〔活血〕
[Exterior Syndrome]
Pulse is floating and tight, symptoms manifest as fever, aversion to cold, and body pain. This indicates an invasion of external pathogens, and dispersing methods are appropriate. For injuries requiring dispersal in the four seasons: in Spring, use Wuji San (Five Accumulations Powder) or Xiangsu San (Fragrant Perilla Powder); in Summer, use Xiangru Yin (Elsholtzia Decoction) or Wuling San (Five-Ling Powder); in Autumn, use Zhengqi San (Righteous Qi Powder); in Winter, use Hejie San (Harmonizing Decoction) with modifications. (All the above formulas are also found in Treatise on Cold Pathogenic Diseases.) If accompanied by alternating chills and fever, add Bupleuri Radix (Chaihu), Peucedani Radix (Qianhu), and Scutellariae Radix (Huangqin). For headache, add Ligustici Chuanxiong Rhizoma (Chuanxiong) and Angelicae Dahuricae Radix (Baizhi). For Hongkong foot (swelling and pain in the feet), add Baizhi, Arecae Semen (Binglang), and Aucklandiae Radix (Muxiang). For phlegm, add Arisaematis Rhizoma (Nanxing), Pinelliae Rhizoma (Banxia), and Mume Fructus (Wumei). For asthma/wheezing, add Ginseng Radix et Rhizoma (Renshen), Muxiang, and Aquilariae Lignum Resinatum (Chenxiang). For cold, add Atractylodis Rhizoma (Cangzhu), Banxia, and Citri Reticulatae Pericarpium (Chenpi).
The above herbs are divided equally, cut into pieces. Decoct with scallion white parts, take on an empty stomach.
Shufeng Baidu San (Dispel Wind and Overcome Toxin Powder) treats various injuries from beating and falling, tendon movements, bone fractures, and bruises from falls or collisions.
Angelicae Sinensis Radix (Danggui), Ligustici Chuanxiong Rhizoma (Chuanxiong), Paeoniae Radix Alba (Baishaoyao), Rehmanniae Radix Praeparata (Shudihuang), Notopterygii Rhizoma et Radix (Qianghuo), Angelicae Pubescentis Radix (Duhuo), Platycodonis Radix (Jiegeng), Aurantii Fructus Immaturus (Zhike), Bupleuri Radix (Chaihu), Poria (Baifuling), Baizhi, Glycyrrhizae Radix et Rhizoma (Gancao), Perillae Folium (Zisu), Chenpi, Cyperi Rhizoma (Xiangfu).
Decoct the above with fresh ginger and Rehmanniae Radix Recens (Shengdihuang), mix with wine, and take.
Jiawei Jiajia San (Modified Jiajia Powder) treats injuries from beating and falling, bone fractures and dislocations, fever, and aversion to cold. It is suitable for weak individuals. For robust individuals, Shufeng Baidu San is preferred.
Danggui, Chuanxiong, Baishaoyao, Shengdihuang, Cangzhu, Magnoliae Officinalis Cortex (Nongpo), Chenpi, Baifuling, Banxia, Qianghuo, Duhuo, Jiegeng, Zhike, Qianhu, Chaihu, Zingiberis Rhizoma Recens (Ganjiang), Cinnamomi Cortex (Rougui), Gancao.
Decoct with fresh ginger. If there is heat, remove Ganjiang and Rougui.
Qianghuo Ruxiang Tang (Notopterygium and Frankincense Decoction) treats injuries from falls and blows, tendon movements, bone fractures, fever, body pain, and cases complicated by external pathogens.
Qianghuo, Duhuo, Chuanxiong, Danggui, Paeoniae Radix Rubra (Chishaoyao), Saposhnikoviae Radix (Fangfeng), Schizonepetae Spica (Jingjie), Moutan Cortex (Danpi), Dipsaci Radix (Xuduan), Carthami Flos (Honghua), Persicae Semen (Taoren), Chenpi.
Decoct the above with Shengdihuang. If there is heat, add Chaihu and Huangqin.
[Interior Syndrome]
If the Liver pulse is firm and long, and pain under the ribs is unbearable, it is appropriate to move blood stasis. Haicang said: If one falls from a height, is struck by heavy objects, or injured by arrowheads or blades, causing stagnant and accumulated blood in the heart, abdomen, or chest that does not dissipate, divide it according to the Upper, Middle, and Lower Jiao, distinguishing the specific locations. For the Upper Jiao, use Yilao Xijiao Dihuang Tang; for the Middle Jiao, use Tao Ren Cheng Qi Tang; for the Lower Jiao, use Didang Tang or similar formulas to purge. Some treat it by decocting with urine and wine. Other methods include decocting with Shengdihuang and Danggui, or including Dahuang. Another method: for deficient patients who cannot tolerate purging, decoct Siwu Tang with Manis Squama (Chuanshanjia) for excellent results. Huarui Shi San (Flower Rhyolite Powder) can also be used, decocted with child's urine or taken with wine. This medicine contrasts with the preceding cold-natured medicines in terms of Yin and Yang, so it must be distinguished carefully. Once the blood stasis is removed, Fuyuan Tongqi San with added Danggui can be decocted and taken.
Zhang Zhongjing treated horse falls and all kinds of bone and tendon injuries.
Dahuang (1 liang, sliced, soaked in hot water, or 0.5 liang), Feibo (red silk fabric), Luankua (tangled hair, size of a chicken egg, burned to ash), Baipuxi (decayed cattail mat, 3 cun), Jiuyong Chuandanbu (cloth used for cooking for a long time, 1 chi, burned to ash), Taoren (49 kernels, skin and tip removed), Gancao (size of middle finger joint, roasted and cut).
These seven ingredients. Decoct with an appropriate amount of child's urine to make a soup, or add one large cup of wine. Add Dahuang next, strain. Divide into three warm doses. First, cut half a Baipuxi into pieces, decoct for bathing, cover with clothes/bedding. After a short while, bowel movement will occur several times. Recovery follows immediately after evacuation. Do not be surprised if the bathwater turns red; this is blood stasis.
Fuyuan Huoxue Tang (Restore Origin and Invigorate Blood Decoction) treats falling from heights, where evil blood flows into the hypochondrium, causing unbearable pain. The Classic states: "Falling causes evil blood to remain inside; great anger causes qi to rise and fail to descend, damaging the liver when it affects the hypochondrium." The Liver and Gallbladder meridians both run through the hypochondrium, belonging to Jueyin and Shaoyang. Therefore, Chaihu should be used as the guiding herb and sovereign drug; Danggui invigorates blood vessels; since urgency causes pain, Gancao relieves urgency and promotes new blood generation (Yang generates Yin), serving as the minister drug; Chuanshanjia, Trichosanthis Radix (Gualuagen), Taoren, and Honghua break and moisten blood stasis, serving as assistant drugs; Dahuang processed with wine sweeps away bad blood, serving as the envoy drug. Their flavors combine, each returning to its meridian, and pain naturally disappears.
Chaihu (5 qian), Danggui, Chuanshanjia (processed), Gualuagen (each 3 qian), Gancao, Honghua (each 2 qian), Taoren (skin and tip removed, 50 kernels), Dahuang (soaked in wine, 1 liang).
Crush the Taoren until paste-like. Cut the remaining herbs like hemp seeds. Take 1 liang per dose with 2 cups of water and half a cup of wine, decoct to 70%, strain. Take warm before meals, stopping when bowel movement occurs. If pain persists after evacuation, take Ruxiang Shenyingsan. (Formula see Abdominal Pain section later.)
Baji Tang (Jiegu) treats falling from heights, internal injuries from blows, dizziness, lethargy, and inability to eat, known as blood closure. Viscera and bowels are blocked.
Bajitian (heart removed), Dahuang (each 0.5 liang), Danggui, Dihuang, Shaoyao, Chuanxiong (each 1 liang).
Grind into powder. Decoct with water, stop when bowel movement occurs.
Danggui Daozhi San (Dongyuan) treats injuries from blows, blood stasis from falling off horses or carts, constipation, redness/swelling/blue-black discoloration, painful dizziness. Internal accumulation of blood stasis leading to near death.
Dahuang (1 liang), Danggui (2.5 qian), Shexiang (a little).
Grind the three ingredients (except Shexiang which is ground separately) into fine powder, mix evenly with Shexiang. Take 3 qian per dose, mixed with one cup of hot wine.
Once internal blood stasis is cleared, or if bone/joint fracture pain is unbearable, treat with Dingtong Jiegu Zijin Dan.
Another Daozhi San treats compression injuries, falling from heights, uncontrollable hematemesis, internal blood stasis causing chest/abdominal fullness, panting, and shortness of breath.
Danggui, Dahuang (each 2 liang).
Grind into fine powder. Take 3 qian per dose at any time, mixed with warm wine.
Duoming San (Jisheng Fang) treats blade wounds, falling from heights, crushing by wood/stone, blood stasis accumulating in the heart/abdomen causing pain, and urinary/fecal retention.
Shuizhi (mixed with Duanshi, slow-roasted until dry and yellow, 0.5 liang), Heiqianniu (2 liang).
Grind into powder. Take 2 qian per dose with hot wine, walk about 4-5 li. Then take another 2 qian of Heiqianniu powder with hot wine to induce expulsion. Must pass clots of evil blood until complete.
Jiming San (Sanyin Fang) treats falling from heights, crushing by wood/stone, any injury where blood stasis accumulates, qi failure approaching death, irritability, headache, inability to cry out. This medicine effectively purges blood stasis and miraculously treats fractures.
Dahuang (1 liang, steamed with wine), Taoren (14 kernels, skin and tip removed).
Grind finely. Decoct in one bowl of wine until reduced to 60%, strain. Take at cockcrow. Pass blood stasis the next day and recover. If sudden qi failure and inability to speak occurs, and medication cannot be obtained in time, forcibly open the mouth and灌 (pour) hot urine immediately for recovery.
Qingshang Yuxue Tang (Clear Upper Blood Stasis Decoction) treats injuries to the upper diaphragm/chest.
Qianghuo, Duhuo, Lianqiao, Jiegeng, Zhike, Chishaoyao, Danggui, Zhizi, Huangqin, Gancao, Chuanxiong, Taoren, Honghua, Sumu, Dahuang.
Decoct with Shengdihuang, mix with aged wine and child's urine, and take.
Xiaxia Posui Tang (Eliminate Lower Break Blood Decoction) treats injuries to the lower diaphragm/abdomen.
Chaihu, Chuanxiong, Dahuang, Chishaoyao, Danggui, Huangqin, Wulingzhi, Taoren, Zhishi, Zhizi, Chi Niuxi, Mutong, Zelan, Honghua, Sumu.
Decoct with Shengdihuang, mix with aged wine and child's urine, and take.
Dazijin Pi San (Great Purple Gold Bark Powder) treats beating/falling injuries, fractures, and internal damage to lungs/liver.
Zijinpi, Jiangzhenxiang, Buguzhi, Wumingyi (roasted, quenched in red wine 7 times), Chuanxuduan, Hupo (ground separately), Niuxi (soaked in wine overnight), Taoren (skin removed, roasted), Danggui (washed, dried), Puhuang (each 1 liang), Dahuang (wrapped in wet paper, baked), Pomiao (ground separately, each 1.5 liang).
Grind into fine powder. Take 2 qian per dose before meals, decoct strongly with Sumu and Danggui in wine, and take.
Posui Yao (Blood-Breaking Medicine) treats beating/falling off horses, falling from heights where skin/flesh are intact. This indicates blood stasis accumulating internally attacking inward, causing inability to speak and delirium. Purging is prioritized. If skin is broken and bleeding, treat as metal wound with excessive blood loss.
Chaihu, Huangqin, Wulingzhi, Zhishi, Danggui, Chishaoyao, Chuanxiong, Shengdihuang, Dahuang, Pomiao, Taoren, Honghua, Sumu.
Decoct with water, mix with wine and child's urine, and take. If skin is broken and bleeding, do not use wine.
Luo's Huarui Shi San treats all metal blade/arrowhead wounds, beating/falling injuries, cat/dog bites, or death due to blood stasis at the injury site. Apply powder to the wound; blood turns to yellow fluid. Reapply powder, and the patient revives without pain. For internal damage where blood enters viscera, decoct child's urine with a little wine, mix one large cup, and take for immediate effect. If intestine protrudes from cow butt but is undamaged, quickly push it back in, suture the abdomen using fine thread or mulberry bark, apply powder on the suture, stop bleeding, and revive immediately. If no mulberry bark, raw hemp thread works. Do not seal the wound tightly to avoid pus formation. If wound is dry, moisten with saliva before applying powder. For postpartum women with incomplete lochia, blood confusion/fainting, evil blood rushing to heart, dead fetus in uterus, or retained placenta leading to death, if the heart area feels warm/broad, quickly mix one cup with child's urine, pass evil matter resembling liver slices, and never suffer from blood wind/qi syndromes again. If blood on the diaphragm turns to yellow fluid, vomit it up immediately or pass it with stool.
Shiliuhuang (4 liang), Huarui Shi (2 liang).
Mix evenly. Line a clay pot with paper paste and salt mud, place herbs inside, seal with mud, dry. Place on brick, write Bagua/Five Elements characters, surround with charcoal, ignite from bottom during Si/Wu hours, burn overnight until cool. Remove, grind finely, sieve through silk, store in porcelain box. Use according to method.
Posui Xiaotong Tang (Dongyuan) treats horse riding injuries, spinal fracture, downward flow of evil blood, severe hypochondriac pain, inability to turn, affecting diet.
