Xue Lizhai treated a man who, in early winter, suffered a burn on his arm causing pain, accompanied by wheezing, coughing, and fever. This was a syndrome of fire-toxin attacking the Lung Metal (Lung organ system). He used Renshen Pingfei San (Ginseng Lung-Pacifying Powder) to treat it, after which the wheezing and coughing ceased. Due to overexertion, he subsequently experienced aversion to cold and fever, indicating Qi and Blood deficiency. He was treated with Bazhen Tang (Eight-Treasure Decoction) augmented with Zhike (Aurantium Fructus), Jiegeng (Platycodon Radix), and Baizhi (Angelicae Dahuricae Radix), which resolved the symptoms. An additional 3 fen of Rougui (Cinnamomi Cortex) was added to assist the medicinal action, warming the Qi and Blood, allowing necrotic tissue to slough off and healing to occur. If the injury initially presented with redness and pain, Shenxiao Danggui Gao (Miraculous Efficacy Angelica Ointment) should be applied externally; mild cases would heal spontaneously, while severe cases would undergo necrosis followed by granulation, showing miraculous efficacy. Alternatively, Cebaiye End (Biota Tops Powder) mixed with beeswax oil for external application is also effective. If fever and thirst are present with dark urine, and the pulse is surging, rapid, and full, Siwu Tang (Four-Substance Decoction) combined with Fuling (Poria), Mutong (Akebia Caulis), raw Gancao (Licorice Root), and stir-fried Huanglian (Coptidis Rhizoma) should be used. If the pulse is surging and rapid but deficient, Bazhen Tang (Eight-Treasure Decoction) is indicated. If the lesion does not ulcerate and appears dark, Sijunzi Tang (Four-Gentlemen Decoction) combined with Chuanxiong (Ligusticum Rhizome), Danggui (Angelicae Sinensis Radix), and Huangqi (Astragali Radix) should be used. If necrotic tissue has ulcerated but new flesh fails to grow, Sijunzi Tang (Four-Gentlemen Decoction) combined with Huangqi (Astragali Radix), Danggui (Angelicae Sinensis Radix), and Paojiang (Prepared Ginger) is appropriate. If aversion to cold persists after healing, it indicates Yang Qi has not yet recovered; Shiquan Dabu Tang (Complete Great Tonifying Decoction) should be urgently administered. Cold-natured medicines must be strictly avoided to prevent further damage to the Spleen and Stomach.
Another man, intoxicated, suffered a scald on his leg from hot water, resulting in ulceration, fever, and thirst with desire to drink. His pulse was surging, rapid, and forceful, indicating pathology caused by fire-toxin. He was prescribed Dihuang (Rehmanniae Radix), Danggui (Angelicae Sinensis Radix), Huangqin (Scutellariae Radix), Huanglian (Coptidis Rhizoma), Mutong (Akebia Caulis), Gegen (Puerariae Radix), and Gancao (Licorice Root). After more than ten doses, all symptoms gradually subsided. Subsequently, Ren (Ginseng), Huangqi (Astragali Radix), Baizhu (Atractylodis Macrocephalae Rhizoma), Gancao (Licorice Root), Chuanxiong (Ligusticum Rhizome), Danggui (Angelicae Sinensis Radix), Shaoyao (Paeoniae Radix), Baizhi (Angelicae Dahuricae Radix), and Mugua (Chaenomelis Fructus) were used as new tissue nearly completed. Due to exertion, sudden chills and fever occurred, attributed to Qi and Blood deficiency. Medicines containing Ren (Ginseng) and Huangqi (Astragali Radix) were continued, supplemented with Wuweizi (Schisandrae Fructus) and Zaoren (Ziziphi Spinosae Semen), stabilizing the condition. The wound closed after another month.
A man burned by fire suffered pain in both arms and difficulty urinating and defecating, indicating fire-toxin transmitted to the Lower Jiao (Lower Burner). Dihuang (Rehmanniae Radix), Danggui (Angelicae Sinensis Radix), Baishao (Paeoniae Radix Alba), Huanglian (Coptidis Rhizoma), Mutong (Akebia Caulis), Shanzhi (Gardeniae Fructus), Chifuling (Polyporus with Hostwood), and Gancao (Licorice Root) were administered. After one dose, urination and defecation became clear and unobstructed, and pain ceased. Thereafter, Siwu Tang (Four-Substance Decoction) combined with Ren (Ginseng), Huangqi (Astragali Radix), Baizhi (Angelicae Dahuricae Radix), and Gancao (Licorice Root) removed necrotic tissue, and after several more doses, new flesh grew.
A woman suffered a scald on her chest, resulting in severe ulceration that failed to heal for two months. Her pulse was large but powerless, with dry mouth, fever worsening in the late afternoon. This indicated Yin-Blood deficiency with fire-toxin taking advantage of the weakness. Siwu Tang (Four-Substance Decoction) augmented with Chaihu (Bupleuri Radix) and Danpi (Moutan Cortex) reduced the fever and cooled the body. Xiaoyao San (Free and Easy Wanderer Powder) augmented with Chenpi (Citri Reticulatae Pericarpium) was then used to nourish Yin-Blood and strengthen the Spleen and Stomach, leading to the removal of necrotic tissue and growth of new flesh.
For any scald or burn causing unbearable pain, or resulting in ulceration or malignant sores, pine bark can be peeled off, dried in the shade, ground into fine powder, mixed with a small amount of Qingfen (Calomel), and diluted with raw sesame oil for external application. If the paste does not adhere, secure it with gauze or silk fabric; scabs will form with miraculous results. It is advisable to prepare this beforehand for emergencies. Naturally shed, thin bark is particularly effective.
The formula of Li Mo Anfu uses Niujiaojiao (Asini Corii Colla) melted with a little water over fire until thick. Dog hair is cut into pieces and mixed with the glue, spread on soft cloth, and applied until the scab falls off without pain. A maid in the household of Hanlin Academician Wu scalded her legs and knees when a rice pot overturned at night. Unable to cry out due to darkness, she discovered ulceration by morning. Treatment with this method resulted in recovery. (From *Baibian Fang*)
Xue Lizhai treated the son of the Feng family suffering from a fire sore. The sudden application of cold medicines externally exacerbated abdominal distension and loss of appetite. Ren Shen Bai Du San (Ginseng Detoxification-Decoction Powder) augmented with Mutong (Akebia Caulis) and Shanzhi (Gardeniae Fructus) was used internally. Externally, Cebaiye (Biota Tops) was stir-fried into powder and applied with sesame oil, leading to gradual recovery. He once used the floating fat from boiled dog broth mixed with Yinzhu (Vermilion) for topical application, which proved even more effective. Using cold medicines forces fire-toxin inward, often leading to untreatable conditions.
Wang Hongxu treated a woman whose lower leg was scalded. A physician applied ice flakes crushed in snow water. Within moments, the leg swelled to the size of a bucket, with excruciating pain. Wang remarked: Fortunately, it is on the lower leg; if it were on the waist or abdomen, the toxin would be trapped inward, making rescue difficult. Diyu (Sanguisorbae Radix) was finely ground and mixed with oil, applied to the area. Pain ceased within half an hour, and repeated applications led to complete recovery.
A maidservant had her foot scalded by charcoal fire, resulting in a hole of rotting flesh on her back. Fulonggan San (Dragon Sitting Earth Powder) mixed with milk was applied externally, curing her in less than three days.
A child’s abdomen was scalded by boiling water. Due to pain, the skin was scratched open. A single application of sesame oil stopped the pain. Dry powder of Diyu (Sanguisorbae Radix) was sprinkled on the broken skin; muscle regeneration began the next day, and unbroken areas healed completely.
Qian Guobin stated: I was staying at an inn en route to Suichang when I heard someone crying out in pain from the next room, unable to sleep. The next day, I asked the innkeeper, who replied: A servant carried a bucket of boiling water upstairs for guests to wash. The servant's son followed with a basin. The bottom of the old bucket gave way, pouring boiling water over the child's head. Now swollen like a bucket with flattened features, the child has not eaten for seven days. I examined him immediately; the room smelled foul. One jin (approx. 500g) of Xiakucao (Prunella Spica) was ground into powder, mixed with sesame oil, and applied thickly to the swelling. Pain and odor ceased instantly. Swelling subsided in three days, and scabs fell off in eight. Consumption of soy sauce products is strictly forbidden. Facial black spots were treated successfully according to this prescription. Later, upon my return, the child's family came to express gratitude.
