All difficulties in childbirth are caused by sitting to deliver too early and exerting excessive force, leading to the danger of transverse or breech presentation. Moreover, at the initial stage of umbilical and abdominal pain, since the fetal body has not yet turned properly, it is mostly unfavorable, and caution should be exercised.
1. If the fetal body is not aligned and the birth canal is not straight, when the mother exerts force to push, the fetus may present transversely or tilt to the left. Although the fetal head presses against the vaginal opening without descending, only the vertex might appear, which is actually not the true vertex but perhaps the frontal angle on either side. Exerting force again will cause delivery. If the occiput of the fetal head is lodged in the anus and the presenting vertex is misaligned, an experienced midwife should quickly and gently push the head back into alignment from the anal region, after which the mother can exert force for delivery. This relies entirely on the skill of the midwife; medical treatment is secondary. If the amniotic fluid has not ruptured and pain is severe, the condition is critical.
2. If the fetus presents with hands or feet first, apply salt to the soles of the feet and scratch them urgently. Also apply salt to the mother's abdomen. Use a small silk needle to prick the fetus's hands or feet to a depth of one to two fen, wipe the area with salt, and gently guide the limbs back inside. The fetus will contract and turn due to pain and fright, thereby correcting its position. (The purpose of acupuncture is to prevent the fetus from struggling upward due to pain, which could endanger the mother's life.)
Prescription for Transverse or Breech Presentation:
Dragon's House Earth (Fulonggan - soil from the center of the stove, preferably red from years of use.)
Grind into fine powder, mix with wine, and take one qian. This medicine is effective for cases where the fetal head presents first.
Prescription for Transverse or Inverted Presentation:
Snake Slough (one piece, complete)
Silkworm Cocoon Paper (one sheet, sealed in a new tile jar with salt mud, calcined until carbonized but retaining properties)
Grind into powder, decoct Frankincense (Ruxiang) soup, and take one qian. Delivery will occur after two doses.
Prescription for Transverse/Inverted Presentation with Hands or Feet First:
Donkey-Hide Gelatin (Ejiao - stir-fried)
Talcum (Huashi)
Abelmoschus Manihot Seeds (Huang Kui Zi)
Take four qian per dose, decoct in water and drink.
Parturition-Inducing Rabbit Brain Pills: Effective for physiological malalignment and difficult labor.
Rabbit brain marrow (collected in the twelfth lunar month)
Cardamom Seed (Mudingxiang - one qian, powdered)
Frankincense (Ruxiang - one qian, separately ground)
Moschus (Shexiang - one fen)
Mash rabbit brain and form pills the size of Chinese quince seeds. Dry in shade, seal tightly with paper, and swallow one pill with warm wine. Hold the pill in the left hand for male infants and the right hand for female infants until expelled.
Thunderbolt Life-Saving Pill: For use during labor or before delivery when eyes roll back, jaw is clenched, face is black, lips are cyanotic, and foaming occurs, indicating imminent death of mother and child.
If cheeks are slightly red, it indicates the fetus is dead while the mother lives; use this prescription urgently.
Snake Slough (in bottle)
Calcined Silkworm Cocoon Paper (one qian)
Ash of Hair (one qian)
Frankincense (Ruxiang - five fen)
Take with wine.
Buddha's Hand Powder (also known as Guixiong Decoction):
Angelica Sinensis Radix (Danggui)
Szechwan Lovage Rhizome (Chuanxiong)
Decoct in wine and water.
Parturition-Inducing Divine Powder: Must be taken only when the fetal vertex reaches the vaginal opening. (Also known as Heishen San)
Soot from the bottom of the pot (Baicaoshuang)
Angelica Dahurica Root (Xiangbaizhi)
Talcum (Huashi) (equal parts)
Grind into powder, take two qian delivered with Guixiong Decoction.
Modified Motherwort Pills: Suitable for various conditions during pregnancy.
Motherwort Herb (Yimucao - four liang)
Atractylodes Macrocephala Rhizome (Baizhu - one liang)
Scutellaria Baicalensis Root (Tiaoqin - eight fen)
Form honey pills the size of beans, take with clear soup. Taking one pill regularly during pregnancy is beneficial.
For individuals with abundant qi, add Costus Root (Muxiang) three qian; for weak individuals, dissolve with An Tai Yin (Fetal-Calming Decoction); for chest and diaphragm discomfort, add Perilla Leaf (Zisu) two qian and Tangerine Peel (Chenpi) two qian. Fine pills can also be made.
Ginseng-Enhanced Guixiong Decoction: When waters break at term but the pelvic bones do not open, causing arrest of labor, take a large dose of Ginseng-Enhanced Guixiong Decoction to boost blood and qi, allowing natural opening and delivery.
Szechwan Lovage Rhizome (Chuanxiong - four qian)
Angelica Sinensis Radix (Danggui - one liang)
Ginseng Radix (Renshen - three to four liang)
Licorice Root (Gancao - one qian)
Motherwort Herb (Yimucao - one qian)
Decoct in water, take continuously for two to three doses. If Modified Motherwort Pills are available, decoct the first four ingredients and use the soup to dissolve one pill. If there is no opening after two to three doses, add Calcined Turtle Plastron (Baiguiban) one liang (vinegar-processed) and a clump of human hair ash (from a woman who has given birth).
Modified Shenghua Decoction: Treats uterine prolapse, failure of the vaginal orifice to close, and non-opening of the pelvic bones.
Ginseng Radix (Renshen - two qian)
Atractylodes Macrocephala Rhizome (Baizhu - one qian)
Astragalus Membranaceus Root (Huangqi - one qian)
Szechwan Lovage Rhizome (Chuanxiong - one qian)
Prepared Licorice Root (Zhigancao - four fen)
Angelica Sinensis Radix (Danggui - two qian)
Cimicifuga Rhizome (Shengma - eight fen)
For uterine prolapse, add Pinellia Tuber (Banxia) eight fen and Schisandra Berry (Bei Wuwei Zi) fifteen grains to utilize sour flavor for astringency; add White Peony Root (Shaoyao) eight fen (vinegar-fried) to tonify and lift. All three conditions result from extreme deficiency of primordial qi. After delivery, if these symptoms occur, first take Shenghua Decoction to eliminate blood stasis, then take Ginseng-Enhanced Shenghua Decoction. For non-opening of pelvic bones, tonify to open; for uterine prolapse, tonify to retract; for failure of the vaginal orifice to close, astringe with sour flavors; for retained placenta, refer to Volume 6, Article 3 of Symptom 29.
