This refers to bone-steaming fever caused by Gan-heat (nutritional deficiency fever), accompanied by various symptoms such as coughing and night sweats. Tang Shi Bie Jia San (Tang's Turtle Shell Powder) and Zhu Du Wan (Pig Stomach Pills) are both appropriate remedies for these conditions.
Gan Di Huang Jian (Dried Rehmannia Decoction) [Source: *Lu Xun Jing*]: Treats pediatric Gan-lao (chronic nutritional deficiency with wasting), pulmonary heat-induced cough, gradual emaciation of the four limbs, and dry heart pulse.
Ingredients: Fresh Rehmanniae Radix Juice 5 liang; Su (Clarified Butter), Sheng Jiang Zhi (Fresh Ginger Juice), Mi (Honey) each 1 liang; Lu Jiao Jiao (Deer Horn Gelatin) 0.5 liang.
Preparation and Administration: First, place the Fresh Rehmanniae Radix Juice in a pan. Simmer over low heat while stirring continuously with your hand. After approximately five or six boils, add the Su. After another five or six boils, add the Mi. Then add the Lu Jiao Jiao, followed by the Sheng Jiang Zhi. Continue simmering over low heat until the consistency resembles thin syrup, then remove from heat. Administer one spoonful in two doses after meals. Avoid all toxic or irritating foods.
[Mr. Wang] Ji Rou Jian Wan (Chicken Meat Fried Pills): Treats children over ten years old suffering from Gan-lao with high fever and emaciation.
Ingredients: Xuan Lian (Coptidis Rhizoma, stamens removed) 2 liang; Yin Chai Hu (Stellariae Radix, rhizome removed, washed); Qin Jiao (Gentianae Macrophyllae Radix, soil removed and cleaned); Zhi Mu (Anemarrhenae Rhizoma); Shi Jun Zi (Quisqualis Fructus, flesh only); Zi Qin (Scutellariae Radix) each 1 liang; Wu Yi (Cantharii Fructus, skin removed); Chuan He Shi (Necleae Semen) each 0.5 liang.
Preparation and Administration: Grind the above ingredients into fine powder. Take one yellow hen weighing about one jin. Keep it in a cage and feed it exclusively with hemp seeds for five days. Then slaughter it, remove feathers thoroughly, make an incision at the rear buttocks to remove intestines and viscera, wash clean, and dry. Place the powdered herbs inside the chicken cavity and sew it shut with thread. Take a small steamer, line the bottom with black beans three cun thick, place the chicken in the steamer, surround it with black beans on all sides, and cover the top with black beans. Steam from sunrise until evening. When cool, remove the chicken, discard the herbs and bones/tendons/wings from the cavity, and grind the clean meat evenly. If too dry, add wine; otherwise, use flour paste to form pills the size of hemp seeds or small mung beans. Take 10, 15, or 20 pills per dose, adjusting according to circumstances. Swallow with prepared Ophiopogonis Decoction on an empty stomach or before bed. For pediatric Gan-lao with bone-heat in children over fifteen, swallow with warm wine. Avoid pork.
[Zhang Shi] San He Yin Zi (Three-Harmony Infusion): Treats obstruction in the San Jiao (Triple Burner), stagnation in the five Zang organs, rebellious Qi with phlegm-saliva, nausea and vomiting of saliva after eating rice, headache and dizziness in the Taiyang region, as well as mountainous miasma (shān lán zhàng qì) causing vomiting, poor appetite, sallow complexion, and cyanotic-black fingernails. It also treats pediatric Gan-lao with milk vomiting, and in adults and children who have recovered briefly from prolonged illness but lack spiritual vitality, experiencing alternating chills and fever. It provides rescue treatment for all these conditions.
Ingredients: Zi Tuan Ren Shen (Purple Cluster Ginseng Radix et Rhizoma) 3.5 liang (washed, sliced); Gan Cao (Licorice Radix et Rhizoma) 1.5 liang (honey-fried, sliced); Mian Huang Qi (Astragali Radix) 5 liang (soaked in wine overnight, washed, sliced).
Preparation and Administration: Place the three ingredients together in a wooden mortar and pound with a wooden pestle into a coarse powder. Take three qian per dose, with three slices of fresh ginger, two cups of water, and three jujubes. Decoct until reduced to 80%, strain, and drink. Take at any time.
Xi Jiao San (Rhinoceros Horn Powder): Treats pediatric bone-heat and detoxifies.
Ingredients: Yin Zhou Chai Hu (Bupleuri Radix from Yinzhou); Chuan Da Huang (Rhei Radix et Rhizoma); Gan Cao (Licorice Radix et Rhizoma, honey-fried); Chuan Xiong (Ligustici Chuanxiong Rhizoma); Fu Ling (Poria); Shao Yao (Paeoniae Radix); Ge Gen (Puerariae Lobatae Radix); Sang Bai Pi (Mori Cortex); Di Gu Pi (Lycii Cortex); Shan Zhi Zi Ren (Gardeniae Semen); Huang Qin (Scutellariae Radix); Bei Mu (Fritillariae Cirrhosae Bulbus) each 0.5 liang.
Preparation and Administration: Grind into fine powder. Take one qian per dose, with one cup of water, one branch of Qing Hao (Artemisiae Annuae Herba), and ten wheat grains. Decoct until reduced to 70%, and take warm. If the condition is severe, add Ma Huang (Ephedrae Herba) and Lian Qiao (Forsythiae Fructus) in equal amounts to the previous formula, grind into powder, and decoct for administration. (Zhu Shi uses this formula for the twenty-first syndrome.)
Die Xue Jian (Turtle Blood Decoction): Treats Gan-lao.
Ingredients: Wu Yi (Cantharii Fructus); Chai Hu (Bupleuri Radix); Chuan Xiong (Ligustici Chuanxiong Rhizoma) each 1 liang; Ren Shen (Ginseng Radix et Rhizoma) 0.5 liang; Shi Jun Zi (Quisqualis Fructus) 21 pieces (shelled); Hu Huang Lian (Coptidis Rhizoma); Xuan Huang Lian (Coptidis Rhizoma) each 7 qian.
Preparation and Administration: Soak the two types of Huang Lian in one cup of Die Xue (Turtle Blood) and 1 liang of Wu Zhu Yu (Evodiae Fructus) overnight. The next morning, stir-fry until dry, then discard the Wu Zhu Yu and turtle blood. Mix the two types of Huang Lian with the remaining herb powders, use millet flour paste to form pills the size of hemp seeds. Take before meals with warm water.
