The Kidney governs the waist and legs. In women, the Kidney is connected to the uterus via the Bao Luo (uterine network). If labor or injury damages the Kidney Qi, leading to weakness, and Wind-Cold pathogenic factors invade the Bao Luo, the pathogenic factors contend with the healthy Qi, resulting in pain in the waist and legs. Treatment should primarily focus on tonifying the Yuan Qi (original Qi), assisted by formulas that dispel pathogens.
[Xue Ji's Commentary] For the aforementioned conditions, if True Yang is exhausted and Cold Evil attacks, presenting with cold hands and feet, headache, aversion to cold, or vomiting and abdominal pain, it is appropriate to use the formula mentioned in this context (referring to warming yang and dispersing cold formulas). If Qi and Blood are deficient and affected by Cold Evil, presenting with aversion to cold, fever, headache, thirst, or vomiting and abdominal pain, Wuji San (Five Accumulations Powder) is appropriate. (From Miscellaneous Diseases: Cold.) If Yuan Qi is deficient and injured by Damp-Heat, presenting with swelling and pain in both shins, alternating chills and fever, body aches, or vomiting and inability to eat, Bingsu Baidu San (Areca and Perilla Detoxification Powder) is appropriate. (From Miscellaneous Diseases: Injury by Dampness.) If Spleen and Stomach are weak, with Yuan Qi sinking, presenting with alternating chills and fever, vomiting, feverish headache, panting and thirst, and physical fatigue, Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction) is appropriate. (From Miscellaneous Diseases: Injury by Fatigue.) If the Essence and Blood of the Three Yin Meridities of the Foot are depleted, with Deficient Fire stirring internally, presenting with internal heat, afternoon tidal fever, thirst with profuse phlegm, and frequent urination, Liuwei Dihuang Wan (Six-Ingredient Pill with Rehmannia) is appropriate. (From Miscellaneous Diseases: Consumption and Weakness.) If the Yang Qi of the Three Yin Meridities of the Foot is exhausted, presenting with aversion to cold, fever, cold hands and feet, expectoration, inability to eat, and slippery/frequent defecation and urination, Bawei Dihuang Wan (Eight-Ingredient Pill with Rehmannia) is appropriate. (From Miscellaneous Diseases: Consumption and Weakness.)
A woman first experienced pain in her lower back and hips, followed by pain in both legs. I considered this to be deficiency-cold of the Three Yin Meridities of the Foot, injured by external pathogens. She was treated with Xiao Xuming Tang (Minor Xu Ming Decoction) and Duhuo Jisheng Tang (Angelica Pubescens and Porcinus Decoction). Symptoms came and went; she felt comfortable in her abdomen only when consuming very hot food. I stated: The pathogen has been expelled, but the Yuan Qi is deficient and cold. Her pulse was indeed deep and fine. She was administered Yangshen San (Kidney-Nourishing Powder) [see this section], gradually recovering, and subsequently cured with Shibu Wan (Ten Tonics Pill) [see Consumption and Weakness].
Another woman suffered from similar symptoms but presented with fever and thirst, preferring cold drinks and food. Her pulse was surging and rapid, but slow and choppy upon deep pressure. I diagnosed this as Blood deficiency with Heat. She was treated with Lingyangjiao San (Antelope Horn Powder) [see Body Pain], removing Areca Nut, and adding Atractylodes Macrocephala Rhizome and Poria, for several doses. She was then treated with Jiawei Xiaoyao San (Modified Free and Easy Wanderer Powder) [see Tubercles] and recovered.
A third woman suffered from the aforementioned condition, with occasional pain in the legs and knees. Her pulse was floating and rapid, but slow and sluggish upon deep pressure. This indicated Yuan Qi deficiency with invasion by Wind-Damp. She was instantly cured by Duhuo Jisheng Tang and stabilized with Bazhen Tang (Eight-Treasure Decoction).
A fourth woman developed the aforementioned condition due to anger, presenting with alternating chills and fever, bitter taste in the mouth, inability to eat, afternoon tidal fever, and internal heat. I diagnosed this as Liver Fire with Blood deficiency. She was instantly cured by Xiaochaihu Tang (Minor Bupleurum Decoction) added with Gardenia Fruit, and subsequently cured with Jiawei Xiaoyao San.
A fifth woman suffered from the aforementioned condition, presenting with chills, fever, and headache, resembling Taiyang Disease (Exterior Syndrome). This was a Cold Evil syndrome. She was stabilized by Bingsu Baidu San and cured by regulating with Buzhong Yiqi Tang.
