There are such cases. I regret that the records remain vague and fail to hit the key point. "Re wind" (Re Feng, heat transforming into wind) refers to a condition where the patient suffers from dizziness of the head and eyes, pain in the mouth, dryness of the nose, emission of hot air, slight aversion to wind, and intermittent fever. Although acrid-cool methods are used, they cannot resolve it. Meng Shen stated: Patients with re wind should consume cow's milk, as its flavor is sweet and cold, and its nature is moistening, which can eliminate muscular heat and extinguish wind naturally. Seeking among herbal materials, Reeds Rhizome (Lugen), Sugarcane Juice (Zhe Jiang), and Pear Juice (Li Zhi) share similar properties and flavors, aligning with the principle in *Neijing*: "When wind invades internally, treat with sweet and cold." When Liver Yang transforms into wind, it rebels against the Spleen and Stomach regions; fluids accumulate into Phlegm, flowing through the collaterals of the Liver and Gallbladder, causing left-sided numbness and chest diaphragm fullness—this is the origin. Since both wind and fire have an upward nature, symptoms such as rising fire, rebellious Qi, and epistaxis occur. This arises from irregular eating and drinking, fatigue, and depression, accumulating over time, not occurring overnight. The treatment method requires clearing Liver fire and strengthening Spleen Qi, which cannot be achieved in a short time. Herbs include Antelope Horn (Lingyang Jiao), Tangerine Peel (Juhong), Atractylodes Macrocephala (Baizhu), Immature Bitter Orange (Zhishi), Gastrodia Elata (Tianma), Pinellia Ternate (Banxia), Poria (Fuling), Licorice Root (Gancao), and Ophiopogon Japonicus (Maidong).
Zhang Shiwan treated a patient who had sweating on only one side of the body (Pian Ju), a pulse that was not soft, and occasional intermissions. This indicated excess Liver Yang and insufficient Stomach Yin, leading to thick phlegm and turbid fire disturbing the interior and obstructing the exterior. Although currently stable, the ancients advocated treating disease before it arises rather than after it has manifested. Wise individuals perceive the significant from the subtle and should certainly pay attention to regulation and care. Herbs included Ginseng Root (Renshen), Dendrobium Nobile (Shihu), Southern Jujube (Nanzao), Pinellia Ternate (Banxia), Poria (Fuling), Honey-fried Licorice Root (Zhigancao), Ophiopogon Japonicus (Maidong), Moutan Cortex (Danpi), and Wheat (Xiaomai).
Huang Lvsu said: Sansheng Decoction (Sansheng Yin) is excellent for cold-type stroke syndromes. However, if there is rebellious deficiency fire and obstructing hot phlegm causing sudden collapse, resembling stroke, Aconite Root (Wutou) and Prepared Aconite Root (Fuzi) are not suitable. Li Zhongzi (Lizhai) treated Vice Minister Wang, whose deficiency fire moved recklessly and carried phlegm. He urgently administered child urine (Tong Niao), after which the patient's mental clarity returned immediately. (See Jiang Xuan's notes.) My cousin Lvzhong, in his prime, due to mental labor and depression, suddenly collapsed with rising phlegm, urinary incontinence, strabismus, and prolonged unconsciousness, resembling stroke. He revived after being given child urine. These cases are caused by fire carrying phlegm and absolutely contraindicate Sansheng Decoction; even Ginger Decoction is unsuitable. (Xu Lingtai's note: Not only is Sansheng Decoction unsuitable, but herbs like Angelica Sinensis (Danggui) and Goji Berry (Gouqi) are also inappropriate. Refer to my case of Gu Tingquan.) Although both present with sudden collapse, the etiologies differ and must be carefully examined. *Taiping Guangji* records that Liang Xin diagnosed a court official, stating: "Wind-phlegm has penetrated deeply; please return home quickly." The official then saw Doctor Gao in Yangzhou. Zhao E diagnosed him and agreed with Liang Xin regarding the critical nature of the illness, but suggested only one method: eat sand pears without limit, chew them, or squeeze the juice if chewing is difficult. After returning home and following this method for ten days, he recovered. This is also evidence of reducing fire and eliminating phlegm. (Xu Lingtai's note: *Zisheng Jing* also states: All strokes arise from excessive heat in the heart and abdomen.)
Xu Lingtai said: There are many people who die suddenly, for various reasons. Among them, seven or eight out of ten can be saved, while only two or three out of ten are incurable. It is merely the lack of a good physician at the moment that leads to these unjust deaths. If a person has no internal or external disease, eats and moves normally, yet dies suddenly, their zang-fu organs and collaterals originally had no pathology. Suddenly contracting external pathogens, such as foul winds, filth, evil spirits, or toxic plagues, can block the airways. Unable to move temporarily, the Great Qi becomes obstructed, leading to confusion and stupor. Over time, obstruction accumulates, tightening like a rope around the neck; when Qi ceases, death occurs. If a physician knows the cause and applies appropriate treatment to unblock Qi and expel pathogens, immediate recovery ensues. Another cause of sudden death is excessive saliva and phlegm blocking the airway; unblocking Qi and descending phlegm restores consciousness, known as Phlegm Syncope (Tan Jue). Good physicians can treat all the above conditions, except for organ failure (Zang Jue). If a person has exhausted their essence and spirit due to mental labor, improper diet, sexual excess, or frequent anger, leaving only a thread of True Yuan intact, they may appear normal. A slight trigger can cause this remaining Yuan Qi to cut off instantly, leading to Qi depletion and Spirit separation, resulting in death within moments. Since it is incurable and untreatable, this is the most acute and untreatable form of sudden death. Encounters with ghosts or karmic retribution are strange events outside the scope of disease.
Dingfeng Wine (Wind-Calming Wine) nourishes Blood and extinguishes wind. Patients with deficiency-wind syndromes often recover upon drinking it. The formula is mild, beneficial for the elderly when taken frequently, free from adverse effects, and truly an excellent prescription. Ingredients: Asparagus Fern Root (Tiandong), Ophiopogon Japonicus (Maidong), Prepared Rehmannia Root (Shudi), Szechuan Lovage Root (Chuanxiong), Eucommia Bark (Wujiapi), Achyranthes Root (Niuxi), and Gentian Root (Qinjiao) each 5 qian; Cinnamon Twig (Guizhi) 3 qian. Place in a silk bag, soak in 20 jin of Fenjiu (Fen wine), 1 jin of pure white honey, 1 jin of red rock sugar, and 1 jin of aged rice vinegar. Mix well, soak in a porcelain jar, seal with tofu skin, weigh down with a large brick, boil for the duration of three incense sticks, remove, and bury in soil for seven days before consumption.
Zhibao Dan (Treasure-containing Pill) treats sudden qi closure from malignant qi, aphasia from stroke, poisoning from various substances, severe heat with irritability, asthma and vomiting, difficult labor with confusion, and retained dead fetus. For these, mix with 1 he of child urine (Tong Niao) and 4-5 drops of fresh Ginger Juice (Shengjiang Zhi), warm and dissolve, administering 3-5 pills yields miraculous results. It also treats vomiting from accumulated heat in Heart and Lung, pathogenic qi attacking the Heart, constipation due to intestinal wind, spiritual confusion, dizziness, dry mouth and insomnia, and delirium from Cold Damage. It treats pediatric epilepsy, acute fright-heat, sudden invasion by alien qi, insomnia, irritability, and convulsions due to wind-phlegm. Two-year-old children take 2 pills with Ginseng Decoction (Renshen Tang). (Xu Lingtai said: This is an essential formula for calming the Spirit and stabilizing the Soul, truly a divine pill.) Rhinoceros Horn (Xijiao) from Siam, sliced; Cinnabar (Zhusha), ground and floated in water (preferably the 'Guanyin Face' variety); Realgar (Xionghuang), ground and floated in water; Amber (Hupo), ground and floated in water; Tortoise Scutellum (Daimao), sliced, each 1 liang; Calculus Bovis (Niuhuang) 5 qian; Moschus (Shexiang), ground; Borneolum Syntheticum (Longnao), ground, each 1 qian; Gold Leaf (Jinfu) and Silver Leaf (Yinfu), 50 sheets each; Liquid Storax (Anxi Xiang) 1 liang, boiled with non-grain alcohol into a paste (substitute with Dry Storax/Hanxi Xiang if unavailable). First, grind Rhinoceros Horn and Tortoise Scutellum into powder, mix with other drugs. Heat the Storax paste in a double boiler until liquefied, then combine with the powders to form pills the size of paulownia seeds. Dissolve 3-5 pills in Ginseng Decoction. *Ben Shi Fang* adds Ginseng Root (Renshen), Southern Arisaema Rhizome (Nandanshen), and Bamboo Residue (Tianzhuhuang). Wang Jinshan said: This is a formula for treating Heart Spirit confusion, penetrating from the exterior to the interior. Rhinoceros Horn, Tortoise Scutellum, Calculus Bovis, and Amber use spiritual substances to open the Heart orifices internally; Cinnabar, Realgar, and Gold/Silver leaves use heavy sedatives to calm the Heart Spirit; assisted by Borneol, Moschus, and Storax to search out hidden obstructions in the orifices. Thus, for heat entering the Pericardium Collateral with crimson tongue and confusion, adding this pill to cooling decoctions drives out Yin and raises Yang, instantly restoring mental clarity, surpassing other medicines. However, if headache precedes confusion and aphasia, it indicates Liver deficiency allowing the Soul to rise to the vertex; Long Bone (Longgu) and Oyster Shell (Muli) should be used to rescue and descend, which Zhibao Dan cannot achieve.
Ye Tianshi treated Patient Lu: Yang pathogens invading collaterals causing hemiplegia and aphasia indicate heat in the Blood. Moxibustion exacerbates collateral heat, worsening the condition and loss of appetite. Cooling is the correct treatment, but gentle agents do not damage Blood and possess wind-extinguishing functions. Herbs: Rhinoceros Horn (Xijiao), Antelope Horn (Lingyang Jiao), Rehmannia Root (Shengdi), Scrophularia Root (Yuanshen), Forsythia Fruit (Lianqiao), Tangerine Peel (Juhong), Scutellaria Baicalensis-derived Phlegm-resolving Herb (Danning), and Acorus Tatarinowii (Shichangpu). (Xu Lingtai said: The formula and theory are excellent.)
Hua Xiuyun said: Limb spasms, hemiplegia, facial deviation, stiff tongue, and speech impediment indicate prior deficiency of the root body, with Wind-Yang carrying Phlegm-Fire to obstruct, causing disharmony of Ying and Wei collaterals. Treatment: In acute stages, first open the orifices; subsequently, tonify Qi and enrich Blood. Once collaterals are filled and unblocked, the disease can be cured. (Xu Lingtai said: These sentences are the general principles.)
