( Xu Zhongguang treated a large number of smallpox cases. Ren Duan recorded and transcribed the following, all of which were his miraculous cures. They are recorded here.)
Case 1: A case of smallpox that failed to rise or swell after six days, presenting with chills and teeth grinding (teeth clenching), abdominal distension, and shortness of breath. It was cured using Qigong San (Miraculous Attacking Powder) and Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction).
Case 2: A case of smallpox where, during the initial fever stage, there was aversion to cold and body tremors. This indicates a struggle between pathogenic factors and healthy qi, with the toxin attempting to emerge but failing to do so completely. It was cured by regulating the condition with Shengma Gegen Tang (Kudzu and Cimicifuga Decoction) added with Ginseng, Saposhnikovia root, Cinnamon Twig, and Pangolin Scales, along with Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction).
Case 3: In a case where the smallpox rash had not fully erupted, the hands and feet began to shake. After administering Yigong San (Extraordinary Function Powder), the symptoms worsened. This is a case of false cold with true heat, where Liver Wood qi is excessive and accompanied by Fire transformation. The appropriate treatment is to calm the Liver and control Fire. It was cured using Shengma Gegen Tang (Kudzu and Cimicifuga Decoction) added with Ligusticum rhizome, Bupleurum root, and Saposhnikovia root.
Case 4: During the fever stage of smallpox, teeth grinding accompanied by wandering eye movements (strabismus) indicates severe Heart Heat. It was cured using Shengma Gegen Tang (Kudzu and Cimicifuga Decoction) combined with Daochi San (Guide Out Redness Powder).
Case 5: During the fever stage of smallpox, chest oppression, mental confusion, and teeth grinding indicate a Liver Heat syndrome. It was cured using Qianghuo Tang (Notopterygium Decoction) added with Kudzu root and White Peony root.
Wan Mizhai’s eldest grandson, aged two, contracted smallpox with fever. Within three days, he experienced chills resembling malaria. His father wept and said, "He will die." Wan smiled and said, "You are a physician; saving lives is like being a boatman. When suddenly encountering wind and waves, if your hands and feet panic, how can you save others? This child has robust Yuan Qi (Primordial Qi) and minimal toxin. Since the pathogen cannot overcome the healthy qi, the chills will subside. Observe, and his rash will likely be sparse." Indeed, only five or seven spots appeared, and he recovered in seven days.
Case 6: A child presented with teeth grinding producing a grating sound immediately upon the onset of fever, along with mental confusion. This is an adverse smallpox case, belonging to Kidney Deficiency. Since the Kidneys govern bones, and teeth are the surplus of bones, insufficient Kidney Water fails to restrain Toxic Fire. The stirring Fire causes the upper and lower teeth to clash audibly. This is what Chen referred to as "withered" (Gao). The child unfortunately died.
Case 7: A child had a very dense smallpox rash. Wan stated, "This child's Spleen and Stomach have been weak since childhood. We should use formulas to tonify the Stomach, making the Blood and Qi vigorous so the rash can mature easily." The family did not listen. After suppuration, the rash failed to crust over on time, leading to widespread ulceration. Symptoms included chills, teeth grinding, and loss of voice. Wan explained, "This trembling is due to widespread ulceration, making sitting and lying difficult and unbearable, rather than mandibular chattering from cold. Teeth grinding here is caused by itchy gum ulcers causing the teeth to rub together, not by convulsive teeth-clenching from mental confusion. Loss of voice resulted from crying day and night due to denied cravings for meat, not from throat ulcers." He used Tianyuan Tang (Restoring Primordial Decoction) added with Angelica dahurica and Saposhnikovia root, secretly adding one slice of Prepared Aconite Root. After three doses, the patient stabilized.
