Chen Sansan treated a pregnant woman who frequently suffered from diarrhea. Chronic diarrhea is often attributed to the Kidney channel, so he used Baizhu (Atractylodis Macrocephalae Rhizoma) four liang, cooked Shanyao (Dioscoreae Rhizoma) two liang, fried Gancao (Glycyrrhizae Radix et Rhizoma) one liang, Jiangzhi chao Zhidu (Eucommiae Cortex processed with ginger juice) and Songhua (Pinus pollen) fried each seven qian. These were made into pills using rice paste, and the patient was cured after taking them. (Xu Dacun notes: This is essentially treating the Spleen.)
Xue Lizhai treated the concubine of Bian Taichang, who suffered from diarrhea during pregnancy. She had self-medicated with drugs such as Zhishi (Aurantii Fructus), Baizhu (Atractylodis Macrocephalae Rhizoma), and Huanglian (Coptidis Rhizoma), resulting in abdominal distension and fullness, phlegm expectoration, fever, aversion to cold, and nausea upon eating. Even when forced to swallow a spoonful, she experienced acid regurgitation and discomfort. There was a proposal to use phlegm-resolving and qi-regulating methods. However, this condition arose from Spleen and Stomach injury leading to phlegm stagnation in the Middle Jiao; treating the phlegm and qi at this stage would further damage the Spleen and Stomach. Therefore, Xue Lizhai used Renshen (Ginseng Radix et Rhizoma), Baizhu (Atractylodis Macrocephalae Rhizoma), and Paojiang (Zingiberis Rhizoma Preparatum) ground into powder, formed into pills the size of millet seeds. The patient held three to five pills in her mouth to swallow slowly at any time, gradually increasing to three hundred pills daily. Subsequently, Liu Junzi Tang (Six Gentlemen Decoction) was administered daily, and she gradually recovered.
The wife of Jinshi Wang Zhengzhengng suffered from diarrhea, food aversion, and vomiting during pregnancy, indicating Spleen Qi injury. Her mother-in-law, worried about this, forced her to drink rice water. Xue Lizhai said, "Rice water can also damage the Spleen and Stomach; there is no need to force it." Instead, Yangwei Tang (Stomach-Nourishing Decoction) was given. The patient vomited sour and bitter fluids and desired to take cooling and fire-draining herbs. Xue said, "If so, the Stomach Qi will be further damaged." The Classics state: "For diseases damaging the Spleen and Stomach, regulate the diet and adapt to temperature changes." Eventually, without medication, she recovered naturally.
A woman, due to anger, suffered from chest and diaphragm oppression and poor appetite. After taking herbs for digestion promotion and qi regulation, her Spleen and Stomach became weaker, appetite decreased, stools were loose and uncontrolled, and over time, yellow watery stools or those with white mucus appeared. Observing her sallow complexion with hidden whiteness, Xue said, "Yellow indicates Spleen deficiency, and white indicates Lung deficiency." In the morning, Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction) was used to raise and tonify Stomach Qi; in the evening, Liu Junzi Tang (Six Gentlemen Decoction) was used to cultivate and tonify Spleen Qi, leading to recovery.
Yi Silan treated the Consort of Prince Qian of Shicheng, who conceived in the sixth month of the Guiyou year and accidentally suffered from diarrhea. The palace physicians used diuretic and leaching agents to stop the diarrhea, after which she suffered from diarrhea for three to five days every month. Some doctors diagnosed it as Spleen diarrhea and used Shenling Baizhu San (Ginseng, Poria, and Atractylodes Macrocephala Powder), which stopped the symptoms after two or three doses, but she still suffered from diarrhea five to seven times monthly. By the third month of the following year, after giving birth, she suffered from continuous diarrhea for half a month, eight to nine times day and night, with all medicines failing. Yi Silan's pulse diagnosis revealed that the Cun (Inch) and Chi (Cubit) pulses on both hands were peaceful, but the Guan (Bar) pulses were surging, large, and forceful. Yi said, "This is a Summerheat disease." He treated her with Huanglian Xiangru Yin (Coptis and Elsholtzia Decoction). After one dose, symptoms halved; after two doses, she was completely cured. Only the Liver pulse remained unsubsided, so Tongyuan Erba Dan (Unblocking Origin Two-Eight Pill) was used for another half month until she recovered. The Prince asked, "The Consort suffered from diarrhea for nearly a year, and no doctor mentioned Summerheat. Why do you say it is Summerheat?" Yi replied, "It is seen in the pulse. The Guan pulses are floating, surging, large, and forceful, thus I know it is Summerheat diarrhea." The Prince said, "The Mai Jing states that Wind causes a floating pulse, and Summerheat causes a deficient pulse. Now the pulse is surging, large, and forceful, not deficient. How can you diagnose Summerheat?" Yi explained, "Summerheat damages Qi; upon initial contraction, it manifests immediately, the pathogen is in the Lung, and the Wei Qi of the skin is affected, hence the pulse is deficient. From last June to now, nearly ten months have passed. The pathogen has moved from the exterior to the interior, accumulating for a long time, and Summerheat heat has deepened daily, hence the pulse is surging, large, and forceful." The Prince said, "The Summerheat disease is confirmed. You judge it is not a postpartum disease; what is your basis?" Yi said, "Postpartum pulse changes appear in the Cun and Chi positions. Since these are now peaceful, how does it relate to childbirth? Moreover, the illness began before delivery, clearly not a postpartum disease." The Prince asked, "Why did other doctors' medications only provide temporary relief and fail to cure the root?" Yi said, "Other doctors used diuretics or tonics, each adhering to their own views, missing the source. The source is Summerheat. If Summerheat-specific herbs are used, how could the root not be eliminated?"
(Xu Dacun notes: This condition is not entirely caused by latent Summerheat.)
Xue Lizhai treated a pregnant woman who suffered from urinary incontinence and internal heat. Her Liver pulse was surging and rapid, but weak upon deep pressure. She sometimes experienced pain in both temples and distension in the hypochondrium. This was Liver Fire with Blood deficiency. He used Jiawei Xiaoyao San (Modified Free and Easy Wanderer Powder) and Liuwei Dihuang Wan (Six-Ingredient Rehmannia Pill), and she recovered quickly. Later, she experienced alternating chills and fever, or fever alone, or anger, causing previous symptoms to recur. He used Bazhen San (Eight-Treasure Powder) and Xiaoyao San (Free and Easy Wanderer Powder) concurrently to clear Liver Fire and nourish Liver Blood, leading to complete recovery.
