**Shanghan (Cold Damage), Ziman (Excessive Fetal Growth/Abdominal Distension), Ziming (Fetal Crying in Utero), Zi Yin (Pregnancy Aphonia), Zhongshu (Heatstroke), Zhongshi (Dampness Invasion), Xiexie (Diarrhea), Liji (Dysentery), Chuangdu (Sores and Toxins), Wu Gu Kuqi (Unexplained Weeping)**
For pregnant women suffering from Shanghan (Cold Damage), treatment should primarily focus on clearing heat and harmonizing. One must treat according to the exterior or interior syndromes manifested in the Six Channels (Taiyang, Yangming, Shaoyang, Taiyin, Shaoyin, Jueyin). It is absolutely crucial not to treat them as ordinary Shanghan cases, as this could damage the fetus and endanger the lives of both mother and child. In treating Shanghan, regardless of the number of days since onset, if chills and fever are present, it indicates the pathogen is at the exterior. The primary formula is Siwei Zisu Hetai Yin (Four-Ingredient Perilla Fetus-Harmonizing Decoction). Adjust the prescription based on the specific symptoms observed.
Perillae Folium (Zi Su Ye, 1.5 qian), Atractylodis Rhizoma (Bai Zhu, 1.5 qian), Scutellariae Radix (Huang Qin, 1.5 qian), Glycyrrhizae Radix et Rhizoma (Gan Cao, 1 qian).
If there are chills, headache, stiff neck, and pain in the lower back and spine, the disease is in the Foot-Taiyang Bladder Channel. Add Notopterygii Rhizoma et Radix (Qiang Huo, 1 qian), Saposhnikoviae Radix (Fang Feng, 1 qian), Chuanxiong Rhizoma (Chuan Xiong, 1 qian), and Ligustici Rhizoma et Radix (Gao Ben, 1 qian). Use three scallions with roots and hairs as a guide, cover with blankets to induce sweating. If there are chills without fever, only headache, dry nose, or stiff neck, the disease is in the Foot-Yangming Stomach Channel. Add Radix Puerariae (Ge Gen), Angelicae Dahuricae Radix (Bai Zhi), and Saposhnikoviae Radix (Fang Feng) in equal parts, along with three white parts of scallions and a handful of Fermented Soybean (Dan Dou Chi), decoct and take to resolve via sweating. If there are alternating chills and fever, dizziness, or vomiting, or subcostal fullness, or chest and hypochondriac fullness, the disease is in the Foot-Shaoyang Gallbladder Channel. Add Bupleuri Radix (Chai Hu, 1 qian) and Ginseng Radix (Ren Shen, 1 qian). For vomiting, add Pinelliae Rhizoma (Ban Xia, 0.8 fen). For chest and hypochondriac fullness, add Aurantii Fructus (Zhi Ke, 1 qian) and Platycodonis Radix (Jie Geng, 1 qian). For dizziness, add Chuanxiong Rhizoma (Chuan Xiong, 1 qian) with ginger as a guide. If there is fever with chills and severe coughing, the disease is in the Hand-Taiyin Lung Channel (Note: Original text says Spleen, but Hand-Taiyin refers to Lung; translated accordingly for medical accuracy while noting original context). Add Ginseng Radix (Ren Shen), Zingiberis Rhizoma (Gan Jiang, roasted), and Paeoniae Radix Alba (Bai Shao, wine-fried) each 1 qian, with ginger and jujube as a guide. If there are chills with curling up, fever, and cold hands and feet, the disease is in the Foot-Shaoyin Kidney Channel. Add Actaeae Rhizoma (Du Huo, 1 qian), Rehmanniae Radix Praeparata (Shu Di Huang, 1 qian), and Asari Radix et Rhizoma (Xi Xin, 1 qian), with ginger and jujube as a guide. If there are chills with cold extremities, cyanosis of lips and mouth, generalized body pain like being beaten, and vertex headache, the disease is in the Foot-Jueyin Liver Channel. Add Fried Angelicae Sinensis Radix (Dang Gui, 1 qian), Fried Evodiae Fructus (Wu Zhu Yu, 1 qian), Notopterygii Rhizoma et Radix (Qiang Huo, 1 qian), and Asari Radix et Rhizoma (Xi Xin, 1 qian), with three scallions with roots and hairs, and ginger and jujube as a guide.
In treating Shanghan, regardless of the days, if there is no chills or headache, but only fever, dry mouth, dry throat, and thirst, the pathogen is in the interior. Use Huanglong Tang (Yellow Dragon Decoction) as the primary formula. (This formula comes from *Huoren Shu* and also treats headache, hypochondriac pain, and vomiting/regurgitation.)
Bupleuri Radix (Chai Hu, 1 qian), Scutellariae Radix (Huang Qin, 1 qian), Ginseng Radix (Ren Shen, 1 qian), Glycyrrhizae Radix et Rhizoma (Gan Cao, 1 qian).
If there is fever, thirst, and difficult urination, the disease involves the Hand-Taiyang Small Intestine and Foot-Taiyang Bladder channels, indicating pathology in the Small Intestine and Bladder fu-organs. Add Atractylodis Rhizoma (Bai Zhu, 1.5 qian), Polyporus (Zhu Ling, 1 qian), Alisma Rhizoma (Ze Xie, 1 qian), Poria (Chi Fu Ling, 1 qian), and Akebiae Caulis (Mu Tong, 1 qian). If there is high fever and extreme thirst, the disease involves the Hand-Yangming Large Intestine and Foot-Yangming Stomach channels. Add Anemarrhenae Rhizoma (Zhi Mu, 1 qian), Gypsum Fibrosum (Shi Gao, 2 qian), Lophatheri Herba (Dan Zhu Ye, one handful), and Oryzae Semen (Jing Mi, a pinch). If there is intense heat, extreme thirst, irritability, and constipation, the disease is in the Foot-Yangming Stomach fu-organ. Remove Ginseng Radix (Ren Shen), add Aurantii Fructus Immaturus (Zhi Shi, fried, 1 qian), Rheum Radix et Rhizoma (Da Huang, baked, 1 qian), and Natrii Sulfas (Mang Xiao, 1 qian), with ginger as a guide. Stop when watery stool is passed. If there is fever, thirst, irritability, insomnia, or dry retching, the disease is in the Foot-Shaoyang Gallbladder channel. Add Ophiopogonis Radix (Mai Dong), Trichosanthis Radix (Hua Fen), Gardeniae Fructus (Zhi Zi), and Ziziphi Spinosae Semen (Suan Zao Ren) each 1 qian, and Bamboo Shavings (Zhu Rou) one large pill. If there is fever with thirst, abdominal pain, and spontaneous diarrhea, the disease is in the Foot-Taiyin Spleen channel. Add Atractylodis Rhizoma (Bai Zhu, 1 qian), Paeoniae Radix Alba (Bai Shao, 1 qian), E-Jiao (E Jiao, 1 qian), and Poria (Fu Ling, 1 qian), with ginger and jujube as a guide. If there is fever with thirst, unceasing diarrhea, and cold hands and feet, the disease is in the Foot-Shaoyin Kidney channel. Remove Bupleuri Radix (Chai Hu), add Zingiberis Rhizoma (Gan Jiang, fried, 1 qian), Halloysit Rubrum (Chi Shi Zhi, 1 qian), Poria (Fu Ling, 1 qian), and Amomi Fructus (Sha Ren, 1 qian), with ginger and jujube as a guide. If there is fever with thirst, dysenteric discharge of pus and blood, and cold hands and feet, the disease is in the Foot-Jueyin Liver channel. Add Angelicae Sinensis Radix (Dang Gui, 1 qian), Paeoniae Radix Alba (Bai Shao, wine-fried, 1 qian), Atractylodis Rhizoma (Bai Zhu, 1 qian), and Poria (Fu Ling, 1 qian), with Mume Fructus (Wu Mei) as a guide.
After recovery from Shanghan, if fever recurs due to improper convalescence, this is called "Lao Fu" (recurrence due to fatigue). Use Huanglong Tang (Yellow Dragon Decoction) plus Anemarrhenae Rhizoma (Zhi Mu, 1 qian), Ophiopogonis Radix (Mai Dong, 1 qian), and Gypsum Fibrosum (Shi Gao, 2 qian), with Lophatheri Herba (Dan Zhu Ye) and Oryzae Semen (Jing Mi) as guides to extract the juice. If fever recurs due to dietary indiscretion, this is called "Shi Fu" (recurrence due to food). Use the aforementioned Siwei Zisu Hetai Yin (Four-Ingredient Perilla Fetus-Harmonizing Decoction) plus fried Aurantii Fructus Immaturus (Zhi Shi), fried Coptidis Rhizoma (Huang Lian), Citri Reticulatae Pericarpium (Chen Pi), and Massae Medicatae Fermentatae (Shen Qu, baked), with ginger and jujube as a guide. If it is an epidemic contagion, treat according to the above methods by differentiating the Six Channels' exterior and interior; or at the initial stage of illness, use Baidu San (Resist Toxicity Powder) combined with fetus-harmonizing herbs to resolve the pathogen.
Ginseng Radix (Ren Shen), Notopterygii Rhizoma et Radix (Qiang Huo), Bupleuri Radix (Chai Hu), Poria (Fu Ling), Peucedani Radix (Qian Hu), Glycyrrhizae Radix et Rhizoma (Gan Cao), Chuanxiong Rhizoma (Chuan Xiong), Platycodonis Radix (Jie Geng), Aurantii Fructus (Zhi Ke), Scutellariae Radix (Huang Qin), Perillae Folium (Su Ye), Radix Puerariae (Ge Gen), Zingiberis Rhizoma (Sheng Jiang), and White Part of Scallion (Cong Bai) as a guide to extract juice.
If Shanghan heat does not resolve and macules appear all over the body, red like brocade patterns, use Jiawei Huaban Tang (Modified Macule-Transforming Decoction).
Ginseng Radix (Ren Shen, 1 qian), Anemarrhenae Rhizoma (Zhi Mu, 1 qian), Gypsum Fibrosum (Shi Gao, 2 qian), Glycyrrhizae Radix et Rhizoma (Gan Cao, 1 qian), Scutellariae Radix (Huang Qin, 1 qian), Gardeniae Semen (Zhi Zi Ren, 1 qian), Rehmanniae Radix (Sheng Di Huang, 1 qian), Lophatheri Herba (Dan Zhu Ye, 20 leaves), and Fermented Soybean (Dou Chi, half he). Decoct in water and take orally.
**Ziman (Excessive Fetal Growth):** When a pregnant woman reaches seven or eight months, the fetus grows large, causing excessive abdominal distension, pressing on the uterus, leading to restlessness in sitting and lying down (named Ziman). Treat with Shutai Yin (Fetus-Binding Decoction).
Atractylodis Rhizoma (Bai Zhu, 1.5 qian), Scutellariae Radix (Huang Qin, 1.5 qian), Perillae Folium (Su Ye, 1.5 qian), Aurantii Fructus (Zhi Ke, 1.5 qian), Arecae Pericarpium (Da Fu Pi, 1.5 qian), Honey-fried Licorice Root (Zhi Gan Cao, 3 fen), and Cardamom Fructus with shell (Dai Ke Sha Ren, fried, 5 fen). Ginger as a guide.
**Ziming (Fetal Crying in Utero):** When a pregnant woman stretches her body or reaches for objects at a height, the fetus loses the object held in its mouth within the abdomen and cries (called Ziming). The treatment method is to have the mother perform a bowing motion like a man; or scatter beans on the ground and have the mother pick them up, so the fetus holds the bean and stops crying.
**Zi Yin (Pregnancy Aphonia):** At eight or nine months of pregnancy, if the voice suddenly becomes hoarse and speechless (named Zi Yin), it is because the Foot-Shaoyin Kidney channel descends to nourish the fetus and cannot ascend to nourish the tongue. After delivery at ten months, the voice will naturally recover; this is not a disease. Medication can be taken, but do not trust quacks who prescribe purgatives indiscriminately.
**Zhongshu (Heatstroke):** The symptoms include fever with thirst, spontaneous sweating, mental confusion, lethargy of the four limbs, and shortness of breath. Use Qingshu Hetai Yin (Clear Summerheat and Harmonize Fetus Decoction).
Astragali Radix (Huang Qi, honey-fried, 1 qian), Ginseng Radix (Ren Shen, 1 qian), Glycyrrhizae Radix et Rhizoma (Gan Cao, honey-fried, 1 qian), Atractylodis Rhizoma (Bai Zhu, 1 qian), Scutellariae Radix (Huang Qin, 1 qian), Coptidis Rhizoma (Huang Lian, 1 qian), Anemarrhenae Rhizoma (Zhi Mu, 1 qian), Ophiopogonis Radix (Mai Dong, 1 qian), and Schisandrae Fructus (Bei Wu Wei Zi, 10 grains).
**Zhongshi (Dampness Invasion):** Caused by walking in rain, catching fog and dew early in the morning, living long-term in damp places, or bathing in cold water after heavy sweating. Symptoms include fever, joint pain, heavy body, headache, and nasal congestion. Use Huangqin Baizhu Tang (Scutellaria and Atractylodes Decoction).
Scutellariae Radix (Huang Qin, 5 qian), Atractylodis Rhizoma (Bai Zhu, 5 qian), Perillae Folium (Su Ye, 2.5 qian), Zingiberis Rhizoma (Sheng Jiang, 5 slices).
**Xiexie (Diarrhea):** For pregnant women with diarrhea, the principle is to tonify the middle and calm the fetus. Use Sijunzi Tang (Four Gentlemen Decoction) plus Atractylodis Rhizoma (Bai Zhu, 1 qian). If accompanied by fever and diarrhea, it is a heat syndrome; add Scutellariae Radix (Huang Qin, 1 qian). If without thirst, it is a cold syndrome; add Prepared Zingiberis Rhizoma (Pao Jiang, 5 fen) and Mume Fructus (Wu Mei, 1). If chronic diarrhea with extreme thirst, add Radix Puerariae (Ge Gen), Aquilariae Lignum Resinatum (Mu Xiang, fried), Zingiberis Rhizoma (Gan Jiang), Paeoniae Radix Alba (Bai Shao, wine-fried), and Terminalia Chebulae Fructus (He Zi Rou), with Mume Fructus (Wu Mei) as a guide. (Equal parts, adjust according to symptoms.)
**Liji (Dysentery):** For pregnant women with dysentery, the principle is to clear heat, harmonize the fetus, move qi, and nourish blood. Use Huangqin Shaoyao Tang (Scutellaria and Peony Decoction). Do not use Rheum Radix et Rhizoma (Da Huang) or Areca Seed (Bing Lang) to avoid miscarriage.
Angelicae Sinensis Radix (Dang Gui, 1 qian), Atractylodis Rhizoma (Bai Zhu, 1 qian), Scutellariae Radix (Huang Qin, 1 qian), Paeoniae Radix Alba (Bai Shao, 1 qian), Coptidis Rhizoma (Huang Lian, 1 qian), Aurantii Fructus (Zhi Ke, 1 qian), Poria (Fu Ling, 1 qian), Citri Reticulatae Pericarpium (Chen Pi, 1 qian), Raw Glycyrrhizae Radix et Rhizoma (Sheng Gan Cao, 1 qian), Aquilariae Lignum Resinatum (Mu Xiang, 5 fen). Mume Fructus (Wu Mei) as a guide.
**Chuangdu (Sores and Toxins):** For breast abscess (Ru Yong), use Tuoli Jiedu Tang (Support Interior and Resolve Toxin Decoction).
Chuanxiong Rhizoma (Chuan Xiong, 1 qian), Angelicae Sinensis Radix (Dang Gui, 1 qian), Angelicae Dahuricae Radix (Bai Zhi, 1 qian), Scutellariae Radix (Huang Qin, 1 qian), Forsythiae Fructus (Lian Qiao, 1 qian), Trichosanthis Radix (Hua Fen, 1 qian), Lonicerae Japonicae Flos (Yin Hua, 1 qian), Glycyrrhizae Radix et Rhizoma nodus (Gan Cao Jie, 1 qian), Citri Reticulatae Pericarpium Viride (Qing Pi, 5 fen), and Gleditsiae Spina (Zao Ci, 10 pieces).
If located on the back or buttocks, the disease is in the Yangming channel. Remove Citri Reticulatae Pericarpium Viride (Qing Pi), add Cimicifugae Rhizoma (Sheng Ma, 1 qian) and Radix Puerariae (Ge Gen, 1 qian). If located on the chest or cheeks, the disease is in the Shaoyang channel. Remove Angelicae Dahuricae Radix (Bai Zhi), add Bupleuri Radix (Chai Hu, 1 qian), Gentianae Radix (Dan Cao, 1 qian), and Gardeniae Fructus (Zhi Zi, 1 qian). If located on the upper back or armpits, the disease is in the Taiyin channel. Add Citri Reticulatae Pericarpium (Chen Pi), remove Citri Reticulatae Pericarpium Viride (Qing Pi), and add Platycodonis Radix (Jie Geng, 1 qian), Morus Cortex (Sang Bai Pi, 1 qian), and Asparagi Radix (Tian Dong, 1 qian). If located near the groin or genitals, the disease is in the Jueyin channel. Remove Angelicae Dahuricae Radix (Bai Zhi), double the amount of Citri Reticulatae Pericarpium Viride (Qing Pi). If located on the palms or soles, the disease is in the Shaoyin channel. Remove Angelicae Dahuricae Radix (Bai Zhi), Citri Reticulatae Pericarpium Viride (Qing Pi), and Trichosanthis Radix (Hua Fen), add Coptidis Rhizoma (Huang Lian, 1 qian), Phellodendri Cortex (Huang Bo, 1 qian), and Akebiae Caulis (Mu Tong, 1 qian).
There are nine locations where carbuncles and toxins are considered untreatable:
1. Futu (Anterior thigh muscle belly; note: original footnote incorrectly states instep, Futu is ST32 on thigh)
2. Fei (Calf muscle belly)
3. Mid-back spine
4. Jianyu (Sides of the back)
5. Neck (Dui Kou)
6. Chest
7. Beard area
8. Mustache area
9. Chin/Jaw area
**Wu Gu Kuqi (Unexplained Weeping):** If a pregnant woman suddenly weeps bitterly without cause, appearing possessed by evil spirits, this is Zang Zao (Visceral Agitation Syndrome). Treat with Shizao Tang (Ten Jujube Decoction). (Note: Standard TCM usually uses Ganmai Dazao Tang for Zang Zao; Shizao Tang is a harsh purge. Translated as per original text.)
Glycyrrhizae Radix et Rhizoma (Gan Cao, 3 liang), Triticum aestivum (Xiao Mai, 1 sheng), Zizyphi Fructus (Da Zao, 10 pieces).
Decoct in 6 sheng of water until 3 sheng remain, strain. Divide into three doses for immediate effect. Then follow with Zhuye Tang (Bamboo Leaf Decoction).
Ginseng Radix (Ren Shen, 1 qian), Ophiopogonis Radix (Mai Dong, 1 qian), Poria (Fu Ling, 1 qian), Honey-fried Glycyrrhizae Radix et Rhizoma (Zhi Gan Cao, 1 qian), Triticum aestivum (Xiao Mai, 1 he), Green Bamboo Shavings (Qing Zhu Rou, one large pill). Ginger and jujube as a guide.
These ten symptoms were not in the secret manuscript; they are now extracted from Mr. Wan Mizhai's Gynecology works to supplement deficiencies, providing learners with a clear path and preventing confusion. However, as the ancients said, physicians should not rigidly adhere to fixed formulas. It depends on the learner's ability to diagnose based on pulse and symptoms, applying wisdom flexibly, rather than being dogmatic.
For pregnant women, it is advisable to regularly take Danggui San (Angelica Powder) from *Jingui Yaolue*:
Angelicae Sinensis Radix (Dang Gui), Chuanxiong Rhizoma (Chuan Xiong), Paeoniae Radix (Shao Yao), Scutellariae Radix (Huang Qin), Atractylodis Rhizoma (Bai Zhu).
For restless fetus and prevention of miscarriage, Du Zhong Wan (Eucommia Pill) is suitable.
Eucommiae Cortex (Du Zhong, 2 liang, stir-fried with water), Dipsaci Radix (Xu Duan, 2 liang, washed with wine).
Grind into powder, make pills using paste from boiled red dates. Take 30 pills per dose with rice drink, on an empty stomach away from meals. Also take Hulian Wan.
Hulian Wan (A sacred medicine for calming the fetus):
Scutellariae Radix (Huang Qin, 4 liang, sinking in water), Atractylodis Rhizoma (Bai Zhu, 2 liang), Lotus Seed Meat (Lian Rou, heart removed, 2 liang), Amomi Fructus (Sha Ren, 1 liang, lightly fried), Honey-fried Glycyrrhizae Radix et Rhizoma (Zhi Gan Cao, 1 liang).
Grind into powder. Separately, use 5 liang of Dioscoreae Rhizoma (Shan Yao), cook into paste with rice drink, and make pills. Take with rice drink.
For postpartum hemorrhage (blood flooding) during pregnancy, use Antai Yin (Fetus-Calming Decoction) plus the following hemostatic herbs for treatment.
