For women with amenorrhea, there are three clinical presentations: First, damage to the Spleen and Stomach, reduced food intake, consumption of Qi and exhaustion of Blood leading to amenorrhea. One should tonify the Spleen and Stomach, nourish Qi and Blood, and wait for Qi and Blood to become abundant and generate, so that menstruation will occur spontaneously. Do not recklessly use emmenagogue agents, which would further damage Middle Qi and dry up Yin Blood, resulting in consumptive disease (Laozai) that is incurable. This is like demanding a thousand gold from a beggar and beating him daily, merely killing his life in vain. Second, due to worry, overthinking, anger, and resentment, causing Qi stagnation and Blood stasis leading to amenorrhea. One should open Qi stagnation and unblock Blood stasis so that menstruation will occur spontaneously. If tonifying agents are used, Qi will become more knotted upon tonification, and Blood will coagulate further, leading to masses and fullness, which is known as "nurturing a tiger and leaving trouble for oneself." Third, due to thick body fat blocking the channels and phlegm-fluids obstructing, leading to amenorrhea. One should move Qi and guide out Phlegm, allowing menstruation to flow; this is called a skilled physician.
