Yang Shi stated: Pi Gan (Spleen Malnutrition) is caused by irregular breastfeeding and feeding, which injures the Spleen and Stomach. It may also result from the nursing mother indulging in raw, cold, fatty, and greasy foods, or from overfeeding the infant causing injury. If milk is given immediately after a meal, it may lead to vomiting. Excessive milk intake combined with prolonged sleep can transform into "Ru Pi" (Milk Lump), characterized by masses in the abdomen and hypochondrium, which is also referred to as "Nai Gan" (Milk Malnutrition). External manifestations include sallow complexion, body heat, abdominal distension with weak lower limbs, vomiting and regurgitation, chest and epigastric fullness, fatigue and crying, inability to digest food and fluids, sour-smelling diarrhea, sleeping with face down, reduced appetite, and pica (eating dirt). This constitutes Pi Gan. Qian Yi advocated using Yihuang San (Benefit Yellow Powder) for treatment. Yang Shiyang advocated using Lingzhi Wan (Trogopterus Dung Pill) combined with Yihuang San. Xue Ji advocated using Siwei Fei'er Wan (Four-Ingredient Fat-Nourishing Pill) to treat malnutrition, and Wuwei Yigong San (Five-Ingredient Benefit-Work Powder) to tonify the Earth (Spleen). Zeng Shirong advocated regulating the Spleen using Shenling Baizhu San (Ginseng, Poria, and Atractylodes Macrocephala Powder) for cases of anorexia.
