Xu Zhongguang stated: A pregnant woman, during the stage of pock filling with pus (Yang Jiang), suffered a miscarriage. Due to excessive blood loss and mental confusion, she died from toxic pathogens sinking inward (Fu Xian).
Another pregnant woman presented with identical symptoms. She was treated with Huangqi (Astragali Radix) 2 liang 5 qian, Renshen (Ginseng Radix) and Danggui (Angelicae Sinensis Radix) each 1 liang, Ejiao (Donkey-hide Gelatin) 5 qian, Gancao (Licorice Radix), Aiye (Artemisiae Argyi Folium), and Paojiang (Prepared Zingiberis Rhizoma) each 3 qian, and Fuzi (Aconiti Lateralis Radix Praeparata) 1 qian. She recovered.
Another pregnant woman delivered normally during the pock-filling stage; the eruption followed a favorable course (Shun Zheng). She was treated with Baoyuan Tang (Preserve Origin Decoction) modified with Chuanxiong (Ligustici Chuanxiong Rhizoma), Danggui, Jingjie (Schizonepetae Herba), Shanzha (Crataegi Fructus), and Yimucao (Leonuri Herba), and recovered.
Another pregnant woman had sufficient pock pus but difficulty forming scabs, with facial redness and afternoon fever. This indicated Spleen deficiency and Blood insufficiency. She was treated with Antai Yin (Fetus-Securing Drink) added with Renshen, and recovered.
Another pregnant woman had evenly distributed, bright but grayish-white pocks, high fever leading to miscarriage, mental confusion, and cold vomiting. This indicated Stomach Qi deficiency and Cold. She was treated with Baoyuan Tang added with Paojiang, Baizhu (Atractylodis Macrocephalae Rhizoma), and Rougui (Cinnamomi Cortex), and recovered.
Another postpartum woman had difficulty in pock emergence, fatigue, and continuous bleeding. This indicated dual deficiency of Qi and Blood. She was treated with Chuanxiong, Danggui, Renshen, Jiang (Zingiberis Rhizoma), Yimucao, and Shengma (Cimicifugae Rhizoma); bleeding stopped and pocks emerged. Subsequently, Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction) was used for recovery.
Another pregnant woman with smallpox recovered after regulation with Antai Yin. In cases of severe fetal pain, Shaoren (Amomi Fructus) stir-fried until black, ground into powder, and taken with wine (1 qian) resulted in immediate recovery.
Another postpartum woman with smallpox had insufficient pus, grayish-white color, body heat, cold limbs, chills with teeth grinding, irritability, and loose stools. This indicated Spleen and Stomach deficiency. She was treated with Yigong San (Benefit the Work Powder) and recovered. There were also cases of unceasing bleeding where medication failed, resulting in death due to inverted pocks (Dao Ye).
A woman postpartum had a favorable eruption but abundant phlegm; clearing heat and resolving exterior symptoms worsened the condition. This indicated Yin deficiency failing to control Yang. She was treated with Liuwei Dihuang Wan (Six-Ingredient Pill with Rehmannia) formula added with Danggui and Maidong (Ophiopogonis Radix), and recovered.
A woman whose smallpox had just healed engaged in sexual intercourse forcibly; the scars changed color, leading to consumption (Lao) and death.
Another postpartum woman had difficulty in pock emergence, irritability, delirium, and madness. This indicated heavy toxicity obstructing and stagnating. She was treated with Chuanxiong, Danggui, Lian (Coptidis Rhizoma), Zi (Saussureae Radix), Shengma, Bang (Arctii Fructus), Gan (Licorice), Shao (Paeoniae Radix Alba), and Chantui (Cicadae Periostracum), and recovered.
Wan Mizhai treated Ms. Cheng, aged twenty, who had smallpox and was five months pregnant. Pulse diagnosis indicated a male fetus. The primary treatment focused on clearing heat and detoxifying, harmonizing the middle, and securing the fetus. Huangqin (Scutellariae Radix) and Baizhu served as the sovereign drugs, while Renshen, Sheng Gancao (Raw Licorice Radix), Danggui, Shengdi (Rehmanniae Radix), Baishao (Paeoniae Radix Alba), and Zisu (Perillae Folium) served as assistant drugs. From initial emergence to pus formation, she suffered no other distress. Upon hearing of a home burglary, she hurried back home. A physician administered one dose of medicine, causing miscarriage; it was indeed a male fetus. Re-evaluating, the pocks turned grayish-white and flat, forming inverted collapse (Dao Xian), due to interior deficiency. Inquiry revealed the prescribed formula was Dusheng San (Single Holy Powder). Wan said: Shan Jia (Manis Squama) and Shexiang (Moschus) are abortifacient drugs. With weak fetal qi and further depleted Qi and Blood, the toxin sank inward; this was incurable. She died three days later.
Zhu Yingwo treated a pregnant woman from an official Zhao family, aged twenty-five, five months pregnant. Smallpox appeared suddenly. Zhu observed that the spots were not yet vigorous. By the fourth or fifth day, pocks on the back were dense like snake skin, leaving no empty space; similarly from below the breasts to the lower abdomen. Her father-in-law and husband deemed her untreatable. Zhu examined carefully and found that the forehead, face, neck to chest, all had distinct, round, open pocks with clear red and white colors. Checking arms and legs, he noted they resembled the face and body, and promised treatability. By the seventh or eighth day, she became extremely distressed, unconscious, ashamed to be covered, naked, drinking a bucket of water daily, including copious amounts of cooling herbs like Huanglian Jiedu Tang (Coptis Decoction for Resolving Toxicity). It was May weather; she was delirious and restless. Zhu ordered soil to fill the floorboards, place mats on the soil for her to lie on, and keep buckets of water on both sides to cool the air, moving her to a bed at night. Such care led to recovery on the eighteenth day. On the ninth day of treatment, noticing stagnant pus, knowing excessive cooling agents were used, Zhu added single-ingredient Mahuang (Ephedrae Herba) 1 liang. Pus surged immediately. This method is miraculous and effective. On the eighteenth day, departing, he instructed the husband: To secure the fetus, administer several doses of Qi and Blood tonics. They did not believe him. Over forty days later, the fetus aborted, but the woman remained well. For such cases, posterior supplementation is indispensable.
Another pregnant woman surnamed Wang, three months pregnant, developed smallpox. Zhu arrived on the fourth day, unaware of previous treatments. The host asked: Why does the woman pass peach-red bloody fluid? Zhu, noting the host's faith in other doctors, remained silent. The next day, large blood clots appeared. The host panicked and sought Zhu urgently. Zhu saw fine, bright pocks, indicating recoverability, and prescribed Antai San (Fetus-Securing Powder): Ejiao Zhu (Processed Donkey-hide Gelatin) 3 qian, Huangqin 3 qian, Shaoren, Baizhu, Danggui Shen (Danggui Root Tip), Shaoyao, etc. Due to abdominal distension, Muxiang (Aucklandiae Radix) 3 fen was added to move stagnant Qi. One dose stopped bleeding and secured the fetus. The host marveled at his divine skill. Zhu said: Three to four doses are needed for the fetus to be safe. The host neglected this. Three days later, bleeding recurred, abdomen distended, and the fetus aborted. The mother's smallpox remained safe. This shows the significance of dosage.
A young woman, three months pregnant, developed smallpox. The doctor saw sparse facial spots and verbally assured mild illness. Zhu observed that although superficially sparse, the Qi and Blood were turbid, tops lacked water droplets, and the tongue was purple-red. He said: The pocks will increase; safety cannot be guaranteed. The host and doctor did not understand. Zhu said: The tongue is the sprout of the Heart; fetal vessels connect to the Hand-Shaoyin Heart Meridian. Purple-black tongue indicates toxicity in the Blood level; how can abortion be avoided? Indeed, after three to four days, pocks spread fully. On the sixth day, bleeding occurred and the fetus aborted. Urgently, Huangqin, Baishao, Baizhu, and Ejiao were used to nourish Blood. Pocks grew slightly with some pus color, but Qi and Blood were still deficient. Despite blood-nourishing agents, blisters appeared on limbs, chest, and back. Urgently, Spleen-tonifying and damp-draining herbs, including Shanyao (Dioscoreae Rhizoma), Liupi (Punicae Pericarpium), and Yiyi (Coicis Semen), were administered for three to five doses. By the fourteenth or fifteenth day, pus formed and recovery ensued. This shows that for pregnant women with smallpox, even slight difficulties make securing the fetus hard; caution is essential.
