness.
Spleen and Stomach (Pi Wei)
The Qi of the spleen and stomach must be harmonious and balanced. The stomach governs reception and digestion, while the spleen governs transformation and transportation.
Do not allow warming or cooling medications to lose their balance, as this damages Yuan Qi (Original Qi), making the disease difficult to cure.
Phlegm and Fluid Retention (Tan Yin)
Phlegm often arises from heavy dampness, where body fluids coagulate and ascend or descend through the Upper, Middle, and Lower Jiao.
Phlegm is involved in almost all diseases; treating it according to symptoms is of immense value.
Cough (Ke Sou)
"Cough" refers to sound without phlegm, while "Sputum" refers to phlegm without sound.
When both phlegm and sound are present, it is called cough. Resolving phlegm and regulating cough leads to natural peace.
Rapid Breathing (Chuan Ji)
Rapid breathing requires differentiation between excess and deficiency patterns of the Lung. It is often accompanied closely by cold or heat pathogens.
Various causes such as phlegm-fire can lead to panting; treatment should not be hesitant.
Wheezing and Hoarseness (Xiao Hou)
Wheezing and hoarseness result from cold-phlegm accumulation in the lung orifices, causing severe panting and difficulty in daily activities.
Resolve phlegm, descend fire, and add conditioning measures. Without a good prescription, the condition will not stabilize.
Malaria (Nue Ji)
Malaria is caused by irregular wind and summer-heat pathogens, characterized by alternating chills and fever.
New-onset malaria can be dispersed or intercepted; chronic cases benefit from combined tonification.
Dysentery (Li Ji)
Dysentery is caused by damp-heat accumulation and Qi stagnation. Bloody dysentery injures the Blood level, while white dysentery injures the Qi level.
Mixed bloody and white dysentery indicates injury to both Qi and Blood; treatment should first clear heat, regulate Qi, and promote bowel movement.
Diarrhea (Xie Xie)
Profuse diarrhea is caused by dampness injuring the Spleen. Treatment involves drying dampness, promoting urination, and tonifying Spleen deficiency.
For chronic diarrhea leading to intestinal laxity and slipping, astringents should be used to lift Zheng Qi (Upright Qi).
Cholera-like Disease (Huo Luan)
This condition presents with vomiting, diarrhea, heart and abdominal pain, muscle cramps, chills/fever, and heaviness of the head and body.
It is caused by a combination of internal injury and external contraction. Rice soup should be avoided initially.
Vomiting (Ou Tu)
"Ou" refers to retching without substance, while "Tu" refers to vomiting substance. Cold or heat injuring the Spleen causes food to be expelled.
Treatment harmonizes the Stomach, clears fire, and resolves phlegm-saliva, primarily using Ban Xia (Pinellia Rhizome) and Sheng Jiang (Fresh Ginger).
Reverse Flow of Food (Fan Wei)
Reverse flow of food and dysphagia are similar conditions, often caused by heat binding in the Yang Ming channels leading to irregular vomiting.
Avoid desiccating agents that may exacerbate evil; nourish Blood and generate fluids, and regulate the Stomach Qi.
Hiccup/Cough Reversal (Ke Ni)
Reversal cough involves Qi rushing upward producing sound, continuous and unceasing, attributed to Fire pathogen conflict.
All upward rushing Qi belongs to Fire; resolving phlegm and descending Qi brings peace.
Acid Regurgitation (Tun Suan)
Acid regurgitation is caused by damp-heat lingering in the stomach opening, creating acid water that stings the heart area.
Treatment resolves phlegm, clears fire, and soothes Liver Qi. A vegetable-based diet aids recovery.
Fullness and Obstruction (Pi Man)
Fullness and obstruction refer to chest and diaphragm blockage, causing persistent stuffiness and discomfort.
Treatment should slowly harmonize using one method of dispersing and one of tonifying; avoid purgatives that harm healthy Qi.
Distension and Fullness (Zhang Man)
Distension feels empty like a drum, with no swelling in the limbs but suffering in the abdomen.
Avoid sour or purgative drugs that damage Heavenly Harmony; the main strategy is to subdue the Liver and tonify the Spleen.
Edema (Shui Zhong)
Edema is fundamentally dampness rooted in the Spleen. Generalized edema is essentially a deficiency pattern.
Treatment promotes urination, strengthens the Spleen, and smooths Qi. Harsh purging of water makes the disease hard to cure.
Accumulation and Gathering (Ji Ju)
There are five types of Accumulation (Ji) and six types of Gathering (Ju), each associated with specific Zang-Fu organs.
Left-sided issues relate to Blood, right-sided to Qi, and central to Food retention and Phlegm. Dissolve them slowly without haste.
Five Jaundices (Wu Dan)
The five jaundices are always caused by dampness and heat. Symptoms include generalized yellowing, sweating difficulties, and bloody urine.
Clearing heat and promoting urination eliminates dampness naturally; this is the primary treatment.
Blood Loss (Shi Xue)
Blood loss is generally due to Heat, where Excess Yang and Deficient Yin cause blood to move recklessly.
Tonify Yin, clear fire, and suppress Yang to guide blood back to its vessels, stopping the bleeding.
Mania and Depression (Dian Kuang)
Depression (Dian) manifests as abnormal laughter or behavior; calming the Spirit and nourishing Blood is an effective formula.
Mania (Kuang) is caused by excessive Phlegm-Fire; treatment soothes the Liver, clears fire, and resolves phlegm.
Evil Influence (Xie Sui)
Evil influences rarely affect robust individuals; they invade those with weak Shen (Spirit) and Qi.
Strengthening Zheng Qi causes evil to retreat; praying to ghosts is futile.
Headache (Tou Tong)
Headaches require detailed differentiation among the twelve meridians, with specific medications for each.
Severe headache involving the entire head and cold extremities indicates True Headache, a critical condition.
Ear Disorders (Er Bing)
Right ear deafness in men is due to sexual indulgence; left ear deafness in women is due to anger.
Deafness in both ears is caused by rich foods; tonifying deficiency and smoothing Qi cures the disease.
Nasal Disorders (Bi Bing)
Loss of smell in nasal disorders is due to latent phlegm-fire in the Lungs.
Congestion upon exposure to wind-cold is best treated by clearing Metal (Lung) and descending fire.
Mouth and Tongue (Kou She)
Mouth and tongue ulcers indicate Heart Heat; cool formulas extinguish the fire gradually.
Bitter taste indicates Liver Heat transferring to the Gallbladder, caused by indecisiveness.
Teeth (Ya Chi)
Toothache is caused by damp-heat and Stomach Fire, or wind and parasites.
Clear fire, dry dampness, and eliminate parasites; topical or oral application is safe.
Eyes and Vision (Yan Mu)
The corners of the eyes belong to the Heart, whites to the Lungs, and eyelids to the Spleen.
The pupil belongs to the Kidney, and the iris to the Liver Wood; the essence of the five Zang organs is reflected in the eyes.
Throat (Yan Hou)
Throat bi (obstruction), also known as tonsillitis, is often caused by excess wine, sex, or emotions.
Phlegm-fluid rising upward causes swelling and pain; dispelling wind and clearing fire restores harmony.
Heart Pain (Xin Tong)
Heart pain is actually epigastric pain; clearing heat and resolving depression yields spiritual efficacy.
Pain caused by worms or Qi stagnation requires careful pulse diagnosis.
Abdominal Pain (Fu Tong)
Abdominal pain can be Heat or Cold, involving fixed blood, food retention, or damp-phlegm.
Intermittent pain suggests Heat; constant dull pain suggests Cold.
Lower Back Pain (Yao Tong)
Lower back pain is not solely due to Kidney deficiency; analyze the three causes and five types.
Treatment may involve warming, cooling, sweating, or purging, combined with Kidney tonification.
Hernia (Shan Qi)
Seven types of hernia must be analyzed: cold, water, sinew, blood, Qi, fox, and atrophy.
Damp-heat inside due to cold causes阴肿 (yin swelling) and sharp lower abdominal pain.
Beriberi (Xiang Gang Jiao)
Swollen feet are called damp beriberi; non-swollen feet are dry beriberi.
Numbness is due to wind, pain is due to cold, and swelling is due to dampness, requiring differentiation and drainage.
Bi Syndrome Pain (Bi Tong)
The five Bi syndromes cause pain and numbness in the limbs due to wind, cold, and dampness invasion.
Dispersing wind-cold and removing dampness ensures pain does not persist.
Wasting-Thirst (Xiao Ke)
Wasting-thirst is divided into upper, middle, and lower types. Upper involves the Liver channel, middle the Stomach.
Lower involves the Kidney. Treatment differs based on appetite (able to eat vs. unable to eat).
Turbid Urine (Zhuo Zheng)
White turbid urine indicates Kidney deficiency; red indicates Heart deficiency with Heat.
Red turbidity requires clearing the Heart; white requires tonifying the Kidney. Resolve phlegm and dry dampness.
Nocturnal Emission (Yi Jing)
Nocturnal emission is due to Heart and Kidney deficiency; tonifying essence and Kidney with astringents is appropriate.
If damp-heat injures the Spleen causing leakage, regulate the Spleen and remove dampness to stop emissions.
Strangury (Lin Zheng)
The five stranguries involve Heat in the Bladder, causing dribbling and painful urination.
Blood, stone, paste, fatigue, and meat stranguries are treated by regulating and clearing heat.
Urinary Retention (Xiao Bian Bi)
Urinary retention has five types: Qi, Blood, Phlegm, Heat, Wind, and Stone.
Emetics are suitable to lift Qi; once the upper orifice opens, the lower orifice flows freely.
Constipation (Da Bian Bi)
Constipation has excess and deficiency types, including Yin and Yang binding.
Use attacking or tonifying methods with warm or cool herbs, matching the syndrome precisely.
Hemorrhoids and Fistula (Zhi Lou)
There are twenty-four names for hemorrhoids and fistulas, caused by wine, sex, Qi, wind, and food.
Unbroken lesions are hemorrhoids; broken ones are fistulas. Treat by dispelling wind, drying dampness, and detoxifying.
Intestinal Bleeding (Chang Feng/Zang Du)
Intestinal wind involves bright red blood before stool.
Visceral toxin involves dark, turbid blood after stool. Cold and heat patterns differ.
Various Worms (Zhu Chong)
Worms arise from damp-heat, with unclear shapes inside the abdomen.
If they attack the heart and abdomen causing pain, either calm or purge them to restore peace.
Women's Diseases (Fu Ren)
Women belong to Yin and are prone to Qi stagnation, which is hard to treat.
Regulating Qi and menstruation is key; Xiang Fu (Cyperi Rhizoma) is a miraculous herb for women.
Conception (Zhong Zi)
Childlessness is sorrowful, but immortals have secret recipes.
Preserving a single point of goodness in the heart ensures abundant descendants.
Pregnancy (Ren Shen)
Maternal illness causes fetal movement; curing the mother heals the fetus.
If the fetus is unstable due to maternal illness, stabilizing fetal Qi ensures maternal safety.
Childbirth (Chan Yu)
Women in labor should not panic; frequent movement is better than lying in bed.
Delivery happens naturally when ripe; if medication is needed, proceed calmly.
Postpartum Conditions (Chan Hou)
Postpartum conditions require supporting deficiency; promoting lochia flow is appropriate.
Various febrile conditions exist; do not lightly induce sweating or purging.
Pediatrics (Xiao Er)
Ten childhood diseases originate from food injury, leading to fever and various illnesses.
Early digestion promotion prevents convulsions, rashes, and phlegm-food retention.
Convulsion (Jing Feng)
Acute and chronic convulsions differ; chronic requires warming and tonifying, acute requires attacking.
Chronic Spleen wind is a critical condition, wasting medical effort.
Glandular Disease (Gan Ji)
The five glandular diseases involve the Heart, Liver, Spleen, Lung, and Kidney. Differentiate cold/heat and fat/thin.
Large belly with visible veins and emaciation require frequent administration of Spleen-harmonizing and accumulation-resolving herbs.
Mass Disease (Pi Ji)
Masses arise between skin and fascia, formed from Liver channel blood stasis.
Growing like pig lungs, they are fatal without a divine prescription.
Smallpox and Measles (Dou Zhen)
Smallpox and measles originate from fetal toxins. Mild fever indicates slow progression; severe fever indicates rapid progression.
Smallpox requires mild warmth; measles require clearing. Adapt flexibly.
Carbuncle and Ulcer (Yong Ju)
Back carbuncles arise from Qi condensation. First defeat toxicity, then support the interior.
Protruding indicates Yang, sunken indicates Yin. Yin is heavier and requires careful conditioning.
Scrofula (Luo Li)
Scrofula appears connected along the neck, formed by deficiency-labor and Qi stagnation.
Tonify Qi and nourish Ying, resolve stagnation; internal resolution shows medicinal efficacy.
Malignant Carbuncle (Ding Chuang)
There are thirteen types of malignant carbuncles, caused by heat-toxicity and evil wind.
Externally puncture and apply medicine; induce sweating to drive toxicity outward.
Leg Sores (Shuang Chuang)
Leg sores combine damp-toxicity and wind-heat, causing swelling, rotting, and cracking.
Remove damp-heat and kill parasites; wash with green onion and pepper decoction before applying paste.
Vitiligo (Dian Feng)
Vitiligo and purple vitiligo are similar to wind patterns; sweat spots are alike.
Internally take wind-dispelling pills/powders; externally apply powder based on appearance.
Various Sores (Zhu Chuang)
All nameless swollen toxic sores rise suddenly with unbearable pain.
Apply excellent medicines frequently to prevent disaster.
Goiter (Ying Liu)
Goiters are mostly due to Qi and Phlegm knotting in the neck.
Blood goiters are the most malignant and should not be grouped with others.
Fractures and Injuries (Zhe Shang)
Fractures and bruises cause unbearable pain; blood stasis may rush to the heart.
Dispel stasis and relieve pain early; tonify deficiency and set bones slowly.
Broken Wind (Po Shang Feng)
Broken wind has many manifestations; sweating, purging, and harmonizing bring peace.
Dispelling wind achieves efficacy; do not treat it lightly.
