The *Waitai Mi Yao* treats infants with unstable breastfeeding, high fever for seven days or more, red facial complexion, and yellow nostrils, fearing the development of *cuokou* (mouth contraction) and trismus. The worms in the mouth resemble snails or yellow-headed white clam shells. Administer half a he of bamboo sap (*Zhuli*) mixed with a small amount of realgar (*Niuhuang*), which leads to recovery. Additionally, wiping the mouth with pork meat causes the worms to exit.
Chao Yuanfang states: Sudden aggregation in an infant's mouth, forming millet-sized growths on the tongue that prevent suckling, is named "Jin" (Lockjaw), caused by fetal heat entering the zang organs, resulting in excessive heat in the heart.
The *Taiping Shenghui Fang* advises guarding against three diseases in newborns: one is *cuokou*, two is *zhejin* (fixed lockjaw), and three is *umbilical wind*. All are acute conditions, with *zhejin* being the most severe, lasting until after one month to be avoided. Symptoms include tight jaw, unstable feeding, gradually diminishing cries, and drooling (Mao notes closed eyes). Observers seeing normal urination and defecation may attribute it to cold or heat pathogens, not realizing the pathology lies in the throat and tongue, where the condition is extremely serious. Skilled practitioners save fewer than three out of ten cases. If preventive care were taken beforehand, such misfortune would certainly be avoided. Upon appearance of this syndrome, urgently inspect the infant's upper palate for spots; first gently scrape them open with fingernails, then administer formulas such as *Dingming San* or *Chensha Quanxie San*. If trismus persists and other medicines fail, take raw *Arisaema Cum Bile* (*Sheng Nantianxing*) peeled and de-umbilicated, grind into ultra-fine powder, mix with a little borneol (*Longnao*), dip a finger in ginger juice, and rub the roots of the large teeth; the mouth will open immediately. Although *Umbilical Wind*, *Cuokou*, and *Jinfeng* differ in presentation, their root cause is identical: generally, interior qi stagnation and blockage. It is suitable to administer decocted soybean paste juice for drinking to expel fetal toxins.
The *Qianjin Fang* states: Newborns have vigorous qi; if minor ailments occur, purgatives must be used promptly. If not purged in time, they become major illnesses difficult to treat. Use *Zishuang Wan* according to dosage.
*Dingming San*: Treats newborn trismus preventing suckling.
*Cicada Slough* (*Chantui*) (14 pieces, remove head and feet) *Scorpio* (*Quanxie*) (27 pieces, remove venom)
Grind into ultra-fine powder, add a little calomel (*Qingfen*), and mix. Dissolve and administer using breast milk.
*Chensha Quanxie San*: Treats newborn trismus.
*Cinnabar* (*Chensha*) (water-washed, half qian) *Scorpio* (*Quanxie*) (head and tail intact, de-venomed, 20 pieces, roasted) *Borax* (*Pengsha*) *Borneol* (*Longnao*) *Musk* (*Shexiang*) (each 1 zi)
Grind into ultra-fine powder. Apply inside lips and on teeth using mother's saliva, or mix with a little pig milk and introduce into the mouth.
*Zishuang Wan* (Formula seen in Bianzheng section.)
[Zhang Huan] *Lisheng San*: Treats newborn trismus and inability to suckle caused by the mother consuming too many hot-natured drugs during pregnancy or eating roasted/fried hot foods, leading to sores like millet grains on the tongue and mouth, with frequent crying, named "Kou Jin".
*Dried Spider* (*Gan Zhizhu*) (1 piece, remove mouth and feet, roast over new bamboo fire to extract oil approx. one shell volume, soak overnight, roast until charred, take powder) *Dried Scorpion* (*Gan Xieshao*) (7 pieces, powdered) *Calomel* (*Nifen*) (powdered, 1 qian)
Grind all together into ultra-fine powder. Take 1 zi per dose, mix with breast milk, and drip into the mouth occasionally.
The *Qianjin Fang* treats trismus: Red indicates Heart Trismus; White indicates Lung Trismus.
Chicken manure white (date-sized), wrapped in cotton, boiled in one he of water for two boils, divided into two doses. (*Shenghui Fang* uses bean-sized 3 pieces, taken with water.)
Alternatively, four sparrow droppings, powdered, applied to nipple for infant to suck. For older infants, use ten pieces.
The *Waitai Mi Yao* treats *Zhejin*, pathology in the pharynx, obstruction size of hemp/mustard seed, causing infant to drool and refuse milk.
Mercury (*Shuiyin*), size of millet grain, administered orally; heals upon swallowing. Measure by intent, not exceeding small sesame size. (Use only in emergencies.)
*Shiliao Bencao*: Crush tea berries (*Chamei*) for juice, pour into mouth; can revive even if dead. (Refers to plant with wet stem end, three leaves, yellow flowers, red fruit resembling raspberry.)
*Taiping Shenghui Fang*: Sichuan pepper (*Chuanjiao*) 1 liang, kneaded into dough wrapped in triangular wonton shape, roasted until yellow and cooked. Cover infant's mouth with cotton, trim tip to chopstick-head size, allowing pepper aroma to enter mouth. Effective; repeat if no effect.
Wormwood Formula: Treats pediatric trismus unable to suckle.
Red-legged centipede (*Chijiao Wugong*) (half strip, remove feet, roast until charred, powder) Musk (*Shexiang*) (small amount)
Mix with pig milk. Pig milk alone can treat pediatric trismus.
Spider Formula: Treats pediatric trismus unable to suckle.
Spider (1 piece, remove feet and mouth, roast until charred, fine powder)
Mix with one he of pig milk, divide into three doses, slowly灌 (pour/administer). Miraculous effect. Adding musk is excellent for treating dental gangrene.
Cow Saliva Formula:
Take saliva from cattle walking eastward, apply to infant's mouth and forehead; effective immediately.
Method for Treating Trismus/Twitching via Nasal Insufflation:
Use *Curcuma Aromatica* (*Yujin*), *Veratrum* (*Lilu*), *Melon Seed* (*Gua Di*), powdered, mixed with water, insufflated into nose.
Mr. Mao states: Infants contracting Wind Trismus within 120 days of birth (same pathology as *Zhejin*) result from maternal disease during conception leading to toxin exposure, unregulated blood/qi at birth, and attack by wind pathogen. Treatment: First administer *Duoming San* to vomit wind-phlegm (see Ji Man Jing Men), then administer *Xingpi San* (see Wei Qi Bu He Men / Man Pi Feng Men) combined with *Jia Yunqi San* (see Wei Qi Bu He Men), and *Xiongzhu San* (see Chi Bai Li) combined with *Zhusha Gao* (see Ji Jing Men). Regular administration leads to cure. If hands are clenched and mouth locked shut, death ensues.
