In the postpartum period, if panting and urgency are caused by external contraction of wind-cold, with pathogenic factors entering the Lung causing Qi obstruction, there will inevitably be coarse breathing and chest distension, or frequent coughing. This is distinct from shortness of breath resembling panting, where Qi fails to connect between the upper and lower body. Treatment should primarily focus on dispersing pathogens while simultaneously tonifying deficiency. The appropriate formulas are Jinshui Liujun Decoction (Golden Water Six Gentlemen Decoction) or Liujunzi Tang (Six Gentlemen Decoction). If the condition arises solely from cold pathogen entering the Lung, resulting in excess Qi and Qi stagnation without underlying deficiency, Lian'an Jian (Lian'an Infusion) or Erchen Tang (Two Aged Agents Decoction) added with Su Ye (Perilla Leaf) may be used. Another scenario involves blood stasis entering the Lung, characterized by a dark complexion and panting, which differs significantly from shortness of breath resembling panting; this is treated with Erwei Rensu Yin (Two-Ingredient Ginseng and Perilla Drink). In cases of Qi deficiency leading to the flooding upward of blood-phlegm, Liujunzi Tang taken together with Shixiao San (Pain-Stopping Powder) is also effective.
Jinshui Liujun Decoction: Treats deficiency-cold of the Lung and Kidney, where water overflows to form phlegm. It is also indicated for elderly patients with Yin deficiency and insufficiency of Blood and Qi, who have contracted external wind-cold, presenting with symptoms such as cough, vomiting, nausea, excessive phlegm, and urgent panting.
Ingredients: Danggui (Angelicae Sinensis Radix), Banxia (Pinelliae Rhizoma Preparatum), Fuling (Poria) (each 2 qian); Shudi (Rehmanniae Radix Preparata) (3–5 qian); Chenpi (Citri Reticulatae Pericarpium) (1.5 qian); Zhigancao (Glycyrrhizae Radix et Rhizoma Preparatum cum Melle) (1 qian).
Preparation and Administration: Decoct 2 zhong of water with 3–5 slices of Shengjiang (Zingiberis Rhizoma Recens) until reduced to 0.8 zhong. Take warm after meals. If loose stools and dampness are present, remove Danggui and add Shanyao (Dioscoreae Rhizoma). If phlegm is severe with Qi stagnation and discomfort in the chest and hypochondrium, add Baijiezhi (Sinapis Semen) (7–8 fen). If severe Yin-cold persists with unrelenting cough, add Xixin (Asari Radix et Rhizoma) (5–7 fen). If exterior pathogen with alternating chills and fever is present, add Chaihu (Bupleuri Radix) (1–2 qian).
Lian'an Jian: Treats wind-cold cough, initial stages of non-stroke conditions (Feifeng), and syndromes of phlegm stagnation with rebellious Qi.
Ingredients: Chenpi (Citri Reticulatae Pericarpium) (1.5 qian); Banxia (Pinelliae Rhizoma Preparatum) (2–3 qian); Fuling (Poria) (2 qian); Xingren (Armeniacae Semen Amarum) (skin and tips removed, crushed); Gancao (Glycyrrhizae Radix et Rhizoma) (each 1 qian); Baijiezhi (Sinapis Semen) (5–7 fen). Note: Do not use Baijiezhi in elderly patients with weak Qi or postpartum women.
Preparation and Administration: Decoct 1.5 zhong of water with 3–5 slices of Shengjiang (Zingiberis Rhizoma Recens) until reduced to 0.7 zhong. Take after meals. Contraindicated in cases of sudden collapse due to Qi deficiency or when Qi is calm and no phlegm is present.
