(Zi Xian, also known as Feng Jing [Wind Spasm], Yi Ming Zi Mao [Sub-mao], or Lei Zhong Feng [Stroke-like Syndrome]. This section also addresses stroke-like syndromes caused by traumatic wounds, blood loss, hematemesis, and epistaxis.)
Zi Xian (eclampsia/epileptic seizures during pregnancy) is a critical condition. If not treated early, it will inevitably lead to miscarriage. The symptoms include lockjaw (mouth closed tightly), stiffness of the neck, spasms and contractures of the limbs ("Yi" has the same meaning as "Chou"), slurred speech, excessive phlegm and saliva, and unconsciousness. Alternatively, there may be sudden dizziness and collapse, inability to speak, resembling stroke syndrome, but it is actually not stroke; therefore, it should not be treated as stroke. Even if there is no phlegm and speech is normal, but wind-like symptoms are present, it is mostly due to blood dryness or blood deficiency. It should not be generally treated with wind-dispelling methods to avoid error. The main formula is Lingyang Jiao San (Antelope Horn Decoction).
Feng Shi stated: When a pregnant woman suddenly collapses stiffly, with excessive phlegm and saliva and unconsciousness, this is due to blood deficiency causing Yin fire to flare upwards, stirring up phlegm. The left pulse is slightly rapid, and the right pulse is slippery and large. This is named Zi Xian. It is suitable to use Si Wu Tang (Four Substances Decoction) to nourish blood, wine-processed Huang Qin (Scutellaria Root) to clear heat, Er Chen Tang (Two-Cured Decoction) to resolve phlegm and regulate Qi. The treatment method should still focus on calming the fetus, and one should not overuse medicines for treating stroke. This arises from blood deficiency generating heat, and excessive heat generating wind; these are internal winds and fires. Nourishing the blood extinguishes the wind naturally. If there is wind-heat in the Heart and Liver, use Gouteng Tang (Uncaria Decoction); if there is blood deficiency in the Liver and Spleen, use Jiawei Xiaoyao San (Modified Free and Easy Wanderer Powder); if there is liver-spleen depression and anger, use Jiawei Guipi Tang (Modified Restore the Spleen Decoction); if there is rebellious Qi and phlegm stagnation, use Zisu Yin (Perilla Infusion); if there is spleen depression and phlegm stagnation, use Er Chen Tang added with Zhuli (Bamboo Sap) and Jiangzhi (Ginger Juice).
Mi Zhai stated: For pregnant women with Qi deficiency complicated by phlegm-fire, presenting with symptoms resembling stroke, sudden collapse followed by immediate awakening, and recurrence after waking, treat primarily with Qingshen Tang (Spirit-Clearing Decoction), and concurrently administer Shouxing Wan (Longevity Pill). I believe that for pregnant women suffering from Zi Xian, some experience blurred vision, blackouts, and cold extremities. This is extremely rare before delivery. However, whenever this syndrome occurs, it is termed "Er Yun" (Fetal Fainting), belonging to issues of Qi and Phlegm. Therefore, for blurred vision and fainting, only Zisu Yin (Perilla Infusion) should be taken. One must be cautious and absolutely avoid taking Suhé Wan (Styrax Pill) and Wuyao Shunqi San (Tso-yao Regulating Qi Powder) and similar medicines. As for cases where traumatic wounds, blood loss, or hematemesis/epistaxis suddenly cause lockjaw, neck stiffness, and back rigidity resembling stroke, these are all caused by blood loss. This point must not be ignored; treat with Jingfang Antai San (Schizonepeta and Saposhnikovia Fetus-Calming Powder).
