Shaoyin disease, characterized by vomiting and diarrhea, cold extremities, and severe restlessness bordering on death, is primarily treated with Wuzhuyu Tang (Evodia Decoction).
In Shanghan (Cold Damage) disease, if the pulse is floating, there is spontaneous sweating, frequent urination, mental restlessness, slight aversion to cold, and cramping of the feet, administering Guizhi Tang (Cinnamon Twig Decoction) to attack the exterior is an error. This mistake leads to jue (cold extremities), dryness in the throat, restlessness, and vomiting. In such cases, Gancao Ganjiang Tang (Licorice and Dried Ginger Decoction) should be given to restore yang qi. If the jue resolves and the feet become warm, Shaoyao Gancao Tang (Peony and Licorice Decoction) should then be administered, which will allow the legs to extend. If there is stomach disharmony manifesting as delirium (chan yu), a small amount of Tiaowei Chengqi Tang (Regulate the Stomach and Purge Downward Decoction) should be given. If excessive sweating was previously induced and moxibustion or burning needles were added, Sini Tang (Frigid Extremities Decoction) is indicated.
Question: The syndrome resembles Yangdan Syndrome (Guizhi Tang pattern), yet treatment according to standard methods exacerbates the condition, leading to cold extremities, dry throat, bilateral calf spasms, and delirium. The Master stated: "By midnight, the hands and feet will become warm, and the feet will extend." Later, this prediction came true. How was this known? Answer: The cunkou (radial artery at the wrist) pulse is floating and large. Floating indicates wind; large indicates deficiency. Wind generates slight heat; deficiency causes calf spasms. The clinical presentation resembles the Guizhi Tang pattern, so Fuzizi (Aconiti Lateralis Radix Preparata) was added to the formula, and Guizhi (Cinnamon Twig) was increased to induce sweating. Fuzizi warms the meridians because of yang collapse. With cold extremities, dry throat, restlessness, Yangming internal binding, and delirious confusion, Gancao Ganjiang Tang is administered again. By midnight, yang qi returns, and the feet become warm. Since the calves are still slightly spasmodic, Shaoyao Gancao Tang is given again, allowing the calves to extend. Using Chengqi Tang to produce slight loose stools stops the delirium. Thus, it is known that the disease can be cured.
After six or seven days of Shanghan disease, following vigorous purgation, if the cun pulse is deep and slow, extremities are cold, the lower pulses (chi position) are imperceptible, the throat is obstructed, pus and blood are expectorated, and diarrhea persists uncontrollably, this is difficult to treat. Mahuang Shengma Tang (Ephedra and Ma Huang Shen Decoction) is indicated.
After eight or nine days of Shanghan disease, following purgation, if there is chest fullness, mental irritability and fright, difficulty in urination, delirium, heaviness of the entire body, and inability to turn over, Chaihu Jia Longgu Muli Tang (Bupleurum Plus Dragon Bone and Oyster Shell Decoction) is indicated.
A pulse that is fine and rapid must not be treated with moxibustion. Fire becomes a pathogenic factor, causing vexation and rebellion, exacerbating deficiency and excess. Blood scatters within the vessels; although fire energy is subtle, its internal attacking power is strong, scorching bones and damaging sinews, making blood recovery difficult.
When the pulse is floating, resolution via sweating is appropriate. If moxibustion is used instead, the pathogen has no outlet and flourishes due to fire. The disease manifests as heaviness and numbness from the waist downward, termed Huoni (Fire Reversal).
In Shanghan disease with a floating pulse, if a physician uses fire methods to force and deplete fluids, causing yang collapse, it inevitably leads to fright, mania, and restlessness with inability to sit or lie comfortably. Guizhi Qu Shaoyao Jia Shuqi Longgu Muli Jiuni Tang (Cinnamon Twig Decoction Without Peony Plus Chinese Asarum, Dragon Bone, and Oyster Shell for Rescue from Reversal) is indicated.
