Zhang Zhongjing merely named Yin Du (Yin Toxin) as a condition primarily treated with exterior-releasing methods.

Yin Du (Yin Toxin) is characterized by a bluish complexion, body pain resembling that of being beaten with a staff, and sore throat. It is treatable within five days of onset, but becomes untreatable after seven days. The primary formula is Shengma Biejia Tang (Cimicifugae Rhizoma and Testudinis Carapax Decoction), excluding Xionghuang (Realgar) and Shujiao (Sichuan Pepper) (see the section on Yang Du for the formula details).

Wang Kentang’s examination: Although Zhang Zhongjing’s writings mention the name "Yin Du," the symptoms he described are limited to a bluish face, body pain like being struck by a staff, and sore throat; he did not describe signs of extreme Yin-cold. Furthermore, his prescribed formulas consisted only of Shengma (Cimicifugae Rhizoma), Gancao (Glycyrrhizae Radix et Rhizoma), Danggui (Angelicae Sinensis Radix), and Biejia (Testudinis Carapax et Plastrum), without using strongly warming or hot herbs. Thus, it can be understood that what Zhang Zhongjing called "Yin Du" was not a disease of Yin-cold nature, but rather an invasion of malignant, abnormal Qi from heaven and earth into the Yin meridians, hence the name "Yin Du." Later theorists assumed this referred to extreme Yin-cold conditions, also calling it Yin Du. They combined Zhang’s descriptions of bluish face, body pain, and sore throat, yet used formulas such as Fuzi San (Aconiti Lateralis Radix Preparata Powder) and Zhengyang San (Upright Yang Powder) for treatment. I believe that while extreme Yin-cold conditions can indeed be named Yin Du, this ultimately does not align with Zhang Zhongjing’s original intent. The Yin Du described by later authors is clearly distinct from that described by Zhang Zhongjing and should not be conflated. The Yin Du described by later authors arises merely from internal injury due to cold food, exposure to violent cold pathogens, overconsumption of cold-cooling medicines causing transformation, or simultaneous external and internal cold injury. It is not caused by the malignant, abnormal Qi of heaven and earth.

*Liuxue Ren* renamed Yin Du as Yin Du Shanghan (Yin Toxin Cold Damage) and advocated treating it by retreating Yin.

**Yindu Gancao Tang (Yin Toxin Licorice Decoction)**: Treats Shanghan (Cold Damage) or seasonal qi diseases where Yin Du forms within one or two days of onset, or transforms into Yin Du six to ten days after taking medicine. Symptoms include heavy body, stiff back, colicky abdominal pain, difficult throat, toxic qi attacking the heart, hardness in the epigastrium, shortness of breath, inability to breathe fully, vomiting and regurgitation, cyanotic lips, blackish complexion, cold limbs, and a deep, fine, and rapid pulse. The body feels as if beaten by a staff, with a sore throat. Treatable within five to six days; untreatable after seven days.

Ingredients: Gancao (Honey-fried), Guizhi (Bark removed), Shengma, Danggui (each 5 qian), Xionghuang (2.5 qian), Shujiao (remove closed-mouth ones, stir-fry until sweat evaporates and remove seeds, 5 qian), Biejia (roasted with butter, 1.5 liang). Chop ingredients, decoct in water. After walking for about five *li*, administer another dose. Cover with blankets to induce sweating; toxins exit via sweat and the patient recovers. If no sweat occurs, administer again.

Initial Yin Du presents with a deep, fine, and rapid pulse. Xu Shuwei stated: Yin Du originates from deficiency-cold of Kidney Qi. It may result from sexual activity or consuming cold foods followed by wind invasion; internally there is latent Yin-cold, externally there is cold invasion. Or, external cold is contracted first, followed by latent Yin-cold, resulting in both interior and exterior Yin, causing Yang Qi to fail to remain. This leads to headache, heavy waist, abdominal pain, eye pain, bodily fatigue without significant fever, cold limbs, cold sweat on the forehead and back of hands, frequent thirst and irritability, mental confusion as if losing something, or ability to walk but feeling somewhat heavy. Pulse diagnosis reveals all six pulses are deep, fine, and rapid; the Chi position is short/small; the Cun position may be imperceptible. If all six pulses are floating and large, or deep but large and not rapid, it is not a Yin syndrome. If too many cooling medicines have been taken, thirst and restlessness will worsen. For such cases, urgently administer Huanyang Tuiyin (Restore Yang and Retreat Yin) medicines to stabilize the condition. The approach is simply to tonify deficiency and harmonize Qi. Suitable formulas include Zhengyuan San (Primary Origin Powder), Tuiyin San (Retreat Yin Powder), and Wusheng San (Five Virtues Powder). Yin syndromes are generally contraindicated for sweating methods. However, if Qi is restored, the pulse is large, and fever persists, using sweating methods is permissible.

**Zhengyuan San (Primary Origin Powder)**: Treats Shanghan where one feels wind-cold invading the limbs, head, eyes, and bone joints. Administer urgently. After walking five *li*, take another dose. Take three doses consecutively; sweating will occur immediately and the condition will resolve. For Yin Du Shanghan, add 5 fen of Tuiyin San to the decoction. It is also effective for dizziness, fullness of Qi, and various heart/abdominal disorders caused by cold or food injury.

Ingredients: Mahuang (nodes removed), Chenpi, Dahuang (raw), Gancao, Ganjiang, Rougui, Shaoyao, Fuzi, Wuzhuyu, Banxia (processed, equal parts). Chop ingredients. Increase Mahuang by half; decrease Wuzhuyu by half. Grind into powder. Take 1 qian per dose with 1 cup of water, 3 slices of ginger, and 1 jujube. Decoct to 0.7 cups. Drink hot. Cover with blankets to induce sweat. Wait until sweat dries before removing coverings. Do not use Mahuang for Yin Du to avoid excessive sweating.

**Tuiyin San (Retreat Yin Powder)**: Treats Yin Du Shanghan with cold limbs, deep/fine pulse, headache, and heavy waist. Take three consecutive doses. For minor cold injuries, take 1 zi per dose mixed with Zhengyuan San, adding a pinch of salt. For Yin Du Shanghan with coughing and counterflow Qi, decoct one dose and sip slowly while warm until symptoms cease.

Ingredients: Chuanwu, Ganjiang (equal parts). Coarsely grind, stir-fry until color changes, cool, then finely grind. Take 1 qian per dose with 1 cup of water and a pinch of salt. Decoct to half a cup, strain, and drink warm.

**Wusheng San (Five Virtues Powder)**: Treats Shanghan with headache, high fever, bone/joint pain, drowsiness, fatigue, cough, nasal congestion, and loss of appetite. Also effective for Shanghan complicated by cold Qi and chronic Yin Du.

Ingredients: Baizhu (1.5 liang), Gancao, Wuweizi, Shigao (each 1 liang), Ganjiang (3.5 liang). Chop. Take 5 qian per dose with 1 cup of water and a little salt. Decoct together. If complicated by cold Qi, add ginger and jujube. For Yin Du, add a small amount of Aye (Mugwort Leaf).

**Haizang's Baixue San (White Bamboo Sap Powder)**: Treats Yin Du Shanghan with chest oppression, irritability, and cold limbs.

Ingredients: Chuanwu (processed, skin and umbilicus removed), Jiegeng, Fuzi (processed), Baizhu, Xixin (each 1 liang), Ganjiang (processed, 0.5 liang). Chop or grind. Mix with warm water and take 1 qian bi.

**Lu Zhidi's Zhengyang San (Upright Yang Powder)**: Treats Yin Du Shanghan with cold limbs, cyanotic nails, cold body, and deep/fine/micro pulse. Highly effective. Use 4-5 stalks of scallion and 4-5 old hornet nests. Burn until charred but preserved (carbonized). Grind into fine powder. Mix with scallion juice and form pills the size of quail eggs. Hold firmly in the palm, tie tightly with a handkerchief. Sweat will appear shortly. Cover with a cotton quilt. If palms become very hot, do not untie. If taking medicine, first take Shengma Tang (5 qian), 3 scallions with roots, 5 slices of ginger, 2 large cups of water. Decoct to 1 cup, strain, drink warm, cover to induce sweat.

As Yin Du deepens, nails turn blue, face turns black, pulse reaches seven beats per respiration, and is deep/fine.

Accumulation of Yin at the lower part causes slight Yang to dissipate at the upper part. Symptoms include heavy/cold limbs, worsening abdominal pain, difficult throat, epigastric distension/hardness, irritability/thirst, continuous cold sweat, occasional delirium, cyanotic nails/face, and fine pulses with seven beats per respiration or more. For these symptoms, urgently moxa Qihai (CV6) and Guanyuan (CV4) acupoints 200-300 moxa cones until limbs warm. Continue administering Jin Ye Dan (Golden Liquid Pill), Lai Su Dan, Wusheng San, Huanyang San, and Tuiyin San.

**Yunü San (Jade Maiden Powder)**:
Chuanwu (skin/umbilicus removed, soaked in cold water for 7 days, thinly sliced, dried, stored in paper bag). For patients, take 1 large qian of powder, add 1 small qian of salt, 1 cup of water. Decoct to 0.7 cups. Drink to suppress Yin Du injection, which may expel material resembling pig blood. If ineffective, take another dose.

**Huanyang San (Restore Yang Powder)**: Treats Yin Du with blue face, cold limbs, irritability, and abdominal pain.
Use sulfur powder mixed with fresh well water, 2 qian orally. After some time, chills or heat may arise; adjust dosage accordingly. Recovery occurs with sweating.

**Fuzi Huanyang San (Aconite Restore Yang Powder, from *Liangfang*)**: Treats Yin Du Shanghan with blue face, four reversals (cold limbs), umbilical/abdominal pain, icy body, and acute cold Qi attacks.
Use 2 processed Fuzi (skin/umbilicus removed), grind to fine powder. Take 3 qian bi per dose. Mix with 0.5 cup fresh ginger juice and cold wine (total 1 cup). Swallow with another cup of cold clear wine. After a short while, take another dose. When the area below the navel feels like fire and the whole body is warmly comfortable, stop.

**Liuxue Ren's Fanyin Dan (Return Yin Pill)**: Treats Yin Du Shanghan with mental irritability, headache, and cold limbs.
Ingredients: Huangqi (Sulfur, 5 liang, separately ground), Fuzi (processed, skin/umbilicus removed), Ganjiang (processed), Guixin (Cinnamon Twig Heart, each 0.5 liang), Xiaoshi (Niter, separately ground), Taiyuan Yuanjingshi (Taeyuan Crystal Stone, separately ground, each 2 liang).
Preparation: In a raw iron pot, layer half the Yuanjingshi powder, then half the Xiaoshi powder. Place Sulfur powder in the middle. Cover with remaining Xiaoshi, then top with remaining Yuanjingshi. Seal with a small bowl. Fire-roast with 3 jin of charcoal until properly heated, ensuring no smoke escapes. Grind finely like flour. Mix the three powders with the previous medicine uniformly. Bind with soft rice dough into pills the size of wuweizi (seed of Chinese bayberry). Take 15-20 pills per dose with decocted Aiye (Mugwort) tea. Frequent administration until sweating occurs. For severe cases, 30 pills. This formula is highly effective. It stops wheezing and vomiting upon entry. If symptoms persist after 3-5 doses, moxa 1 cun below the navel. Moxa continuously day and night without limit on cone count. Ensure moxa cones are not too small, or efficacy will be lacking. If limbs are cold and lower abdomen hard, moxa two lines on either side of the lower abdomen (1 cun away) plus the center line (three points total). Combine with Danggui Sini Tang (Angelica Sinensis and Cinnamon Twig Decoction) and Fanyin Dan; frequent administration is necessary for internal-external penetration. Delay leads to death. If Yin syndrome includes lower abdomen obstruction, retracted testicles, and severe abdominal pain nearing death, urgently moxa Shimen (CV5), 2 cun below the navel. Combine with Fanyin Dan and Danggui Sini Tang added with Wuzhuyu and Ginger. Avoid standard diuretic and cold-slippery medicines.

**Huoyan San (Flame Powder)**: Treats severe Shanghan complications.
Ingredients: Bo Shang Huangqi (Imported Sulfur), Hei Fuzi (Black Aconite, raw, skin/umbilicus removed), Xin La Cha (New Wax Tea, each 1 liang).
Preparation: Grind to fine powder. Mix 1 sheng of good wine with the powder in a new bowl. Dry over fire. Place on tile. Under each bowl, burn 1 fist-sized piece of cooked Ai (Mugwort). Lift tile without igniting until smoke ceases. Cool, scrape powder into porcelain box. Take 2 qian per dose with 1 cup of wine. Decoct to 0.7 cups. Flames may rise; do not be alarmed. For Yin Du Shanghan with cold limbs, deep/fine pulse, vomiting/diarrhea, five-heart irritability, chest hardness, early latent Yin preventing fluid intake, or absent pulse: Take one dose. If vomiting occurs, take another. Post-ingestion warmth indicates recovery. If pathogens descend to organs but exterior remains unresolved, high fever and surging pulse indicate appropriate use of exterior-releasing medicines. If exterior resolves, sleep ensues with sweating. If slight stagnation and solid pulse exist, use purgatives to move organ Qi. Gradually use harmonizing and spleen/stomach tonifying soups/powders. If three doses fail, the case is untreatable.

In severe Yin Du with eight beats per respiration, the pulse is felt only when pressed deeply against the bone.
Severe Yin Du symptoms resemble earlier stages but are heavier. The pulse is detectable only when pressed deeply against the bone; superficial pressure yields nothing. Beats per respiration exceed normal limits or are uncountable. At this stage, herbal medicine is difficult to effect. Moxa 300+ large moxa cones (date-sized) below the navel. If limbs do not warm, it is untreatable. If the condition stabilizes, assist with previous Sulfur and hot medicines. As Yin dissipates and Yang returns, gradually reduce hot medicines and treat harmoniously to achieve recovery.

**Liuxue Ren's Congbai Wei Fa (Scallion White Ironing Method)**: Treats Yin deficiency with Yang collapse, cold body, absent pulse, near-death breathing, unconsciousness, and Yin Jue (Cold Reversal) refractory to other treatments. Wrap scallions tightly like an arm, chop thickly, remove roots and leaves, keeping only the white part (~2 cun long). Form into flat cakes. Heat one side over fire until thoroughly hot but not burning. Apply the hot side to the patient's umbilicus and below. Use a hot iron to press the scallion cake, forcing heat into muscles. Prepare 3-4 cakes; replace when one cools. After some time, the patient should revive, limbs warm, and sweat appear, indicating recovery. Follow with Sini Tang (Four Reversals Decoction) to warm the interior.

**Haizang's Note**: For Yin syndromes refractory to all medicines, unable to swallow fluids, cold body, absent pulse, short breath, unconsciousness: The Scallion White Ironing Method is less effective than steaming with bran mixed in strong vinegar and applied in a cloth bag. The latter method is faster.

**Daijiu Tuqi Gao (Moxa-Replacing Navel Paste)**:
Ingredients: Fuzi, Malanzi (Portulaca Seeds), Chuangszi (Cnidium Fruit), Wuzhuyu, Rougui (equal parts).
Preparation: Grind to powder. Mix 1 spoon of white flour, 1 spoon of herb powder, and fresh ginger juice into a paste. Spread on paper, circle ~3 cun. Apply to Guanyuan (CV4) and Qihai (CV6) below the navel. From evening to morning, its heat equals 100 moxa cones. Also effective for lower back pain. Alternative method: Grind Dingxiang (Clove), Bibo (Long Pepper), Ganjiang, and Mushi (Oyster Shell, carbonized). Place in palm, mix with saliva to mud-like consistency. Cover the yin organ with hand until warm sweat appears.

For Yin Du Shanghan with cold limbs: Grind Wuzhuyu to powder, mix with warm wine, place in raw silk bag, and apply hot compress to soles until circulation restores. Alternatively, wash limbs with warm decocted medicines to restore connection.

**Huisheng Shengao (Life-Restoring Divine Ointment, from *Liangfang*)**: External treatment for male/female Yin Du Shanghan.
Ingredients: Musi (Oyster Shell, calcined), Ganjiang (processed, each 1 qian).
Preparation: Grind to powder. For males, mix with female saliva, warm in hands, and tightly cover the testes until sweat appears. For females, mix with male saliva, warm in hands, and tightly cover the breasts until sweat appears. Testicles and breasts are the root of male/female life, representing Kan and Li. For Yin syndrome with urinary/fecal retention or miscellaneous Yin syndromes with retention: This external method is suitable. Retention for several days is urgent; otherwise, avoid use.

**Yang Trapped within Yin**:
**Poyin Dan (Break Yin Pill)**:
Ingredients: Huangqi (Sulfur), Shuiyin (Mercury, each 1 liang), Chenpi, Qingpi (each 0.5 liang).
Preparation: Melt Sulfur in a pot. Add Mercury and beat evenly with an iron rod until no droplets remain. Pour into a black tea cup. Grind finely. Add Chenpi and Qingpi. Bind with wheat paste into pills the size of wuweizi. Take 30 pills. For irritability, take with cold salted water; for Yin syndrome, take with Aiye (Mugwort) tea for good effect.

Case: A patient initially presented with cold limbs, pounding pain below the navel, and body pain like being beaten—indicative of Yin syndrome. Urgently administered Jin Ye Dan, Poyin Dan, and Laifu Dan. Pulse became deep and slippery. Deep indicates Yin; slippery indicates Yang. Although a Yin syndrome, the appearance of a Yang pulse suggests potential for survival. As Zhang Zhongjing stated: "When Yin disease shows Yang pulse, there is hope for life." Moxa Qihai and Tiantu 100 cones. When limbs warm, Yang returns, and sweating resolves the condition.
Question: How does the combination of slippery and deep pulses indicate survival?
Answer: Zhang Zhongjing said: "Xi yan chen, ming yue hua" (Rapidly opening and sinking is called slippery). Why? Chen (deep) is pure Yin; Xi (opening/rising) is correct Yang. Their harmony creates the slippery pulse. Ancient texts describe slippery pulses as flowing and rotating, similar to rapid pulses. Zhang’s three words suffice. These terms are difficult to understand. Xi means closing/opening; "Zhang er shang he" implies rising/yang. Chen means suddenly descending; implies sinking/yin. Just as it opens and closes, it suddenly sinks. "Yan" means suddenly. Zhang’s description of the slippery pulse is precise. His words follow principles, making them hard to grasp; readers must ponder carefully.

**Pili San (Thunderbolt Powder)**: Treats Shanghan where Yin excess blocks Yang. Patients exhibit irritability/heat but lack desire to drink.
Preparation: Carbonize 1 Fuzi, grind to powder. Take as one dose, mixed with honey water. Cure achieved by forcing out cold Qi, allowing hot Qi to ascend and sweat to emerge.

**Tao Hua's Recommendations**: For Yin Du with cyanotic nails, deep/fine/rapid pulse: Sini Tang. For absent pulse: Tongmai Sini Tang. Other selectable formulas: Yindu Gancao Tang, Zhenwu Tang, Houpo Wan, Baizhu Tang, Rougui San.

Formulas for restoring Yang and retreating Yin: Pili San, Zhengyuan San, Tianxiong San, Fuzi San.

Remedy: Jin Ye Dan.

Moxa/Fumigation: For cold reversal and retracted testicles, fumigate with beans roasted in hot vinegar according to method.

Ironing/Moxa Methods:
1. Shanghan directly invading Yin meridians (True Cold) or Yin Du: Body pain like beating, colicky abdominal pain, vomiting, heaviness, unconsciousness, rigid/cold limbs, cyanotic nails/lips, inability to take oral medicine, deep/fine or absent pulse. Take a handful of scallions, remove roots/leaves, keep 3 cun of white, mash into cake. Fill umbilicus with 0.5 fen of Shexiang (Musk). Place scallion cake on umbilicus. Iron with fire, replacing 3-4 times. Once slightly conscious, pour fresh ginger juice and administer Huiyang Jiuti Tang (Restore Yang and Rescue Extremity Collapse Decoction). If still unconscious, moxa Qihai and Guanyuan 20-30 cones to transmit heat inward, force evil outward, and restore Yang. If limbs warm and sweat appears, consciousness returns (good prognosis). If limbs remain cold, no sweat, and unconscious, recovery is unlikely.

**Wu Kun's Discussion**: Why take Fuzi Tang cold for Yin Du Shanghan? This treats Yin extremity at the bottom and floating Yang at the top.
Case Study: A patient with Shanghan for 10+ days had deep/fine pulse, warm hands, cold feet, constipation, red face, vomiting/irritability/thirst, inability to tolerate medicine except for sips of cold water or pieces of watermelon. Vomiting occurred shortly after ingestion. This is internal Yin-cold forcing floating Yang/fire to gather in the chest and rush up to the throat, causing red face and vomiting.
Treatment: Use 1 large Fuzi. Wrap in dough with fresh ginger juice, bake熟, remove dough, peel Fuzi, slice into 8 pieces. Decoct with 3 qian Renshen (Ginseng) and 3 qian processed Ganjiang in 2 zhong of water to obtain 1 zhong. Immerse the decoction in cold water to cool it completely before administering. Cure achieved.
Reference to *Neijing*: "To regulate reverse cold/heat, cold and heat must be managed oppositely." Taking hot medicine cold allows the cold body to dissipate first, then the hot nature activates, resolving the disease without violating patient comfort.
Critique: Modern practitioners hesitate to use Fuzi for Yin syndrome, waiting until Yang is exhausted, which is too late. Treat according to the disease. Do not hesitate.
Regarding Jia Yin Shanghan (Clasping Yin Cold Damage): Caused by sexual activity (latent Yin inside) plus cold exposure (external cold). Both interior and exterior are Yin; Yin Qi dominates, weakening Yang. Pulse is deep, feet cold. Urgently use Renshen to strengthen pulse and tonify Yuan Qi, assisted by Fuzi to warm Kidney meridian, scatter cold, retreat Yin, and restore Yang. Without these two, how to save the patient?
Proverb: "Shanghan often kills those with deficient lower bodies." True. Xu Xueshi emphasized preserving True Qi as the root of life. Failure to assess True Qi deficiency/excess and instead aggressively clearing heat or using sweating/purgatives/cold medicines leads to worsening Yin-cold and critical danger.

Another Formula: Drink rooster blood dropped into non-ash hot wine. Cover with blankets to induce sweat.
