[Yin Swelling]
[Dai] When women experience vulvar swelling, it is caused by deficiency and damage combined with the invasion of wind pathogen. The collaterals of the uterus are deficient, allowing wind pathogen to invade; the wind qi attacks the yin region, clashing with blood and qi, causing qi stagnation and obstruction. The couli (skin pores) become congested and closed, preventing dispersion, thus resulting in swelling.
[Xue] If the aforementioned symptoms belong to Qi and Blood deficiency, use Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction) to lift and tonify. If it is Damp-Heat in the Liver channel, use Longdan Xiegan Tang (Gentian Liver-Draining Decoction) to drain and clear. If there is Yin swelling, itching, coldness, or prolapse (Yin Ting), refer to the subsequent discussion for cross-reference. A woman presented with swelling and stifling sensation in the yin, astringent and stagnant urination, swelling in both hypochondria, internal heat with afternoon tidal fever, irregular menstruation, and alternating chills and fever. This was due to Liver-Spleen depression and anger, sinking of Yuan Qi, and congestion of Damp-Heat. In the morning, she took Guipi Tang (Restore the Spleen Decoction) added with Chai Hu (Bupleuri Radix) and Sheng Ma (Cimicifugae Rhizoma) to resolve depression, tonify Spleen Qi, and raise Yuan Qi. In the evening, she took Jiawei Xiaoyao San (Modified Free and Easy Wanderer Powder) to clear Liver Fire, generate Liver Blood, and eliminate Damp-Heat. After several doses of each, all symptoms resolved. Subsequently, she took Sijunzi Tang (Four Gentlemen Decoction) added with Xiong Gui (Ligustici Chuanxiong Rhizoma), Dang Gui (Angelicae Sinensis Radix), and Dan Pi (Moutan Cortex) to regulate and tonify the Liver and Spleen, restoring regular menstruation.

[Yin Itching]
[Dai] Vulvar itching in women is caused by parasitic worms. The "Three Worms" reside between the intestines and stomach. Due to Zang-Fu deficiency, the worms become active and erode the interior of the yin. The worms generate heat; mild cases cause itching, while severe cases cause pain.
[Xue] The aforementioned symptoms originate from the Liver channel. Treat internally and externally with Longdan Xiegan Tang (Gentian Liver-Draining Decoction) and Xiaoyao San (Free and Easy Wanderer Powder). Crush Tao Ren (Persicae Semen) into a paste, mix with Xiong Huang (Realgar) powder or chicken liver, and insert into the vagina to control the worms. A woman presented with chest and diaphragm discomfort, internal heat with thirst, poor appetite, limb fatigue, stifling and itching in the vagina, and red, astringent urination. This was caused by depression and anger injuring the Liver and Spleen. She recovered after taking Guipi Tang (Restore the Spleen Decoction) added with Shan Zhi (Gardeniae Fructus). Later, due to anger, pain extended to the lower abdomen. She took Xiao Chai Hu Tang (Minor Bupleurum Decoction) added with Shan Zhi, Xiong Gui (Ligustici Chuanxiong Rhizoma), Dang Gui (Angelicae Sinensis Radix), and Bai Shao (Paeoniae Radix Alba); pain ceased. She recovered after taking Jiawei Xiaoyao San (Modified Free and Easy Wanderer Powder) added with Shan Zhi. Later, due to overwork, swelling occurred at the lesion site with persistent astringent urination. She recovered after taking Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction) added with Shan Zhi (Gardeniae Fructus), Fu Ling (Poria), and Dan Pi (Moutan Cortex). A woman presented with internal vulvar pain and itching, intermittent discharge, poor appetite, and fatigue. This was due to Liver-Spleen Qi deficiency and downward pouring of Damp-Heat. She stabilized after taking Guipi Tang (Restore the Spleen Decoction) added with Dan Pi (Moutan Cortex), Shan Zhi (Gardeniae Fructus), Bai Shao (Paeoniae Radix Alba), and raw Gan Cao (Glycyrrhizae Radix et Rhizoma). A woman presented with internal vulvar itching and pain, internal heat, fatigue, and poor appetite. This was due to Liver-Spleen depression and anger, depletion of Yuan Qi, and Damp-Heat. She recovered after taking Ren Shen (Ginseng Radix), [missing character, likely Huang Qi], Dang Gui (Angelicae Sinensis Radix), Bai Zhu (Atractylodis Macrocephalae Rhizoma), Chen Pi (Citri Reticulatae Pericarpium), Chai Hu (Bupleuri Radix), fried Shan Zhi (Gardeniae Fructus), Che Qian Zi (Plantaginis Semen), Sheng Ma (Cimicifugae Rhizoma), Bai Shao (Paeoniae Radix Alba), Dan Pi (Moutan Cortex), and Fu Ling (Poria). If itching and pain in the yin are caused by worms, it also belongs to Liver Wood pathology. Crush Tao Ren (Persicae Semen) and Xiong Huang (Realgar) and insert into the vagina to kill worms, while still using herbs to clear the Liver and resolve depression. Some use chicken liver insertion as a method to expel worms. One formula involves crushing fresh peach leaves, wrapping in cotton, inserting into the vagina, and changing two to three times daily.
Da Huang San (Rhubarb Powder) treats vulvar itching in women.
Da Huang (Rhei Radix et Rhizoma) (slightly stir-fried), Huang Qin (Scutellariae Radix), Huang [missing character, likely Huang Qi], (stir-fried with honey, each 1 liang), Chi Shao Yao (Paeoniae Radix Rubra), Xuan Shen (Scrophulariae Radix), Dan Shen (Salviae Miltiorrhizae Radix et Rhizoma), Shan Zhu Yu (Corni Fructus), She Chuang Zi (Cnidii Fructus) (each 0.5 liang).
Grind into fine powder. Take 2 qian before meals, mixed with warm wine.
Guangji Formula treats persistent vulvar itching in women.
Ran She Dan (Boa Gallbladder), Xiong Huang (Realgar), Liu Huang (Sulfur), Zhu Sha (Cinnabaris), Xiao Shi (Nitrumsulfatum), Wu Yi (Pruni Semen) (each 0.5 liang), Li Lu (Veratri Radix) (0.25 liang).
Grind into fine powder and mix evenly. Combine with pig fat from the twelfth lunar month to form a paste. Make a bandage like a finger, 1.5 cun long, apply medicine, and insert into the vagina. Change once daily. When changing, wash with pig pepper root (3-5 liang) boiled in slightly hot water, dry, and insert for better effect.
Another Formula: Use Xiao Ji (Cirsii Herba) in any amount, boil in water to make a decoction, and wash hotly three times daily.
Cui Shi's Formula for unbearable vulvar itching.
Burn Xing Ren (Armeniacae Semen Amarum) to ash. Wrap in cotton while hot and insert into the vagina. Change twice daily.
Another Formula: Wash with garlic boiled in water. Another method uses Gou Qi Gen (Lycii Radix Cortex).
Another Formula: Ye Lang Ya (Pyrethri Tenuifolii Radix) (2 liang, finely cut), She Chuang Zi (Cnidii Fructus) (3 liang). Boil in 3 sheng of water for ten boils. Take hot.
Another Formula: Insert hot chicken liver into the vagina. If worms are present, they will be expelled.
Another Formula: Cut cow liver into 5-cun pieces, tie with a string, and insert into the vagina. After half a day, worms enter the liver; remove it. Pig liver also works.
Shenghui Fang treats vulvar itching and pain caused by worms, swollen eyes, yellow skin, desire for men, leukorrhea with bleeding, shortness of breath, and craving for delicious food. Use a 1-chi carp, remove head meat, grind bones into powder, fry until yellow-black, mix with pig fat, place in a silk bag, and insert into the vagina as usual. Stop when reaching the painful area; worms will exit naturally.
[Yin Pain]
Qianjin Formula for persistent "small household marriage pain" (dyspareunia/vulvar pain).
Gan Cao (Glycyrrhizae Radix et Rhizoma), Sheng Jiang (Zingiberis Rhizoma Recens) (each 0.3 fen), Bai Shao Yao (Paeoniae Radix Alba), Gui Xin (Cinnamomi Ramulus) (each 0.2 fen).
Finely cut. Boil in 2 sheng of wine for three boils. Strain and take warm. Effectively miraculous.

Hai Piao Xiao San (Cuttlefish Bone Powder) for female "small household marriage pain".
Wouyu Gu (Sepiae Endoconcha) (2 pieces).
One ingredient, burn and grind into fine powder. Take with wine using a square-inch spoon measure, three times daily.
Buyi treats vulvar swelling not descending and "small household marriage pain".
Dong Qing Ye (Ilicis Cornuti Folium), Xiao Mai (Tritici Semen), Gan Cao (Glycyrrhizae Radix et Rhizoma).
Equal parts, decoct in water for washing.
Wei Shi's Formula treats female vulvar pain. Stir-fry Qing Yan (Halitum) until hot, wrap in cloth, and apply as a hot compress.
[Yin Ulcer]
[Dai] Vulvar ulcers in women are caused by the activity and erosion of the Three Worms or Nine Worms. Various worms reside between the intestines and stomach. If Zang-Fu are harmonious and Qi-Blood abundant, they cannot cause harm. If meridians are injured by labor and intestines/stomach are deficient, worms become active and erode the yin. Mild cases cause itching or pain; severe cases cause ulcers. Palpating a slippery and rapid pulse in the Shaoyin channel indicates vulvar ulceration.
[Xue] These symptoms result from emotional depression transforming into fire, damaging the Liver and Spleen, with downward pouring of Damp-Heat. External signs include growths in the vagina resembling snakes, mushrooms, or cockscombs, or sores with damp itching, ulceration with discharge, or swelling with stifling pain. Internal signs include fatigue, internal heat, irregular menstruation, loss of taste, afternoon tidal fever, chest/hypochondriac discomfort, urinary distension, red/white leukorrhea, and dysuria. Treatment: For swelling/pain, use Siwu Tang (Four Substances Decoction) added with Chai Hu (Bupleuri Radix), Shan Zhi (Gardeniae Fructus), Dan Pi (Moutan Cortex), and Dan Cao (Gentianae Radix).
