Wei Yuanheng, while making friends with a Daoist companion at Dasheng, climbed to the summit of Mount Tai. Arriving at the ruins of Emperor Xuanzong's former residence, he found several lines of broken stele inscriptions, faintly resembling ancient script. After washing them, he obtained a song: "Zhu Ya Zao Jiao (Styloceris Siliqua) and Sheng Jiang (Fresh Ginger), Xi Jiao (Rhinoceros Horn), Sheng Ma (Cimicifugae Rhizoma), and Shu Di Huang (Rehmanniae Radix Praeparata). Mu Lv (likely referring to a specific herb or typo in original context, kept as transliteration), Han Lian Cao (Ecliptae Herba), Huai Jiao Zi (Sophorae Flos), Xi Xin (Asari Radix et Rhizoma), and He Ye (Nelumbinis Folium) should be in equal proportion. Qing Yan (Halitum) roasted in equal parts, ground into fine powder for best use. Brushing teeth, firming gums, and blackening beard—who knew there were such immortal prescriptions in the world?" A note below He Ye states it refers to cutting out the heart of the lotus leaf. Thus, he copied it and returned. Prominent officials of the court all followed this prescription for practice, achieving rapid and effective results. (From *Huangchao Lei Yuan*.)
Zhu Duanzhang said: "When I was commissioned to serve in the Huaixi幕府 (military government office), severe toothache erupted. A doctor extracted the painful tooth with forceps, then crushed a handful of herbal medicine, soaked it briefly in hot water, and dabbed the painful area with the liquid using his hand, which immediately stopped the pain. I later sought this prescription and used it to treat many people with good effect. The herb is Zou Mian Di Song Cao (Scutellaria barbata). Common people mistakenly call it Di Cong (Allium chinense). Shen Kuo specifically discussed Di Song in *Bi Tan*, noting its seed is called He Shi (Herba Pedicularis), which is indeed this plant. Qian Jicheng's prescription uses one piece of He Shi, broken and placed in the tooth gap. Gao Jian's prescription decocts He Shi with rice vinegar for gargling. Alternatively, decoct Fang Feng (Saposhnikoviae Radix) and He Shi in water for holding and gargling, or grind the herb and pack it into the painful area; all are effective. (From *Bencao Gangmu*.)
Zhou Mi recorded in *Zhaya Tang Za Chao* regarding treatment for toothache and swelling: Use Hei Dou (Glycine Max) boiled in wine juice, gargle, and it heals immediately. Wang Xiuzhu said: It was verified as effective in his pavilion.
The immortal Zheng Siyuan often rode tigers. His old friend Xu Yin sought treatment for toothache. Zheng said: Insert tiger whiskers while still warm into the tooth gap, and it will heal immediately. He pulled out a few strands and gave them to him. (From *Yi Shuo*.)
Li Jizhi from Zezhou suddenly suffered from toothache, frowning and silent. Luan Jingxian saw him and asked: Why are you unhappy? He replied: Toothache. Luan said: Do you remember what Dai Ren (Zhang Congzheng) said? This is excess heat in the Yangming Meridian. It should be treated with drastic purgation. So he administered Zhou Che Wan (Boat Cart Pill) 70 pills. After finishing the dose, he met several close friends who invited him to drink. He was urged to drink several cups of hot wine. The medicine was activated by the hot wine, causing him to vomit everything. After vomiting, the pain stopped. Li laughed greatly and said: Dai Ren is an immortal! Three or five days later, the pain returned. He took over 100 pills of the previous medicine, had several bowel movements, and then recovered. (From *Rumen Shiqin*.)
Yao Yingfeng observed a person with a red mole on the lip and no other illness, and told him: This is 'Chi Jin' (a sign indicating dental disease), and you will die in three years. The man was angry and did not respond. Three years later, his teeth indeed decayed, and he came seeking help. Xie replied: This is heavenly punishment; you cannot exceed the limit. He indeed died. (From *Qiantang County Annals*.)
Gong Zicai treated a patient with floating and painful teeth, dark complexion, dry mouth and thirst, worsening in the late afternoon. This was due to Spleen deficiency. He used Bu Zhong Yi Qi Tang (Tonify the Middle and Augment the Qi Decoction) and Jia Jian Ba Wei Wan (Modified Eight-Ingredient Pill) to cure him.
A man whose feet felt hot whenever his teeth swelled. This was deficiency fire of the Three Foot-Yin meridians. Using Jia Jian Ba Wei Wan prevented recurrence.
Li Xiaoyuan suffered from full-mouth toothache, ulceration, mobility, inability to eat or drink. This was Ya Gan (Dental Gangrene); various doctors failed. Suddenly, a Daoist transmitted a prescription: Chuan Jiao (Sichuan Pepper) stir-fried 1.5 qian, Tong Qing (Malachite) 1 qian, Peng Sha (Borax) 1 qian. Grind these three into powder. Apply a small amount to the affected area; salivation occurs immediately, and recovery follows.
Yi Silan treated a patient with dental disease. Whenever after sexual intercourse, his teeth felt elongated, unbearable pain ensued, unable to tolerate hot or cold liquids. Anger also triggered pain. Ten years prior, it was mild; ten years later, it was severe, lasting 3-5 days per episode, with extreme groaning, leading to celibacy. Bu Shen Wan (Tonify Kidney Pill) and Qing Wei Yin (Clear Stomach Drink) were ineffective. One day, the disease flared up, and he went seven days without food or drink. Pulse diagnosis: Cun and Guan positions were normal, but Chi positions were洪数有力 (surging, rapid, and forceful), indicating excessive fire evil in the Kidney Meridian. He administered Zi Shen Yin (Nourish Kidney Drink), gargling and swallowing two bowls. He immediately felt heat rising from the Dantian, exiting through the throat. Another two bowls were taken, pain ceased instantly, teeth could be tapped, and it never recurred. Prescription: Huang Bo (Phellodendri Cortex) 3 qian, Qing Yan 1 qian, Sheng Ma 1 qian. Decoct in five bowls of water, frequent gargling and swallowing. The patient asked: I have had dental disease for 20 years, tried over 100 medicines without effect. You used only three ingredients and succeeded. Why? I replied: Teeth belong to the Kidney; all pain belongs to Fire. Your pulse is surging, rapid, and forceful, hardening upon pressure. Deep, soft, and slippery is the Kidney pulse; surging, rapid, and forceful is the Heart pulse. The Kidney pulse should be deep and soft but is surging and rapid; this is the unconquered (Heart/Fire) insulting the conquered (Kidney/Water). Wife overpowering husband (Yin overcoming Yang); fire evil exists in the Kidney Meridian. Sexual activity moves Ministerial Fire, sending evil fire upward, causing teeth to feel elongated and painful. Kidney is Mother of Liver; Liver is Child of Kidney. Liver governs anger. Anger generates Mother Qi; Wood generates Fire, intensifying Fire, causing teeth to elongate and hurt. Those using Qing Wei Yin do so because gums belong to the Yangming Stomach Meridian. This suits those with surging Stomach pulses. Your Stomach pulse is peaceful, meaning the Stomach is healthy. What benefit does clearing Stomach heat provide? Not only no benefit, but cold-natured herbs damage the Stomach, hindering digestion. Kidney governs bones; teeth are surplus bone. Kidney fire causes teeth to elongate. Using Bu Yin Wan (Nourish Yin Pill) containing Gan Jiang (Zingiberis Rhizoma) and other hot herbs adds fire to fire, worsening pain. Thus, Huang Bo is used as Sovereign herb to nourish Kidney Water and drain Kidney Fire. Qing Yan acts as Guide herb. Sheng Ma lifts and disperses Kidney fire evil. Upon ingestion, Dantian heat rises and exits via the throat. Once the Kidney is cleared, teeth settle naturally. Why clear Stomach or tonify Kidney separately?
Yu Dongfu commented: This case explains medical principles most precisely. Accurate pulse recognition combined with clever application ensures success. The mediocrity of Qing Wei San (Clear Stomach Powder) is indeed blameworthy. Even generic yin-nourishing herbs fail here. These three ingredients are clearly justified; missing one is unacceptable, adding one is unnecessary. In my hometown, a patient suffered toothache for years, unresponsive to drugs. Ye Tianshi cured him completely using Shan Yu Rou (Corni Fructus), Wu Wei Zi (Schisandrae Fructus), Nv Zhen Zi (Ligustri Lucidi Fructus), Han Lian Cao each 3 qian, Niu Xi (Achyranthis Bidentatae Radix) and Qing Yan each 1 qian. This utilizes sour and salty descending properties to guide Kidney fire back to the Kidney Meridian, worthy of immortality alongside Master Yi's formula. Xiong noted: Mrs. Xie Zaihua suffered right upper gum pain, worsening over a month, unable to sleep for four nights, whole-body tremors, cold feet, red face, red tongue without moisture. Lesion was not swollen; pulse was soft and spiritless; qi rush felt cold. This was Liver Fire. Administered Zi Shen Wan (Nourish Kidney Pill) 3 qian delivered with salt water; healed after one sleep.
Lv Dongzhuang treated Xu Kaiyong, who suffered pain starting from the upper gum radiating from the ear root, unbearable suffering. Doctors used Ping Wei (Calm Stomach) and fire-clearing herbs, worsening the condition. Pulse: Guan position stagnant, Chi position weak. Prescription: Shu Di Huang (Prepared Rehmanniae Radix) as Sovereign, Du Zhong (Eucommiae Cortex) as Minister, Nv Zhen Zi, Gan Cao (Licorice Root), Huang Bo, Shan Yao (Dioscoreae Rhizoma), Shan Yu Rou as Assistants. His father, Qingchen, asked a doctor about it. The doctor said: Great error, do not take it. Reason: Dental disease is Yangming Fire; what does it have to do with the Kidney? Why use Kidney-tonifying herbs? Qingchen said: If so, I understand the brilliance of this formula. He invited Lv again. Lv said: Disease manifests above, but treatment must start from below. There is a sequence; do not demand quick results. Continue previous formula for several doses, then add Ren Shen (Ginseng Radix), Bai Zhu (Atractylodis Macrocephalae Rhizoma), Fu Shen (Poria with Hostwood), Gan Cao, Bai Shao (Paeoniae Radix Alba), Suan Zao Ren (Ziziphi Spinosae Semen), Yuan Zhi (Polygalae Radix), Dang Gui (Angelicae Sinensis Radix), Huang Qi (Astragali Radix), Dan Pi (Moutan Cortex). Pain reduced but persisted. Pulse: Both Chi positions responsive; Right Guan and above peaceful; Left Guan still stagnant. Said: Now I will eliminate it for you. Used Bu Zhong Yi Qi Tang combined with Long Dan Cao (Gentianae Radix); healed. Minor relapses later resolved with modified previous formula. Advised: Though minor, the root is lower. Maintain careful regulation to prevent frequent recurrence.
Sun Wenyuan treated the wife of Kunchi Neiren for toothache. She fainted three times one night. Next day, both cheeks swelled red, pain unbearable, aversion to wind/cold, unable to sleep or eat. Prescribed Qing Wei Tang (Clear Stomach Decoction) with Shi Gao (Gypsum Fibrosum) as Sovereign, Bai Zhi (Angelicae Dahuricae Radix) as Minister, Lian Qiao (Forsythiae Fructus) as Assistant, Bei Xi Xin (Asari Radix et Rhizoma) as Envoy. Pain vanished after taking it. Externally: Ming Fan (Alumen) powder, one large Wu Bei Zi (Galla Chinensis) stuffed with alum, roasted over charcoal until alum turns red, ground to powder. Apply to teeth occasionally; pain stops immediately. This formula is highly effective.
Sun Huozhai broke his teeth chewing chicken bones, bleeding until fainting. Since then, every time roosters crowed, unbearable pain ensued; otherwise, no pain. Any banquet involving chicken soup triggered pain, even without touching it. For years, his family banned keeping chickens. Relatives inviting him had to warn cooks not to use chicken. Later, someone advised eating yellow weasel. Family caught one, cooked, and served it. Sun, finding the meat strange and disgusting, forced himself to drink a few sips of broth. Afterwards, he ate chicken normally without any pain. (New Case.)
Lord Wen's Formula treats tooth swelling and pain due to Wind-Heat attacking upwards. Du Huo (Angelicae Pubescentis Radix) and Di Huang (Rehmanniae Radix) each 3 qian, ground to powder. Take 3 qian each time, decoct in one bowl of water, take warm with dregs, before bed. (From *Bencao* records.)
Xue Lizhai treated Mao Zongbo for toothache. Stomach pulse was weak. Used Bu Zhong Yi Qi Tang plus Sheng Di Huang (Rehmanniae Radix) and Dan Pi to cure.
Yang Kaogong had loose teeth and thirst, attributed to Spleen/Stomach deficiency and intense Yin Fire. Used Bu Zhong Yi Qi Tang plus stir-fried black Huang Bo for four doses, then Jia Jian Ba Wei Wan; symptoms resolved instantly.
Wang Shiyu had loose teeth and exposed gums, preferring cold drinks/food. This was Stomach Meridian Damp-Heat. First used Cheng Qi Tang (Order the Qi Decoction) to reduce fire, then Qing Wei San to regulate and firm teeth, finally Liu Wei Wan (Six-Ingredient Pill with Rehmannia) to tonify Kidney; cured.
A man with evening fever, internal heat, toothache, gum ulceration, extracting small insects. This was deficiency fire of Three Foot-Yin meridians and Damp-Heat of Foot-Yangming Meridian. First used Tao Ren Cheng Qi Tang (Peach Kernel Order the Qi Decoction) for two doses, then Liu Wei Wan; cured.
Fu Ling (Poria), Shi Gao, Long Gu (Fossilized Bones) each 1 liang, Han Shui Shi (Calcitum) 2.5 liang, Bai Zhi 0.5 liang, one large Shi Yan Zi (Swallow's Egg-shaped Stone) and two small ones. Grind to powder. Rub teeth morning and night. Wang Wenhanqing from Fanshi obtained this from Lin Fuzhe Shoushou, who got it from Li Cheng, military governor of Luoyang in early dynasty. Shoushou lived past 90, teeth not loose, no wind/insect issues. Lord Wang is now 90, can still bite meat. Truly a divine formula. (From *Xu Yijian Zhi*.)
Xue Lizhai treated a woman with toothache due to anger, chills/fever, vomiting. Qing Wei herbs worsened it. This was Liver Fire injuring Stomach, compounded by cold herbs. Used Liu Jun Zi Tang (Six-Gentlemen Decoction) plus Bai Shao, Chai Hu (Bupleuri Radix), Shan Zhi (Gardeniae Fructus); cured.
A woman with fever, toothache, worsening in late afternoon, irregular menstruation. This was Spleen Meridian Blood Deficiency. Used Xiao Yao San (Free and Easy Wanderer Powder) plus Sheng Ma; healed. Relapsed due to anger; same formula plus Chuan Xiong (Ligustici Chuanxiong Rhizoma) cured her.
A woman with stomach burning/spicy sensation, severe heat/pain, later loose teeth. This was Phlegm-Fire in Stomach. Used Er Chen Tang (Two-Aged Decoction) plus Huang Qin (Scutellariae Radix), Huang Lian (Coptidis Rhizoma), followed by Yue Ju Wan (Regulating Pills); healed.
A woman with toothache due to anger, chills/fever. This was Liver Fire insulting Spleen Earth. Used Xiao Chai Hu Tang (Minor Bupleurum Decoction) plus Chuan Xiong, Dang Gui, Fu Ling, Bai Zhu, Shan Zhi; pain stopped. Xiao Yao San with modifications resolved chills/fever.
A woman with painful, mobile gums after childbirth, resolving after a month. This was Qi/Blood deficiency agitation. Later pregnant near term, given Shi Quan Da Bu Tang (Great Tonifying Decoction of Ten Substances) for two doses, instructed to decoct and take postpartum; teeth remained stable.
Tingwei Zhang Zhongliang had loose teeth; used An Shen Wan (Peace the Kidney Pill). Kaogong Yang Zhongyu had loose teeth; used Bu Zhong Yi Qi Tang. Shiyu Wang Jichuan had loose teeth/exposed gums; used Cheng Qi Tang. Wenxuan Zheng Boxing had tooth/head pain; used Qiang Huo Fu Zi Tang (Notopterygii and Aconiti Radix Decoction). Yan Jinxian had toothache; used Liang Ge San (Cool the Chest Powder). Guo Zhifang had toothache from overdrinking; used Qing Wei San. Dang Libu had Wind-Heat; used Xi Jiao Sheng Ma Tang (Rhinoceros Horn and Cimicifugae Decoction). Zhu Gongbu had Qi/Blood deficiency; used Shi Quan Da Bu Tang. Shen Dayin had head/tooth pain, heavy head, cold hands/feet; Cold-Wind invading Brain. Used Ma Huang Fu Zi Xi Xin Tang (Ephedra, Aconite, and Asari Decoction). All cured.
A man with toothache, rapid/solid pulse, constipation. Used Fang Feng Tong Sheng San (Saposhnikovia Unblocking the Exterior Powder); healed.
A man with toothache, rapid/forceful Stomach pulse. Used Qing Wei San plus Shi Gao, Jing Jie (Schizonepetae Herba), Fang Feng; healed in two doses.
A man with severe toothache, rapid/solid Stomach pulse. Used Cheng Qi Tang one dose; pain stopped.
A man with toothache, floating/weak pulse. Used Bu Zhong Yi Qi Tang plus Huang Lian, Sheng Di Huang, Shi Gao; no recurrence.
An elderly man with toothache starting afternoon, worse at night, rapid/solid Stomach pulse. Used Liang Ge San plus Jing Jie, Fang Feng, Shi Gao.
Professor Wang said: An old woman with chronic toothache was taught to cross thumbs, moxa at the tip of the thumb seven moxa cones, preventing future pain. Likely Waiguan (TE5) point, Hand-Shaoyang Meridian, 2 cun posterior to wrist depression. A boatman's wife in Quanzhou had chronic toothache; moxa near lateral malleolus of hand; pain ceased forever.
