[Xue Ji] The Qi of all Yang meridians in the human body converges at the head and gathers at the face. If sores or ulcers occur here, it is due to disharmony of the Zang-Fu organs and stagnation of Qi and Blood in the Yang meridians. Etiologies may include congenital constitution leading to Kidney Yin deficiency with Liver Fire hyperactivity; exposure to fetal toxins from the mother; the wet nurse suffering from the Six Exogenous Pathogens (Six Evils) or Seven Emotions; the wet nurse consuming rich, greasy foods and strong wine; or the infant ingesting sweet, fatty, and heavy flavors. Although the causes differ, one must differentiate the affected meridians and examine the specific etiology to treat accordingly.
If the sore appears at the outer canthus (Rui Zi), anterior to the ear, upper cheek, extending to the nose and inner canthus, it belongs to the Small Intestine Meridian.
If the sore appears on the vertex, temporal region, lower cheek, posterior to the ear, and both sides of the occiput, it belongs to the Gallbladder Meridian.
If the sore appears on the anterior cheek, around the nostrils, and on both sides of the Renzhong point, it belongs to the Large Intestine Meridian.
If the sore appears on both sides of the nostrils, lower lip, Chengjiang point, Jiache point posterior to the ear, hairline anterior to the ear, and forehead/skull, it belongs to the Stomach Meridian.
If the sore appears at the inner canthus, upper frontal eminence, extending to the posterior occiput and neck, it belongs to the Bladder Meridian.
Having identified the meridian, treatment should be differentiated by meridian. For cases due to innate Kidney Fire, use Liuwei Dihuang Wan (Kidney). For cases due to fetal toxins, use Xijiao Xiaodu Wan (see previous section). For cases due to food accumulation and Gan syndrome, use Siwei Fei'er Wan (Gan Syndrome). For cases due to the wet nurse's consumption of rich foods, use Dongyuan Qingwei San (Dental diseases). Since diseases of the Fu organs inevitably involve the Zang organs:
Sores on the center of the forehead belong to the Heart Meridian. If accompanied by fever and preference for cold drinks, it is Excess Heat; use Daochi San (Heart). If accompanied by fever and preference for hot drinks, it is Deficiency Heat; use Yangxin Tang.
The left cheek belongs to the Liver Meridian. If accompanied by stiffness of the neck, it is Excess Heat; use Chaihu Qinggan San (Heat). If accompanied by teeth grinding and irritability, it is Deficiency Heat; use Liuwei Dihuang Wan.
The right cheek belongs to the Lung Meridian. If accompanied by wheezing, coughing, and preference for cold drinks, it is Excess Heat; use Xiebai San (Lung). If accompanied by fever and coughing, it is "Fire Punishing Metal" (Heart Fire insulting Lung Metal); use Renshen Pingfei San (Cough).
The area around the nose belongs to the Stomach Meridian. If accompanied by fever, preference for cold drinks, and hard yellow stool, it is Excess Heat; use Xiehuang San (Spleen). If accompanied by fever, preference for hot drinks, and greenish-white stool, it is Deficiency Heat; use Yigong San.
Sores on the chin and earlobes belong to the Bladder Meridian. The Kidney has no Excess patterns; only Dihuang Wan is used.
If the ulcer has ruptured and fails to heal for a long time, one must examine the dominance of Zang organ Qi and the transmission of pathogenic factors, prioritizing the regulation and tonification of the Spleen and Stomach. If the disease is caused by residual heat from the wet nurse, treating the mother first will allow the infant's disease to heal spontaneously.
Yangxin Tang: Treats sores and ulcers caused by Heart Qi deficiency with Deficiency Heat attacking upward.

Grind each separately into powder. Dust dry onto the lesion. Or mix with oil for application. For scabies, add Gufan (Calcined Alumen) 3 qian.
[Head Sores]
Pediatric head sores are caused by the Six Yang meridians being attacked by toxic heat, forming sores on the head. If the head presents with scattered patches, chronic dryness and itching, sparse hair, and scales resembling dandruff, this is due to accumulated heat attacking upward, named Tuo Chuang (Baldness Sores). Although the sore is on the head, people only know to apply external medicine for cure, but it recurs within a month. Why? Because the head is the convergence of all Yang meridians. In the *Hongfan* Five Elements, Fire flares upward. Toxic heat attacks upward, two Yangs scorch each other, hence sores appear on the head. The method should resolve old accumulated heat and guide away Heart Meridian irritability.
[Worldly Doctors] Treat chronic Tuo Chuang: Apply fine Huanglian powder externally. Internally, take Kushen Wan after meals; externally, apply Kushen powder mixed with oil. Cure in two months.
[Danyang] Another prescription: Use Fangfeng Tongsheng San processed with wine. Remove Dahuang, grind separately into powder, moisten with wine, dry, and grind again. Take 1 qian, decoct in water, 4-5 times daily. Effective after 30 doses.
