Xu Zhongguang treated a case of smallpox where the patient presented with fever and occasional epistaxis (nosebleed). The appropriate treatment was to induce sweating to release the exterior. He used Shengma Gegen Tang (Cimicifuga and Kudzu Root Decoction) added with Jingjie (Schizonepetae Herba) and Niubangzi (Arctii Fructus), resulting in recovery.
In another case, a smallpox patient vomited blood on the fourth day of eruption. The pustules were depressed and pale, accompanied by restlessness, delirious speech, cold limbs, and a cool body. This indicated that the heat-toxin in the blood was not yet fully resolved, but had been further damaged by the administration of cooling and purgative medicines. Now, with blood depletion and toxin resolution, the patient was deficient. The priority was to tonify Qi. He used Baoyuan Tang (Preserve Vitality Decoction) added with Ganjiang (Prepared Zingiberis Rhizoma), Fuzi (Prepared Aconiti Lateralis Radix), Chuanxiong (Chuanxiong Rhizoma), Danggui (Angelicae Sinensis Radix), Chantui (Cicadae Periostracum), Baishao (Paeoniae Radix Alba), and Nuomi (Oryzae Sativae Germen), leading to recovery.
In a third case, a patient experienced hematochezia (blood in stool) while the smallpox was erupting. Symptoms included lethargy, reduced appetite, thirst, and cold limbs, all indicative of Spleen deficiency. He used Sijunzi Tang (Four Gentlemen Decoction) added with Shengma (Cimicifugae Rhizoma), Chenpi (Citri Reticulatae Pericarpium), and Paojiang (Prepared Zingiberis Rhizoma), resulting in recovery.
In a fourth case, on the eighth day of smallpox, the base of the lesions was red, swollen, full, and painful. The patient was vexed, thirsty, desired cold drinks, and passed black blood in the stool. This was due to internal accumulation of heat-toxin. He used Xijiao Dihuang Tang (Rhinoceros Horn and Rehmannia Decoction) to cure it.
In a fifth case, after the crusting phase of smallpox, the patient had fever, cough with bloody sputum, hoarseness, and epistaxis. This was attributed to Fire insulting the Metal of the Lung. He used Huanglian Jiedu Tang (Coptis Decoction for Resolving Toxins) added with Maidong (Ophiopogonis Radix), Xijiao (Rhinocerotis Cornu, now commonly substituted with Buffalo Horn), Danpi (Moutan Cortex), Zhimu (Anemarrhenae Rhizoma), and Niubangzi (Arctii Fructus), leading to recovery.
In a sixth case, after the crusting phase, the patient suffered from persistent cough. This was due to toxin stagnation in the Lung. Treatment focused on clearing the Lung, resolving stagnation, and detoxifying, resulting in recovery.
In a seventh case, after the crusting phase, the patient passed purple blood in the stool. This indicated toxin stagnation in the lower jiao. He prescribed Yujin (Curcumae Radix) 5 qian and Niubangzi (Arctii Fructus) 1 qian, taken as 0.5 qian per dose, administered with Dengxincao Tang (Juncus Decoction).
In an eighth case, following smallpox, the patient experienced loose stools and hematochezia after defecation. This was due to weak Spleen Qi. He used Yigong San (Extraordinary Power Powder) added with Shengma (Cimicifugae Rhizoma), Diyü (Sanguisorbae Radix), Huanglian (Coptidis Rhizoma), combined with Xiaochaihu Tang (Minor Bupleurum Decoction), resulting in recovery.
In a ninth case, post-smallpox hematuria with dysuria (blood strangury) occurred. This was due to heat binding in the lower jiao. He used Bai (Phellodendri Cortex), Zhimu (Anemarrhenae Rhizoma), Tong (Akebiae Caulis), Di (Rehmanniae Radix), Xiaoji Gen (Cirsii Japonici Radix), Oujie (Nelumbinis Rhizomatis Nodus), Danggui (Angelicae Sinensis Radix), Hua (Talcum), Gancao Shao (Tip of Glycyrrhizae Radix et Rhizoma), and Zhuye (Bambulae Folium). The patient recovered after taking the medicine.
A six-year-old girl presented with vomiting of large amounts of dark purple-black blood during the eruption of smallpox points. However, her lips and tongue were moist, her complexion and demeanor were favorable, her spirit was calm, and her qi was vigorous. She ultimately remained unharmed. Since her father had a history of blood disorders, this was considered a congenital predisposition rather than toxic pathogenic factor. Therefore, no detoxification or Spleen-tonifying measures were taken, and the condition resolved naturally. Although the symptoms appeared dangerous, the clinical presentation and spirit were harmonious; one cannot deem the vomiting of dark purple-black blood during smallpox eruption as a fatal sign.
Wan Mizhai stated: Lin Xiao from my village, in his early twenties, contracted smallpox. Initially presenting with fever, he also had blood in his urine. Upon hearing this, I said, "It is incurable." When asked why, I replied: "In the spring of the Yiwei year, eighteen people from the Xu family in Taocun'ao, Qishui, died of smallpox, all presenting with blood in the urine. Lin Xiao died three months later."
