[Lou Quanshan] Postpartum extreme panting is critical and often fatal.
[Guo Jizhong] What is the cause of rapid breathing and panting in the throat after childbirth? The answer is: Ying (Nutritive) Qi is blood, Wei (Defensive) Qi is air. Ying Qi circulates within the vessels, while Wei Qi circulates outside the vessels. They move up and down together, referred to as Ying and Wei. Due to excessive blood loss during delivery, Ying blood suddenly exhausts, causing Wei Qi to lose its master and gather exclusively in the lungs, thus inducing panting. This is named "Lone Yang and Absolute Yin" and is difficult to treat. If lochia (postpartum discharge) does not flow smoothly, and stagnant blood coagulates, steaming upward to the lungs, it also causes urgent panting; simply administering Duoming Dan (for retained placenta) will stop the panting once the blood is expelled.
Chen Wuyue commented: Postpartum urgent panting is indeed fearsome. If caused by stagnant blood steaming upward to the lungs, one may still expect efficacy from Duoming Dan. However, if caused by contracting wind-cold, or due to emotional distress/anger, or consuming salty/cold foods and drinks, Duoming Dan is not universally applicable, let alone for cases of Lone Yang and Absolute Yin. If Ying blood is suddenly exhausted, a large dose of Zhuxiong Tang can also be life-saving. For wind-cold injury, Xuanfuhua Tang is suitable. For emotional depression stagnation manifesting outwardly, Xiaotiaojing San is suitable, taken with decoction of Sangbaipi (Morus Albae Cortex) and Xingren (Prunus Armeniacae Semen). For food injury, Jian Shi Wan, Wuji San, and Xionglao Tang are suitable.
[Xue Ji] For the aforementioned conditions, if Spleen and Lung Qi are deficient, use Liujunzi Tang (Six Gentlemen Decoction) with Jiegeng (Platycodonis Radix). If accompanied by external pathogens, add Zisu (Perilla Folium). If Middle Qi is deficient and cold, use Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction) with Paojiang (Prepared Zingiberis Rhizoma) and Rougui (Cinnamomi Cortex). If Yang Qi is deficient and collapsing, add Fuzi (Aconiti Lateralis Radix Praeparata). If stasis blood enters the lungs, urgently use Erwei Rensu Yin (Two-Ingredient Ginseng and Perilla Decoction). One parturient presented with urgent panting, spontaneous sweating, and cold hands and feet, frequently protecting her umbilical and abdominal area with her hands. This indicated Yang Qi deficiency and collapse; she was cured after four doses of Shenfu Tang (Ginseng and Aconite Decoction). The wife of Wu Hui from Pujiang had swollen feet during pregnancy. In early July, two months postpartum, after bathing, she immediately developed panting, being able only to sit and not lie down for five months. She feared wind but felt slightly relieved by warmth. Her Guan pulses were pulsating, while Cun and Chi pulses were both deficient; all medicines failed. Using Mudanpi (Moutan Cortex), Taoren (Persicae Semen), Guizhi (Cinnamomi Ramulus), Fuling (Poria), Ganjiang (Zingiberis Rhizoma), Zhishi (Aurantii Fructus Immaturus), Houpo (Magnoliae Officinalis Cortex), Sangbaipi (Morus Albae Cortex), Zisu (Perilla Folium), Wuweizi (Schisandrae Chinensis Fructus), and Gualouren (Trichosanthis Semen), decocted and taken, she felt immediate relief. After two or three doses, she could sleep, and her phlegm disappeared. This was treated as stasis blood combined with cold invasion.
Xuejie San (Dragon's Blood Powder) treats postpartum stagnant blood rushing to the heart, chest fullness, upper panting, and imminent death; it is suitable for administration.
True Xuejie (Dragon's Blood) (If unavailable, replace with Zikuang [Sappan Wood]) Moyao (Myrrha)
Equal parts of the above, ground into fine powder, repeatedly ground until completely fine. Take two qian per dose, mixed with child's urine and good wine (half a large cup), boiled once, and taken warm. Immediately after birth, take one dose then go to bed; take another dose after a long interval. The stagnant blood will naturally descend along the meridians without rushing upward, preventing various diseases.
Yunqi Rensu Yin (Yunqi's Ginseng and Perilla Decoction) treats postpartum blood entering the lungs, black complexion, and panting approaching death.
Renshen (Ginseng Radix et Rhizoma) (1 liang, ground to powder) Sumu (Sappan Wood)
Take two bowls of water, boil the Sumu to obtain less than one bowl of liquid, remove dregs, and mix in the Renshen powder. Adjust dosage as needed; the effect is miraculous.
Xuanfuhua Tang (Inula Flower Decoction) treats postpartum wind-cold injury, panting, cough, abundant phlegm-saliva, and restlessness in sitting or lying.
Xuanfuhua (Inulae Flos) Chishao (Paeoniae Radix Rubra) Jingjiesui (Schizonepetae Spica) Banxiaqu (Pinelliae Fermentatum) Qianhu (Peucedani Radix) Gancao (Licorice Root, honey-fried) Fuling (Poria) Wuweizi (Schisandrae Chinensis Fructus) Xingren (Prunus Armeniacae Semen, peeled and tip removed, bran-fried) Mahuang (Ephedrae Herba) (All equal parts). In another prescription, Shaoyao and Fuling are doubled.
Cut into pieces. Take 4 qian per dose, with 1.5 cups of water, 3 slices of Shengjiang (Fresh Ginger), and 1 jujube. Decoct to 0.7 portion, remove dregs, and take warm before meals. (Not suitable for those with sweating.)
For panting caused by consuming salty, frozen drinks or food, Jian Shi Wan is suitable. (See previous section on Chest Oppression.)
Wuweizi Tang (Schisandra Decoction) treats postpartum urgent panting, hidden pulse, and cold limbs.
Wuweizi (Schisandrae Chinensis Fructus, pounded and fried) Renshen (Ginseng Radix et Rhizoma) Xingren (Prunus Armeniacae Semen) (Each 2 qian) Maimendong (Ophiopogonis Radix, core removed) Chenpi (Citri Reticulatae Pericarpium) (Each 1 qian)
Add 3 slices of ginger and 2 jujubes, decoct in water, and take.
Dabu Tang (Great Tonifying Decoction) treats postpartum cases beyond 100 days, presenting with bluish-green facial swelling, pale lips, urgent breathing with sweating, indicating severe deficiency; this should be administered urgently.
Dangguiwei (Angelicae Sinensis Radix Head) Dachuanxiong (Ligustici Chuanxiong Rhizoma) Dabaizhu (Atractylodis Macrocephalae Rhizoma) Baishaoyao (Paeoniae Radix Alba) Bail茯苓 (Poria, use large amount) Renshen (Ginseng Radix et Rhizoma, use large amount) Huangqi (Astragali Membranacei Radix, use large amount) Wuweizi (Schisandrae Chinensis Fructus) Shudihuang (Rehmanniae Radix Preparata) Ganjiang (Zingiberis Rhizoma, moderate amount) Gancao (Licorice Root, small amount)
Cut into pieces, decoct in water, and take. If symptoms do not recede after two doses, add Chuanwu (Aconiti Radix) and Muxiang (Aucklandiae Radix), separately ground into powder and added to the decoction. If diarrhea occurs, add Hezi (Terminalia Chebulae Fructus), Roudoukou (Myristicae Semen), and Susuke (Papaveris Capsulum).
